July 2016 Dear Surgical Supplier: As President of the Southern California Chapter of the American College of Surgeons, I invite you to participate in our annual scientific meeting at the Four Seasons Biltmore, Santa Barbara, California, on Friday, January 20 through Sunday, January 22, 2017. As the largest chapter of the College, our membership numbers over 2,000 and includes surgeons in both clinical practice as well as those in academic medical centers in Southern California. Surgeons from many specialties attend the meeting, including General Surgery, Cardiothoracic Surgery, Head and Neck Surgery, Trauma, Colo-Rectal, Oncology, Bariatric Surgery, Plastic Surgery, Orthopedic Surgery, Pediatric Surgery, and Vascular Surgery. As in past years, members of other chapters of the College are also invited to the meeting, giving an average attendance of well over 300 surgeons. At the suggestion of past exhibitors, the exhibit area will be open from 7:00 am 4:00 pm on Friday and Saturday. The $2,000 participation fee for exhibitors includes one table top exhibit space strategically placed near the lecture room to provide maximum exposure to all meeting participants. (Take advantage of the Early Bird rate of $1,750 for payments received by September 30th!) The Poster Session of scientific presentations will be held adjacent to the exhibit area and all co ee and refreshment breaks will be in the exhibit area. And, we provide sandwiches and drinks in the exhibit area during the open lunch hours on Friday and Saturday. Exhibitor company name, address, and telephone numbers will be printed in the official program. Companies electing to support a particular social function or specialty section with an additional donation will receive special recognition in the program and from the podium (see the enclosed list of opportunities). In addition, all supporters have the opportunity to have one piece of Chapter approved company sales material inserted in each delegate s registration packet. Southern California Chapter American College of Surgeons EXHIBITOR AGREEMENT THIS AGREEMENT IS BETWEEN the Southern California Chapter, American College of Surgeons hereafter known as CHAPTER, and (COMPANY) hereafter known as EXHIBITOR. EXHIBITOR agrees to exhibit at the Annual Scientific Meeting of the CHAPTER to take place January 20-22, 2017, at the Four Seasons, Biltmore in Santa Barbara, California. EXHIBITOR will be provided a table top exhibit area upon which EXHIBITOR may display and/or demonstrate products throughout the hours of the Annual Scientific Meeting. The Exhibit hours will be 7:00 am 4:00 pm on January 20th and 21st. EXHIBITOR will be provided one draped 6 table and two chairs. EXHIBITOR agrees to provide CHAPTER payment of $1,750 by September 30, 2016 to secure the Early Bird exhibit rate. Or, if after September 30, EXHIBITOR agrees to provide CHAPTER with payment of $2,000 if paid by December 15. A fee of $2,250 is required for agreements received after December 15, 2016. CHAPTER agrees to acknowledge EXHIBITOR in the Program if Agreement and full payment is received by December 15, 2016, by printing the company name, address and telephone number. In addition, CHAPTER agrees to furnish EXHIBITOR two tickets to the Friday Evening Reception at no cost to EXHIBITOR. Additional tickets to one or more social functions are available at the same charge as for all meeting participants. The Four Seasons Biltmore prohibits posting of any kind on columns, walls, floors or other parts of the building or furniture without prior written consent of The Four Seasons Biltmore. Repair of any damages in connection with the improper posting or use of improper materials by EXHIBITOR will be at the sole expense of EXHIBITOR. EXHIBITOR assumes entire responsibility and hereby agrees to protect, indemnify, defend and save CHAPTER, and The Four Seasons Biltmore and its employees and agents harmless against all claims, losses and damages to persons and property, governmental charges or fines and attorney s fees arising out of or caused by EXHIBITORS installation, removal, maintenance, occupancy or use of the exhibition premises or a part thereof, excluding any such liability caused by the sole negligence of The Four Seasons Biltmore and its employees and agents. In addition, EXHIBITOR acknowledges that CHAPTER, and The Four Seasons Biltmore, do not maintain insurance covering EXHIBITOR S property in that it is the sole responsibility of EXHIBITOR to obtain business interruption and/or property damage insurance covering such losses by EXHIBITOR.
