Boulevard Ave. Dept. 301 Bismarck, ND 58505-0200 2011-2012 North Dakota Cancer Coalition Community Grant Program FINAL REPORT Project Title: Wellness Works-Lifelong Health Applicant Organization: Rolette County Public Health District (RCPHD) Project site(s): Rolette County employees of Microlap Technologies, Inc. This company manufactures and modifies high-precision industrial components from synthetic ruby, sapphire, ceramic and other extremely hard materials. This company has a work force of approximately 50 employees. In 2008 the employees of Microlap Relay for Life Team raised $12,700 for Rolette County towards a fight for the cure for cancer. Target Audience(s): 47 Employees of Microlap Technology, 25% are men employees, 75% are women employees, 40% of the members are Native American, 55% are Caucasian, and 4% chose not to indicate ethnicity. This information was collected from the Personal Wellness Profile each employee filled out at the start of the work site wellness program. Employees of Microlap spend the majority of their day at a microscope or with a small highly precise measuring instrument. Their job requires them to be sitting for most of the day. Project Goal(s): #1: To increase the frequency of physical activity and nutrition habits among the Wellness Works-Lifelong Health program. #2: To increase the number of participants who eat 5 or more servings of a variety of vegetables and fruits each day. #3: To decrease the number of program participants who use tobacco products. #4: To increase the number of participants who complete age and gender preventive health exams. Project Partners and their role: Four members of the RCPHD staff assisted on the day of data collection ; obtaining weight, height, waist and girth measurements and known fat percentage, blood pressures and heart rate, blood sample obtained for cholesterol, hemoglobin s and blood sugars,performed lung function test, and strengthening percentages. Participants had been asked to fast for 12hours prior to blood testing.

The following are the presenters for the wellness program lunch and learn: Wade Burgess LPT from Presentation Medical Center presentation on Developing a Healthy Lifestyle and Becoming More Physically Active. Kimberly Samuelson, Zumba instructor from Presentation Medical Center had the participants involved in Zumba activities and invited participants to join classes. Karen Armstrong NDSU Home Economist Rolette County Extension Office, presentation on Choosing Healthy Fats, Choosing Healthy Carbohydrates, Healthy Sodium Intake, and Achieve/Maintain a Healthy Weight. Judy Martinson RCPHD Tobacco Coordinator presentation on ND Quit, what tobacco does to your teeth and what is in a cigarette, also handed out quit and spit kits. Dr. Greg Volk Clinical Psychologist from Volk Human Services Center Devils Lake gave a presentation on Enhancing the Mind/Body Connection and Coping with Stress. Dr. Volk also gave a session of free Yoga to the participants. Martha Williams PA from Northland Community Clinic presentation on Getting Preventive Check Ups yearly. She spoke on Blood Pressure, Cholesterol, Blood Glucose, Breast Cancer, Cervical Cancer, Prostate Cancer, Osteoporosis, STD s, Hearing/Vision/Dental Exams and safety practices such as seat belts. Project costs: NDCC: $5,000.00 Inkind: $8,032.07 Total: $13,032.07 Timeframe: Planning steps started February 1, 2012 until April 1, 2012. Implementation steps started April 1, 2012 until May 23, 2012. Evaluation steps started May 23, 2012 until June 8, 2012. Steps to complete project: Planning steps: The determination for need of this project was data collected from the Rolette County Health Rankings 2011 North Dakota. Rolette County is ranked number 40 out of 42 counties for how healthy the county is and 42 out of 42 in the health factor what influences the health of the county. The Wellness Works-Lifelong Health project was discussed with Microlap Technologies, Rolla, ND they have approximately 50 employees and 50% of these employees do not have health insurance. Implementation steps: *Strategy; 30 minute Wellness Walk 2 times per week, Microlap Administration verifies that 42-53% of the program participants 2

are participating in a workplace walking program as a result of the Wellness Works-Lifelong Health program. *Strategy; Participants will take part in the Try It sessions, 20 program participants have taken part in the classes offered for Zumba, Yoga and Body Language Fitness. *Strategy; participants will partake in the Lunch and Learn opportunities as a mechanism of motivation and follow the Personal Wellness Logs provided on a weekly basis. 85-95% of the Microlap employees attended the 4 Lunch and Learn presentation that were offered. Every other week opposite of the Lunch and Learn a newsletter was delivered to each participant at Microlap at their Wednesday employee meeting with each participant receiving a total of 4 newsletters. Cancer Awareness/Prevention Display Boards were set up regarding Breast Cancer/Women s Way, Colorectal Cancer, Lung Cancer, and Skin Cancer along with handout materials for the participants to pickup. The displays were up and changed during the time of April 1 to May 23. Table Tents were also changed at intervals regarding the topics of Women s Way, Breast Cancer, Colorectal Cancer, Prostate Cancer, and Skin Cancer during this same time period. 45% of the Microlap participants verbalized increased intake of fruits and vegetables as they are actively participating in a Biggest Loser Contest within the company. The post Personal Wellness Profile Comprehensive Assessment has been postponed until September 2012 as the Microlap administration has requested this for an incentive to keep the employees active in the changed lifestyle they have started. The success of this strategy will be determined at that time. *Strategy; provide information and onsite referral to ND Quitline/Quitnet throughout the 12 week program; Assist the participants in completion of the request for counseling process. *Promote the ND Quitline/Quitnet as the primary source of tobacco cessation support for all tobacco users desiring to make a quit attempt.*monitor ongoing use of the Quitline/Quitnet to evaluate the use of the service. Post program Personal Wellness Profile Comprehensive Assessment has been postponed until September 2012 per Microlap Administration request. The success of this strategy will be determined at that time. The Lunch and Learn held on May 9 th included how to access and the function of the ND Quitline/Quitnet. From the time period of April 2012 to May 2012 Quitnet use increased by 20% and Quitline participation in Rolette County increased by 15%. At this Lunch and Learn 9 Spit Kits and 21 Quit Kits were requested and distributed by Rolette County Tobacco Prevention staff to participants of the program. 3

* Strategy; provide onsite completion of stool quiac testing. Assist in scheduling of age and gender preventive exams; hemoglobin, glucose, cholesterol and urine testing will be offered at reduced charge as part of the Individualized Wellness Profile; 96% of Microlap participants completed Lipid Panel/Glucose and urinalysis and the remaining 4% brought in their own lab results from their provider. 27% of the participants over the age of 50 years asked for the 3 FIT test for stool quiac and completed the test. Participants were provided the literature regarding the Colonoscopy Screening program at the Heart of America Medical Center in Rugby, ND. 21% of the women over the age of 40 years were assisted in scheduling mammograms locally with 16% completed by 06/08/2012 and the remaining 5% are scheduled for a later date in June 2012. An additional 26% of women over the age of 40 years were referred to Women s Way Program per Deb Quiring at Lake Region District Health, Devils Lake, ND. Evaluation steps: One of the tool used to evaluate the success of our project was the Personal Wellness Profile Comprehensive Assessment a measurement of one s current health status. This is a tool published by Wellsource Inc. Health Assessment and Prevention Systems. The participants used signup for the Wellness Walk, the Try It sessions, and the Lunch and Learn programs. The Actual level of success were at the level projected or above. We projected 50% to participate in the Wellness Walks and 53% are participating. 25 participants were projected for the Try It sessions and 20 took part in the sessions. 50% were projected to participate in the Lunch and Learn and 85-95% participated. 100% of the newsletters were received as they were personally handed to each participant. Microlap administration has a list of the participants in the Biggest Loser Contest and this is at 45% of the program participants, part of this program is increasing fruits and vegetables and decreasing fats and sugars. In regards to the measurement of the Quitline/Quitnet, comparison of the nutritional data and comparison of the preventive exam data this will be obtained when the post Personal Wellness Profile Comprehensive Assessment is completed in September per the request of the Microlap administration. 96% of the participants already completed the Lipid/Glucose and urinalysis, 27% of the participants completed the 3 FIT stool test., 16% of 21% of the participants have completed scheduled mammograms and 5% will complete later in June, this data is from the technicians at Presentation Medical Center where mammograms were performed. We are waiting on the schedule dates for the 26% of women referrals sent to Women s Way at Lake Region District Health, Devils Lake, ND. 4

Materials used or developed: Personal Wellness Profile Comprehensive Assessment by Wellsource Inc. (Health Assessment and Prevention Systems) pre and post questionnaire used to accumulate data. Mammogram Survey (attachment) developed by Linda Warner RN and the Women s Way Referral form from NDDH. Lunch and Learn Evaluations developed by Barbara Frydenlund RN. Post Presentation Survey developed by Barbara Frydenlund RN. Also used were the attendance documentation sheets at each Lunch and Learn and Try It sessions. The Monthly ND Quitline/Quitnet use report and the completion of the Personal Wellness Profile Comprehensive Assessment will be used to track the number that quit tobacco and are using the Quitline/Quitnet. Outcome(s): When the team members approached Microlap Technologies to participate in the Wellness Works-Lifelong Health project we knew they had approximately 50 employees and when the email came to us that 47 of their employees would like to participate we were over whelmed with pride. The outcome of this project has gone beyond what the team members anticipated. The Microlap employees have commented on their evaluations, I learned more about good food and bad food and how to stay healthy and maintain a healthy weight then I thought I ever would from just these few weeks of these girls coming to our work place. Another employee comments, It is a great reminder to not take good health for granted and what you need to do to maintain that good health. Good information on the Yoga, I will be trying this class. Great information about smoking and what is in a cigarette and the help that is available. Great program great job. Microlap administration has commented to team members that they see a real difference in their employees, they are bringing healthier lunches to work,they are outside walking on their breaks, some have cut back on their smoking instead of smoking every break they will take a walk, thus overall the employees are more health conscious. Lessons Learned: The program became more time involved than anticipated thus requiring more man power to complete the project on time. The team members did learn on a positive note how needed the information for health preventive education was needed. We also were amazed at the number of participants that have barriers to accessing care. 5

Recommendations: The enthusiasm of Microlap administration was very positive and thus the great turn out of number of participants in the program. The presenters for each Lunch and Learn were eager to tell their story on prevention; we were not turned down by anyone that was asked. Those that were asked for the Try It Sessions were eager to give of their time and talents. Team members decided that if the project were to be run again it would run over a longer period of time. We would also collaborate with other agencies to offset a portion of the cost of the program. Next Steps: Microlap is interested in repeating the program in 2013, but to repeat the program would be a cost to the employer unless additional funding would be obtained.
