Funding O ptions Available for Montrose Therapy Services

Funding Options There are number of ways to access Montrose services including the National Disability Insurance Scheme (NDIS) and private health insurance. We can help you identify available funding. Just call us on 1800 193 362.

Medicare Chronic Disease Management Plan (CDMP) Who is eligible? Anyone with a chronic medical condition. (A chronic medical condition is one that has been, or is likely to be, present for at least 6 months or is life limiting). How much funding is available and what services can be accessed? Up to five subsidised sessions per calendar year. The Medicare rebate is $52.95 per session. NB. There is a gap payment that you will be required to pay after the rebate. The gap payment cannot be covered by your private health fund. The following services can be accessed through Montrose: Mental Health Workers* (some social workers) Occupational Therapists Physiotherapists Speech Pathologists Psychologists (higher rebate of $84.80) This information is current at the date below. Please refer to the Medicare website for updates Helping Children with Autism (HCWA) Children registered with HCWA can access up to $12,000 (maximum $6,000 per year) to pay for early intervention services. These services include: Occupational therapy Psychology Speech Pathology A Paediatrician or Psychiatrist must diagnose ASD and provide family with a letter for the Autism advisor. This information is current at the date of this leaflet. Please refer to the Dept. of Social Services website for updates.

Better Start Who is eligible? Children who are diagnosed with the following: Cerebral palsy Deaf blindness Down syndrome, including mosaic Down syndrome Fragile X syndrome with full mutation Hearing impairment Sight impairment Prader-Willi syndrome Williams syndrome Angelman syndrome Kabuki syndrome Smith-Magenis syndrome CHARGE syndrome Cornelia de Lange syndrome Rhett's Disorder Cri du Chat syndrome; or Microcephaly How much funding is available and what services can be accessed through Montrose? Children who are diagnosed with the following: Children registered with Better Start can access up to $12,000 (maximum $6,000 per year) to pay for early intervention services. These services include: Physiotherapy Occupational therapy Psychology Speech Pathology You will need to check with your private health insurer as to whether you have General Treatment Cover and if it includes the above services. See back page for private health and NDIS funding information.

Private health insurance Private health insurance general treatment cover (also known as ancillary or extras cover) can offer outof-hospital services that are generally not funded by Medicare, such as: Physiotherapy Occupational therapy Speech Pathology Psychology You will need to check with your private health insurer as to whether you have General Treatment Cover and if it includes the above services. National Disability Insurance Scheme (NDIS) The NDIS is the new way of providing support for Australians with disability. It is being progressively rolled out across Queensland. Montrose is a registered provider of services under the scheme. Please see our website for more information about the NDIS and when it will be available in your area. WE RE HERE TO: Listen to you and help you achieve your goals. Help you prepare for the NDIS assessment and planning by supporting you to request specific services you need. Assist you to prioritise those supports and work within a budget. Self-Funded If your child is not able to access any of the above funding options, you are still able to access our services privately.
