Cmmunity Needs Assessment Rush Cpley Medical Center 2017 Preventin and Screening Initiatives Rush Cpley Medical Center s (RCMC) main service areas include Kane Cunty and Kendall Cunty, Illinis. The 2014 Kane Cunty Cmmunity Health Needs Assessment findings listed cancer as the number ne leading cause f death in Kane Cunty, accunting fr 25.7% f deaths in 2010. Risk factrs assciated with cancer include tbacc, diet, physical activity and envirnmental/ccupatinal expsures. Anther area f grwing cncern is HPV vaccinatin, which can prevent certain viruses being linked t cancer. In Illinis, the HPV vaccinatin rate fr girls ages 13-15 is 44% and ages 13-17 is 47.7%. This includes girls that have received the entire three series vaccinatin. The Kane and Kendall Cunty Needs Assessment lacked data specific t HPV vaccinatin rates fr the cunties they serve therefre data frm the 2014 Natinal Immunizatin Survey was utilized. Alng with HPV vaccinatin, ur center fcused n clrectal cancer screening. Accrding t the American Cancer Sciety clrectal cancer is the secnd leading cause f cancer-related deaths when bth sexes are cmbined. The Rush Cpley Cancer Care Center treats an average f 75 cln cancer patients a year. There are different methds fr clrectal cancer screening and f these the gld standard is a clnscpy. The Rush Cpley Cancer Care Center is cmmitted t preventative screening t find clrectal cancer at an early stage and has signed the Natinal Clrectal Cancer Rundtable pledge t reach 80% screened by 2018. Accrding t the 2014 CHNA, cancer is the leading cause f death in this cmmunity, with cln cancer representing 8.9% f all cancers diagnsed between 2007 and 2011. This is the furth mst prevalent cancer diagnsis during that time frame. In Illinis it is estimated by the Behaviral Risk Factr Surveillance System (BRFFS) data that nly 62.9% White Nn- Hispanic men and wmen have received clrectal screening, 60.1% f Black (including Hispanics), and 47.7% f Hispanic. The 2014 CHNA estimated that in Kane Cunty 73.9% have had a clnscpy hwever it desn t reflect the numbers brken dwn by race and ethnicity t demnstrate the disparity in rutine clrectal cancer screening. One f the majr risks fr clrectal cancer is smking. In Kane cunty 13.6% f the ppulatin currently smkes and the highest rate f 15.7% ccurs within the suthern part f Kane cunty, which is a large prtin f Rush Cpley s service area. Smking is higher amunt fr thse wh are 18-39 years ld, lw incme, males and live in suthern Kane Cunty. Gals fr Initiatives I. The RCMG Directr f Clinical Services and Advance Practice Prviders is wrking their EMR team t pull a reprt in 2018. Rush Cpley physicians and clinical staff attended a CME n Human Papillmavirus (HPV) and Cancer: Strategies fr Preventin hsted n Octber 18, 2017. The speakers were Suzanne Bergen, M.D. and Nicle C. Keller, D.O., FAAP frm RCMG. There were 50 attendees wh included RCMG physicians, nurses, residents, ther clinical staff, Advcate Medical Grup physicians and the American Cancer Sciety. This cmmittee has created a letter which will be sent t lcal area dentists in the beginning f 2018. The letter will include infrmatin n HPV vaccinatin as well as the gals and bjectives f this initiative. There will als be an pprtunity t prvide educatin and resurces fr patients. Prgram fr tbacc cessatin. The prgram is facilitated by tw Rush-Cpley trained emplyees. Freedm Frm Smking is ffered n a quarterly basis in English at RCMC and in Spanish at the VNA Health Care. There were 7 participants that attended the prgram in 2017: 5 cmpleted the prgram and quit smking. This equates t a prgram cmpletin rate f 71% in 2017. This data desn t include the Nvember 2017 prgram. Barriers with HPV Vaccinatin Educatin and Smking Cessatin Initiative One barrier facing the HPV cmmittee was cnsistency f attendance frm cmmunity partners due t staff turnver. Anther barrier was lack f data specific t HPV vaccinatin fr RCMC s service area. Meetings and Outreach The Cmmunity Health Outreach Crdinatr met with lcal FQHCs t identify barriers assciated with CRC screening. It was discvered that there are very limited resurces in lw cst clnscpies fr patients that are uninsured. The cmmittee cntinues t wrk with FQHCs t gather data specific t the uninsured ppulatins and a are waiting n this data t research funding pprtunities such as grants r hspital resurces fr clnscpies. Tw lunchens were scheduled with African American and Hispanic/Latin faith based cmmunity leaders. A lunchen fr Cathlic Churches that have Spanish services was held n August 20, 2017 and three leaders attended. The Fx Valley Minister Healthcare Alliance was held n February 22, 2017 and fur leaders attended. Faith based leaders were educated n clrectal cancer statistics fr Kane and Kendall cunties and areas f fcus in rder t increase screening rates. They were presented with a three series educatinal prgram n the imprtance f clrectal cancer screening, statistics, risk factrs, screening methds and screening resurces in their cmmunity. The faith based leaders were interested in this prgram, hwever, they didn t respnd t fllw in rder t crdinate the prgram. The faith based leasers frm the Spanish masses sent representatives t the lunchen since they had their annual meeting with the Bishp. The representatives fr the parishes suggested that the best time t crdinate the prgram wuld be after the hliday seasn, hwever this time frame did nt end up wrking. The Cmmunity Health Outreach Crdinate cntinued t reach ut and had a successful meeting n September 21, 2017, t establish prgrams at the parishes. Since this meeting the Cmmunity Health Outreach Crdinatr has been making annuncements during mass and has prgrams scheduled fr the beginning n 2018 at tw churches. Marketing The infrmatin fr clrectal rectal cancer screenings was disseminated in a variety f frmats t the cmmunity, healthcare prviders and centers in the area. Educatin and Screening Rush Cpley s Cancer Care Center educated 51 cmmunity participants abut the imprtance f fllwing prep instructins, getting a clnscpy and risk factrs fr clrectal cancer. Get the Scp n the Scpe prgram fcuses n the imprtance f clnscpy screening, which is then fllwed by a shrt presentatin, Dn t Dread the Prep aids in reducing anxiety and fear regarding the prep fr a clnscpy. Get the Scp n the Scpe was presented by a gastrenterlgist and surgen and Dn t Dread the Prep by nursing staff frm Digestive Health Services and the Pain Clinic. Get the Scp n the Scpe event was held twice in 2017. The first prgram was held in March and Dr. Syed Zaidi presented. There were 36 participants that attended the event and three participants scheduled their clnscpy. The secnd prgram was held in September at the Kendall Cunty Health Department lcated in Yrkville. Dr. Tmmy Pacana and Melissa Jenkins presented n clnscpy screening and clnscpy prep. There were 15 attendees and ne participant was interested in scheduling their clnscpy. The participant evaluatins stated that they gained knwledge, the prgram met their expectatins and they feel better equipped t discuss clrectal cancer screening with a physician. The CRC Cmmittee began in 2016 and since then there has been an increase in clnscpy screening amng the tw (Hispanic/Latin and African American) cmmunity target ppulatins. At Rush Cpley there has been an increase f 20.9% amng Hispanic/Latins patients between 2016-2017 and an increase f 75.2% between 2015-2017. In the African American patients there was an increase f 14.3% between 2016-2017 and 52.7% between 2015-2017. Amng all races/ethnicities there has been an increase in clnscpies f 34.9% frm 2015-2017. Participants that received a free ifobt Kits cmpleted an intake frm and the Cmmunity Health Outreach Crdinatr explained the ifobt Kit t them. Each ifobt Kit was accmpanied by a letter frm the Caner Care Center explaining the test, testing instructins and a prepaid envelpe t mail the test back. The letter and instructins were available in English and Spanish. ifobt Kit were returned t the lab fr results and a fllw up call wuld take place fr psitive findings. Additinally, a letter was sent t all participants with results and instructins. The letter wuld infrm the participant if they needed t schedule a clnscpy and that the infrmatin was als prvided t their primary care physician.
The Cmmunity Health Outreach Crdinatr attended Cmmunity Health Fairs, presented at Cmmunity Talks and partnered in Kendall Cunty Free Clinic t free ifobt Kits. Rush Cpley s Cancer Care Center was able t prvide 17 free ifobt Kits in 2017. Frm the 12 ifobt kits handed there have been five were returned and they were negative. The Cmmunity Health Outreach Crdinatr cntacted the participants that didn t return the ifobt Kit. Each received tw calls and vicemails.
