There are several factors that may contribute to a couple s inability to conceive, and the resulting infertility may be attributed to a number of possible causes in the female partner, the male partner, or in some cases both. The cause of infertility may often be complex. Your completing the Self-assessment form will help us to evaluate your condition so that your treatment can be started as soon as possible. Our Clinic We are located at 2110 Décarie Boulevard in Montreal, between de Maisonneuve and Sherbrooke streets. We are a 5 minute walk from the Vendome metro station on the orange line. We are also served by many bus lines: 24, 17, 102 and 105. Our opening hours are Monday to Friday from 7:30 am to 18:00 pm. We are closed on weekends. There is also parking available for our patients next to the building. Self-assessment form In order to be officially registered as a patient at OriginElle, you must complete the selfassessment form. In addition to your basic contact information, this form will also provide the doctor and IVF coordinator with a better understanding of your circumstances and medical history. Please send your completed form by post, by fax to 514-369-0188 or by e-mail to patientinfo@originelle.com, as soon as possible so that we can make an appointment for your first consultation with the doctor. If you need help filling out the self-assessment form, one of our coordinators will be available to assist you. Please call our clinic to talk with a coordinator. Previous medical records If you have been a patient at another fertility clinic or have undergone any previous treatment associated with assisted reproductive therapy, we ask that you contact the other clinic directly and have them send us your medical records by fax at 514-369-0188 as soon as possible. Each clinic has their own policy on transferring patient records so keep in mind that for certain centers there may be a delay of up to several weeks before we receive your file. It is necessary for us to receive the records before your initial consultation with the doctor, as this will help him to decide the most appropriate course of treatment. At the end of this document you will find the attached form entitled: Consent for the Release of Medical Records that you fill in and give to your previous clinic to request your file.

New Patient Checklist To ensure that you receive the best care possible, we ask both the female and male partner to complete our Self-assessment form before their first consultation with one of our physicians. Montreal, QC, H4A 3J3 Other tests which may be required after your consultation with the physician are as follows: Female serology tests, to be completed either at a hospital or at our clinic General serology and hormone tests, between days 2 and 5 of your cycle Progesterone blood test, 7 days before your next expected period For example, if your cycle is 28 days, this test would be on day 21 AMH blood test, at any time during your cycle Available at our centre, at CDL Laboratories or at the St-Luc and St-Mary s Hospitals Female baseline ultrasound, to be completed at our clinic between days 2 and 5 of your cycle Female sonohysterogram, to be completed at our clinic between days 6 and 10 of your cycle Female mock transfer, to be completed at our clinic Note: this is often done at the same time as the sonohysterogram Male general serology test, to be completed either at a hospital or at our clinic Male sperm analysis test, to be completed at our clinic Please ensure that you read the next few pages which provide more detailed information.

FEMALE PARTNER Blood Tests There are three different blood tests that the female partner may have to complete at different times during her cycle, as part of the premiliminary investigations. Also, there may be a delay of several weeks before we receive the results of all blood tests completed at a hospital. If you decide to perform all the blood tests at our centre, we charge $655.00 which includes all general serology and hormone tests, progesterone test and AMH test. 1) General serology and hormone test The general serology and hormone test will measure the levels of several reproductive hormones such as FSH, LH, E2, prolactin, TSH and testosterone. This test will also determine your blood group, your Complete Blood Count (CBC), as well as your immunological status for HIV, hepatitis B (HBV), hepatitis C (HCV) and rubella. - When: Between days 2 and 5 of you cycle - Cost: $195.00 - Place: Available at any hospital at no cost for patients with a RAMQ card or at Originelle Fertility Clinic & Women s Health Centre 2) Progesterone test The progesterone test is a female reproductive hormone test that evaluates the ovulation status. This hormone test must be evaluated one week before your next expected menstrual cycle. For example, if your cycle is 28 days long, this test should be done on day 21. However, if your menstrual cycle is irregular, advise your physician so that he/she can advise you. When: 7 days before the start of your next menstrual cycle - Cost: $93.00 - Place: Available at any hospital at no cost for patients with a RAMQ card or at Originelle Fertility Clinic & Women s Health Centre 3) AMH test The AMH test, which means: Anti-Müllerian Hormone test, is a reflection of a woman s ovarian reserve. - When: At any time during your menstrual cycle - Cost: $175.00 (for CDL and OriginElle) - Place: Available at the St-Luc and St-Mary s Hospitals at no cost for patients with a RAMQ card. Also available at CDL Laboratories or at OriginElle Fertility Clinic & Women s Health Centre Note: The prices are subject to change without prior notice.

Preliminary Investigations There are three preliminary tests that the female partner may have to complete. Though you may have completed similar tests at a previous clinic, it is necessary that you repeat these tests at our clinic for medical purposes. If you decide to perform these three preliminary investigations at our centre during the same menstrual cyle, we offer a package at $400.00. 1) Baseline ultrasound The baseline ultrasound is a baseline vaginal ultrasound of the ovaries and the uterus in order to record the number of follicles in each of your ovaries, and detect any abnormalities such as cysts and fibroids. A woman s uterus and ovaries are continually changing from cycle to cycle and before beginning your treatment, the doctor requires you to complete this test at our centre for diagnostic purposes. - When: Between days 2 and 5 of your cycle - Cost: $120.00 - Place: Only at OriginElle Fertility Clinic & Women s Health Centre 2) Sonohysterogram The Sonohysterogram is an investigation to evaluate the uterine cavity and fallopian tubes. A thin catheter is inserted through the cervix and saline is injected with ultrasound guidance. The obstruction of saline flow may reflect a blockage in the tubes. This test also allows the detection of fibroids or intrauterine polyps. There is some discomfort associated with this procedure; as such, we recommend that the female partner take 2 Advil and 2 Tylenol an hour before the procedure. - When: Between days 6 and 10 of your cycle - Cost: $250.00 - Place: Only at OriginElle Fertility Clinic & Women s Health Centre 3) Mock transfer The Mock transfer is a procedure whereby a small catheter is inserted into the uterus. This test allows the measurement of depth and direction of the uterus. Therefore, the doctor will choose the most appropriate catheter to be used for the embryo transfer during in vitro fertilization (IVF). It is best to conduct this test with a comfortably full bladder. Therefore, refrain from using the washroom one hour before the procedure. At the time of your appointment, you will be given a prescription for an antibiotic that you must take for 5 days after the procedure. - When: Between days 6 and 10 of your cycle - Cost: $100.00 - Place: Only at OriginElle Fertility Clinic & Women s Health Centre IMPORTANT NOTE: Any test results for blood tests or preliminary investigations are valid for 12 months providing there has been no surgical intervention. Note: The prices are subject to change without prior notice.

MALE PARTNER The male partner must complete two preliminary investigations which are the general serology test and semen analysis. There may be a delay of several weeks before we receive the results of all blood tests completed at a hospital. Unlike the female partner, there is no particular order for these tests to be performed. Indeed, the male partner may decide to do these at any time. However, the female partner cannot start any treatment until the male partner has completed these two tests. Preliminary investigations 1) General serology test The general serology test is an investigation of blood cells in order to evaluate the presence of infectious diseases such as HIV, hepatitis B (HBV), and hepatitis C (HCV). - When: At any time - Cost: $265.00 - Place: Available at any hospital at no cost for patients with a RAMQ card or at OriginElle Fertility Clinic & Women s Health Centre - Previous results: The results of this test are valid for 12 months. 2) Sperm analysis The sperm analysis is an investigation of the factors that may contribute to infertility, such as: low count, motility or morphology. This test allows for the evaluation of the quality and the quantity of the sperm. There is also a considerable amount of variability between samples collected from the same male patient, from day to day. For this reason, this test may need to be repeated up to three times depending on the results and therefore, fees may apply each time. It is important to note that this test must be performed after 3 days of abstinence, in other words: no ejaculation. - When: At any time - Cost: $175.00 - Place: Only at OriginElle Fertility Clinic & Women s Health Centre - Previous results: The results of this test are valid for 12 months. Note: The prices are subject to change without prior notice.

Next step Infertility is a problem experienced by both partners, for this reason, we ask that both parties be present during the first consultation. Additional information If you are a Quebec resident and are eligible for RAMQ coverage of your treatment, please ensure that you bring your RAMQ Medicare card to all of your appointments. If you reside outside of the province of Quebec or do not have a valid RAMQ Medicare card, a fee will be charged at the time of your appointment. Note that if you have private insurance, they may reimburse you for the cost of the preliminary investigations and blood tests, depending on your policy. Do not hesitate to ask at the reception for the required forms. Also, the Quebec Government provides a 50% tax credit on fertilityrelated expenses. Your coordinator will gladly provide you with this tax credit form. If you must cancel or change a scheduled appointment, call at 514-369-06-88 or send us an e- mail at patientinfo@originelle.com at least 2 business days your appointment. Otherwise, a fee of $60.00 will apply and your appointment will be postponed to a date depending on the availability of the schedule. Note: The prices are subject to change without prior notice.
