1 Comprehensive medicine review Talk one-on-one with a pharmacist. Learn more about your different medicines and how they work. Understand side effects, possible interactions and more. After your review, you ll get a complete list of your medicines and an action plan that you can bring with you to your next doctor s visit. 2 Targeted medicine review And, every few months, the program will review your prescription claims to make sure there are no issues that need attention. We may contact your doctor if we spot a potential problem with your medicines. 97% 97 percent of people who have participated said they learned more about their medicines.*
Your review is at no extra cost to you. It s part of your Medicare Part D prescription drug plan. The MTM program helps you: Be safe Feel good Save money Get started! It looks at all of your medicines, as well as any supplements and vitamins you take. It can help make sure your medicine works as well as possible. It can help your wallet stay healthy, too. Your review is private, and will last about 30 minutes. After your MTM pharmacist reviews your list, you can talk about any concerns you may have. Learn about possible drug interactions Identify over-the-counter products that may affect your medicines Hear about treatment options that you and your doctor can discuss, to help you get the best outcome from your medicine If your medicine review uncovers a serious issue, your pharmacist can even work with your doctor to help resolve it.
