The Magic Bracelet ATT: Ari & Stacy Goldberg 18 N. Merkle Rd. Bexley, OH 43209 stacy@rinasmovie.com www.rinasmovie.com www.makeafilmfoundation.org www.facebook.com/magicbraceletevent Although mitochondrial disease (mito) took Rina Goldberg from this world just two-weeks after her 15th birthday, her spirit is transcendent and continues to inspire. Rina s never had any doubt that her original story, The Magic Bracelet, would make it to the big screen raising crucial awareness about mitochondrial disease and spreading her life mantra: Love Life, Dream Big, Be Positive. Rina s last wish and among her final words to her Mom were "Promise to take care of my film. Make A Film Foundation did just that when they engaged some of Hollywood s most prominent filmmakers and talent to volunteer. Academy Award-winning writer Diablo Cody ( Juno ) adapted Rina's story into a short film script and Jon Poll ( Charlie Bartlett, Meet the Fockers ) directed. The film stars Bailee Madison ( Parental Guidance ), Academy Award-nominated Hailee Steinfeld ( True Grit ), Academy Award-winner J.K. Simmons ( Whiplash ), James Van Der Beek ( Dawson's Creek ), Jackson Rathbone ('Twilight' Franchise) and Kaitlin Doubleday ('Empire'). Since The Magic Bracelet s Hollywood red carpet premiere in May 2013 it has been accepted into 40 film festivals worldwide and won 10 awards (including 3 nods). We hope that you will consider becoming a corporate sponsor for the Ohio Premieres of The Magic Bracelet and its companion Behind the Scenes documentary on Thursday, September 22 nd at the historic Drexel Theater in Columbus. Cast member and OSU super fan J.K. Simmons has been invited to attend, as well as Make A Film Foundation Founder and Executive Director, Tamika Lamison. Occurring during International Mitochondrial Disease Awareness Week, your support will benefit Make A Film Foundation and the country s two major mitochondrial disease research, support and advocacy organizations United Mitochondrial Disease Foundation (UMDF) and MitoAction. We are honored that UMDF Founder and CEO, Charles Mohan, will be attending. Rina kindly never took No for an answer when it came to her movie. This event has four sponsorship levels and your partnership will help MitoAction to create a community of support for those affected by mitochondrial disease, UMDF to fund research towards a cure for mito and enable Make A Film Foundation to grant more film legacies to children who have serious or lifethreatening medical conditions. The attached packet provides more details about Rina s story, this special event and sponsorship details. Thank you for your kind consideration. Sincerely, Ari & Stacy Goldberg

Event Sponsorship Proposal Presented in Cooperation with The Magic Bracelet Ohio Premiere to benefit Make A Film Foundation, MitoAction, & United Mitochondrial Disease Foundation In celebration of Mitochondrial Disease Awareness Week Honorary Event Chairs Miriam & Bernard Yenkin Drexel Theatre Bexley, OH September 22, 2016

Event Program The Drexel Theatre in Bexley will host the Ohio Premiere of the award-winning short film The Magic Bracelet at a fundraising screening on September 22, 2016. Proceeds from the event will support Make A Film Foundation and the two primary mitochondrial disease research, support and advocacy organizations United Mitochondrial Disease Foundation and MitoAction. The Magic Bracelet screening will be preceded by a showing of its companion behind the scenes documentary "Love Life, Dream Big, Be Positive and followed by Q & A with Rina Goldberg s parents Ari and Stacy. Invited guests include Academy-award winner JK Simmons and Make A Film Foundation Executive Director Tamika Lamison. Other members of the star-studded cast and crew will be invited to participate via Skype. The Bittersweet Story Behind The Magic Bracelet Rina Goldberg was diagnosed with mitochondrial disease at age 10. In July 2009 a family friend suggested that Rina help him come up with ideas for his fledgling movie production company as a distraction from the intense medical challenges she faced daily. What ensued was truly magical. While the movie concept was a fun fictional story, it served as an avenue in which her vision and voice could reach the world, raising crucial awareness about mitochondrial disease in a light hearted fashion and spreading the mantra by which she lived - Love Life, Dream Big and Be Positive. The last 10-weeks of Rina s life were spent on pediatric home hospice. Her last wish and among her final words to her Mom were "Promise to take care of my film. After Rina died in December 2010, two-weeks after her 15th birthday, her mother Stacy kept her promise to Rina when she pitched the project to the Los Angeles-based Make A Film Foundation. In June 2011 MAFF chose Rina as their next recipient to create her legacy film. Academy Award winner Diablo Cody agreed to adapt the script, leading to recruitment of A-list cast and crew, with filming done in December 2012. The gala red carpet premiere of Make A Film Foundation/Rina's Film, The Magic Bracelet, was held at the Writers Guild Theater in Beverly Hills on May 18, 2013. Notable Cast & Crew of The Magic Bracelet Script Adaptation: Diablo Cody (Academy Award Winner - Juno) Director: Jon Poll (Charlie Bartlett, Meet the Fockers) Bailee Madison (Just Go With It, Good Witch) Hailee Steinfeld (Academy Award nominee - True Grit, Pitch Perfect 2) J.K. Simmons (Academy Award Winner - Whiplash) Jackson Rathbone (Twilight Franchise) James Van Der Beek (Dawson's Creek, CSI: Cyber) Kaitlin Doubleday (Empire) Scarlett Estevez (Daddy s Home, Lucifer)

What is Mitochondrial Disease? Mitochondria exist in nearly every cell of the human body. They are like tiny batteries within our cells. Mitochondria generate 90 percent of the energy the body needs to see, hear, walk, and breathe. Mitochondrial disease occurs when mitochondria fail. When the mitochondria fail, the body becomes unable to convert food and oxygen into energy. When these tiny power plants start to fail in the human body, the failure begins to affect organ systems, and the result is mitochondrial disease. The parts of the body that need the most energy are the most affected such as the heart, brain, muscles, and lungs. Every 30 minutes, a child is born who will develop a mitochondrial disease by age 10. Experts believe that one in 2,000 people are affected by mitochondrial disease. Further research indicates that one in 200 people carry a mutation that could develop into a mitochondrial disease in their lifetime. This group of diseases is approaching the prevalence of childhood cancers. For many, mitochondrial disease is an inherited genetic condition. In some cases, the body s mitochondria have been affected by environmental factors. In other cases, mitochondrial disease seems to be a random occurrence. At this time, there is no known cure for mitochondrial disease. Charitable Recipients Make A Film Foundation (MAFF) grants film wishes to children with serious or life-threatening medical conditions, helping them to create short film legacies by teaming them with noted actors, directors and writers who donate their time, energy and passion to making these films. MAFF believes the art of filmmaking is not only healing and therapeutic, but also offers hope and builds self-esteem. MitoAction creates a community of support that reaches every child, adult, and caregiver affected by a mitochondrial disease. Effective awareness and advocacy will guarantee that every patient has access to specialized care and a network of support and information that improves the journey for every individual affected. United Mitochondrial Disease Foundation (UMDF) exists to promote research and education for the diagnosis, treatment and cure of mitochondrial disorders and to provide support to affected individuals and families. As the largest non-governmental funder of mitochondrial disease research, the UMDF and its Scientific Advisory Board of experts have awarded nearly $11 million in grants to the most promising mitochondrial disease research proposals leading to a range of invaluable new breakthroughs. The UMDF is considered the premier foundation for mitochondrial disease by the world medical community. Testimonials I bought a copy of Rina's movie and plan to show it to as many people as I can. The message goes beyond Mitochondrial Disease. So many people live long lives and do not understand what living is about. Rina's movie is so moving and shows people what it means to truly live. - Tina from Plainfield, IL [The Magic Bracelet] really made me realize how good I have life. That movie definitely inspired me to do great things. - Bailey from Crooksville, OH The two films, Behind the Scenes and The Magic Bracelet, are together the most moving films I've ever seen. - Ilana from Haddonfield, NJ Both Rina and her movie bring awareness to mito without a sense of "doom and gloom" attached to it. The illness is there, and it is clearly serious, but Ashley's (Bailee Madison) joyful spirit comes through and outshines it. It's just beautiful! - Lisa from Sonoma Valley, CA I definitely learned a lot about Mito from watching this movie, and I also think it has a really great message. It skillfully combined a mix of information and lightheartedness. It taught me a lot without being too depressing. I really liked this movie. - 8th Grader from Austin, TX Sponsorship Opportunities $2,500 $1,000 $500 $250 Name Company Name (if applicable) Address City State Zip Telephone Email Movie Mogul- $2,500 Executive Producer - $1,000 Director - $500 Cameraman - $250 Checks may be made payable to Make A Film Foundation and sent to 18 N.
