Utilization Management Policy Name: Topical Tretinoin Products Restricted Product(s): All topical tretinoin containing products require medical necessity review Atralin (tretinoin) Altreno (tretinoin) Retin-A (tretinoin) Retin - A Micro/ pump (tretinoin microsphere) Tretin X (tretinoin) Veltin (clindamycin phosphate- tretinoin) Ziana (clindamycin phosphate- tretinoin) Unrestricted/Suggested Alternative(s): erythromycin gel 2% (generic Erygel ) clindamycin phosphate gel 1% (generic Cleocin-t ) clindamycin phosphate-benzoyl peroxide (Benzaclin) clindamycin phosphate-benzoyl peroxide (Duac) benzoyl peroxide -erythromycin (Benzamycin) adapalene (Differin) acne diagnosis required Generic topical tretinoin products require medical necessity review only o tretinoin cream (Retin-a) o tretinoin microsphere gel (Retin-a micro pump) o clindamycin phosphate -tretinoin (Ziana) FDA Approved Use: for the topical treatment of acne vulgaris Rationale: Tretinoin products can be used in attempts to treat conditions that have not been validated by the FDA. In particular, tretinoin can be used for cosmetic treatments, such as fine wrinkles, which is not medically necessary. This program ensures that members are receiving these medications for conditions that have the appropriate level of evidence. Other products such, as generic topical tretinoins, topical antibiotics, and adapalene treat the same condition at a substantially lower cost to members with equal results. Criteria Summary: FDA approved use; medical necessity; trial of effect and lower cost product(s) Page 1

Criteria for Approval of Restricted Product(s): 1. The patient is using for the treatment of acne vulgaris, actinic keratosis, or ichthyoses; AND 2. For members requesting brand name tretinoin containing products, a trial and failure of, or clinical intolerance/contraindication to generic tretinoins is required; AND 3. For formularies that exclude (non-formulary) the requested medication, Non-formulary Exception Criteria applies (outlined below)* Duration of Approval: 730 days (2 years) *Non-formulary Exception Criteria Non-Formulary Exception criteria applies on formularies which exclude requested product(s). Satisfactory completion of criteria points (above) may satisfy some, or all, portions of the Non-Formulary Exception Criteria. This criteria is summarized as: a) Request must be for an FDA approved indication; AND b) Patient must have a trial and failure of up to TWO formulary medications or a clinical contraindication/intolerance to those medications not tried.
