Asthma Treatment Plan Student Parent Instructions The PACNJ Asthma Treatment Plan is designed to help everyone understand the steps necessary for the individual student to achieve the goal of controlled asthma. 1. Parents/Guardians: Before taking this form to your Health Care Provider, complete the top left section with: Child s name Child s doctor s name & phone number Parent/Guardian s name Child s date of birth An Emergency Contact person s name & phone number & phone number 2. Your Health Care Provider will complete the following areas: The effective date of this plan The medicine information for the Healthy, Caution and Emergency sections Your Health Care Provider will check the box next to the medication and check how much and how often to take it Your Health Care Provider may check OTHER and: v Write in asthma medications not listed on the form v Write in additional medications that will control your asthma v Write in generic medications in place of the name brand on the form Together you and your Health Care Provider will decide what asthma treatment is best for your child to follow 3. Parents/Guardians & Health Care Providers together will discuss and then complete the following areas: Child s peak flow range in the Healthy, Caution and Emergency sections on the left side of the form Child s asthma triggers on the right side of the form Permission to Self-administer Medication section at the bottom of the form: Discuss your child s ability to self-administer the inhaled medications, check the appropriate box, and then both you and your Health Care Provider must sign and date the form 4. Parents/Guardians: After completing the form with your Health Care Provider: Make copies of the Asthma Treatment Plan and give the signed original to your child s school nurse or child care provider Keep a copy easily available at home to help manage your child s asthma Give copies of the Asthma Treatment Plan to everyone who provides care for your child, for example: babysitters, before/after school program staff, coaches, scout leaders PARENT AUTHORIZATION I hereby give permission for my child to receive medication at school as prescribed in the Asthma Treatment Plan. Medication must be provided in its original prescription container properly labeled by a pharmacist or physician. I also give permission for the release and exchange of information between the school nurse and my child s health care provider concerning my child s health and medications. In addition, I understand that this information will be shared with school staff on a need to know basis. By the time a child reaches the Middle School, they are usually capable and are encouraged to self-administer their emergency medication. This is especially important during field trips, sports, after school activities and school lock downs when a nurse may not be immediately available. Board Policy 5111.2 permits a child to carry and self-administer his/her own emergency medication only if the attached forms are completed in full. The doctor s order must be renewed annually. If your child is unable to self-administer emergency medication, please contact the school nurse to discuss possible alternatives. If your child no longer has this condition or does not require emergency medication, please have the physician write a note stating this so your child s health record can be updated. Any student who self-administers his/her emergency medication must have his/her medication with them at all times. Students may not be allowed to participate in after school activities, field trips, sports, etc. unless they have their emergency medication with them. If your child requires an epipen, the parent/guardian has the option of a trained epipen delegate to be onsite during times when the school nurse is not available (i.e. field trips, after school activities, etc.). Parents should indicate on the form Permission or refusal to appoint designee either to accept or decline this option. Epipen the physician must also complete the Individual Healthcare plan for Severe Allergies/anaphylaxis; the reverse side must be signed by both the physician and parent/guardian. Per Board Regulation the school nurse or designee shall arrange for the transportation of the pupil to a hospital emergency room by emergency services personnel (9-1-1) after the administration of epinephrine, even if the pupil s symptoms appear to have resolved. Asthma the physician must also complete the Asthma Treatment plan; the form must be signed by both the physician and parent/guardian. If your child does not have the forms on file and were to have an asthma attack at school, 911 would be called and your child would be sent to the nearest emergency room for his/her safety. The following guidelines will be used for the self-administration of medication for asthma or other potentially life-threatening illness or a life-threatening allergic reaction. 1. Parents or guardians must provide the Board with written authorization for the Self Administration of Emergency medication. 2. The authorization must be signed by parent/guardian. 3. A copy of the authorization must be kept on file in the nurse s office and may be reviewed by staff who may have contact with this student. 4. Written doctor s orders must be provided to the school nurse on the form provided by the school district. All applicable sections must be completed and signed by the doctor, reviewed and signed by the school doctor, and reviewed and signed by the school nurse. 5. The request must be renewed annually. 6. Students should keep the medication for asthma or other potentially life-threatening illnesses or a life threatening allergic reaction with them at all times. 7. It is strongly recommended that back-up medication for asthma or other life-threatening illnesses or a life-threatening allergic reaction be kept in the nurse s office. 8. Pupils self-administering medication shall report each incident to a teacher, coach or other individual designated by the school district to be in charge of the pupil during school activities. The student must inform the nurse EACH time the medication is administered so it can be documented in the student s health record.
