Delta Sigma Theta Sorority, Inc. Orange County Alumnae Chapter Empowering Males to Build Opportunities for Developing Independence ABOUT Delta Sigma Theta Sorority, Inc. is a private, non-profit organization whose purpose is to provide assistance and support through established programs in local communities. EMBODI: Empowering Males to Build Opportunities for Developing Independence is the signature program of our sorority under the leadership and vision or our 24 th National President Cynthia M. A. Butler-McIntyre. The Orange County Alumnae Chapter will continue to expand their commitment in empowering young African-American males, ages 13-17, with life skills and access to build opportunities for developing independence for success. EMBODI provides an opportunity for the Orange County Alumnae Chapter to enrich and enhance the education that young teens receive in public schools throughout the Central Florida area. GOALS The goals for EMBODI are: EMBODI will focus on learning experiences for improved academic performance through self-efficacy and experiential learning. EMBODI will provide access and application of diverse learning opportunities through cultural and community support. EMBODI will reduce negative responses to conflict through restorative justice and resolutions. EMBODI will participate in community service activities through outreach. EMBODI will create opportunities to develop a higher level of self-confidence and resiliency. EMBODI will provide activities to increase community involvement, family bonding and interaction. EMBODI will seek to provide recreational activities that will develop positive social skills and improved teamwork effort. EMBODI will provide opportunities for cohesion for by involvement of families in structured activities in safe community environments. EMBODI will provide opportunities for interaction with community professionals and positive male role. EMBODI will provide career exposure. PURPOSE The aim of the EMBODI program is to develop young men with positive attitudes towards themselves and others, and who are effective and optimistic problem solvers, that are confident about their future. The EMBODI program is one response to imperatives of the future. It is an exciting and innovative way for the women of the Orange County Alumnae Chapter with a vision to turn their considerable resources and energies to developing new purpose, new skills and new leaders from the ranks of young men. The EMBODI program offers supervised and

structured experiences that will help participants grow to be productive citizens with high selfesteem and an appreciation for the demand of an increasingly technological society. COMMITMENT The EMBODI Committee is committed to young men who demonstrate the potential to succeed, but who may not have the necessary support systems to meet their highest potential. We are committed to addressing the educational and social development needs of middle and high school males. Our overachieving goal is to guide these young men into manhood with educational support and professional guidance that will teach them how to create opportunities for academic success, to develop their abilities and talents, and to become change agents in the transformations of their communities. EMBODI Criteria for Participation Participation into EMBODI will be held once a year. The following criteria will be used to determine eligibility: Males entering or attending middle or high school ages 13-17 Submit a completed application Submit all applicable consent forms to participate with parental signature Submit a Code of Conduct with parental signature Attend the mandatory student/parent orientation session Program Information Please familiarize yourself with the following guidelines: 1. Attendance We ask that your child be an active participant in the EMBODI program. Attendance at all regular scheduled meetings (the 2 nd Sunday of every month beginning in September from 3pm 5pm at Mt. Pleasant Baptist Church (unless otherwise specified), occasional midweek workshops, community service, and other planned activities. Participation is voluntary, but it requires a strong level of commitment. 2. Transportation Parents will provide transportation to and from every EMBODI session. Members of Delta Sigma Theta Sorority, Inc. are not permitted to provide transportation for students selected to participate in the EMBODI program. 3. Permission slips A trip specific permission slip must be signed and returned to the EMBODI Committee before any student will be allowed to attend an offsite trip/activity otherwise he will not be able to attend that trip. 4. Drop Off/Pick Up Your child must be signed in and out by an adult approved by the parent/guardian at every session. If a student is picked up late three (3) times he may not be able to continue participation in the program. 5. Appropriate behavior becoming of a young man should be exemplified at all times. Students must comply with the rules and regulations established in the program. ORANGE COUNTY ALUMNAE CHAPTER EMBODI APPLICATION 2017-2018 2

Code of Conduct I will respect all participants (other youth and adult volunteers) by not using foul, hurtful or obscene language or engaging in physical violence, bullying (including cyber-bullying) or other aggressive behaviors that threaten the safety of others. Such offense may result in a loss of privileges and my parent or guardian may be notified. I will respect the property rights of others. This means do not damage or deface the building or property within the building where activities are held. Do not damage or take the personal property of any other participant or volunteer; and do not use Delta s name or any symbol or logo on any clothing, books, bags or other items. I will clean up all work areas properly and return supplies to their proper place after using them. I will listen carefully to directions and when someone else is talking. I will stay within the designated areas within the building and respect designated quiet areas. I understand that my attendance will be monitored and more than two (2) unexcused absences may result in me being dropped from the program and may also prohibit me from being able to participate on any field trips. I will cooperate and participate in organized activities. I will be on time for all scheduled activities, and be open to new ideas. I will assume full responsibility for all personal belongings. Please leave valuables at home. I will not bring any weapons, cigarettes/drugs, alcohol, or anything illegal to any activity at any time. Such items and usage during the activity may result in immediate dismissal from the program. I understand that unacceptable behavior will not be tolerated. If the unacceptable behavior continues, after the initial occurrence I may be removed from the program. I understand that if I am sent home early due to misconduct, it will be my parent s responsibility to provide transportation regardless of the time of day or night. I understand that any additional costs for transportation will be my parent s responsibility. I understand that my attitude and behavior are central to the success of this activity sponsored by the Orange County Alumnae Chapter of Delta Sigma Theta Sorority, Inc. Therefore, for the good of this activity as well as for myself and my fellow group members, I agree to abide by the statements above: Student Signature: I have read and understand the Code of Conduct. Consent to Participate I voluntarily give permission for to participate in the EMBODI Program sponsored by the Orange County Alumnae Chapter of Delta Sigma Theta Sorority, Inc. I am authorized to give permission for the student to participate in the program. My child s participation is completely voluntary. Delta Sigma Theta Sorority, Inc. Orange County Alumnae Chapter and its related entities will make every effort to protect the welfare of the EMBODI participants; however the program committee members are not responsible for ensuring the physical, mental, social and medical health of the program participants. As a parent/guardian, I am responsible for the welfare of my child. The committee may suspend a student s participation if their behavior does not reflect the spirit of the program. I understand and agree that these images will become the property of the Orange County Alumnae Chapter, which shall have complete ownership of the Images. I agree to hold harmless and release and forever discharge EMBODI of Orange County Alumnae Chapter and any of its officers and members, national executive board, employees, representatives, and agents from any and all liability arising from or in connection with the taking, use, publication, or dissemination of such materials. Copies of these photos may be distributed to the parent upon request. I do hereby agree to release and hold harmless EMBODI of Orange County Alumnae Chapter of Delta Sigma Theta Sorority, Inc, its officers, National Executive Board, employees, members, representatives, agents and assigns from any and all claims, costs, suits, actions, judgments, and expenses for any damage, loss, or injury to my child or damage to my child s property arising from my child s participation in field trips, other than damage, loss, or injury that results from gross negligence or intentional infliction of harm by the EMBODI of Orange County Alumnae Chapter of Delta Sigma Theta Sorority, Incorporated, its officers, National Executive Board, employees, members, representatives, agents and assigns. The Orange County Alumnae Chapter strongly recommends annual health examinations and dental checkups. Our policy on health and safety implies a responsibility to the participants for their protection.
