Central Oregon Acupuncture Financial Policy I understand and agree that all services rendered to me by Central Oregon Acupuncture (COA) are charged directly to me and that I am personally responsible for payment. I understand that I am responsible for obtaining information from my insurance company regarding Acupuncture and/or Alternative Care benefits. I further understand that benefits quoted to me or to COA by my Insurance Company are not a guarantee of payment and I am ultimately responsible for all charges incurred. Initials Some Insurance Plans combine Acupuncture, Massage Therapy, Chiropractor and Naturopath all under Alternative Care. I understand that I am responsible for knowing and keeping track of my benefits. Central Oregon Acupuncture Informed Consent to Receive Treatment By signing below, I do hereby voluntarily consent to be treated with acupuncture and/or substances from the Oriental Materia Medicia by a licensed acupuncturist at Central Oregon Acupuncture (COA). I understand that acupuncturists practicing in the state of Oregon are not primary care providers and that regular primary care by a licensed physician is an important choice that is strongly recommended by COA practitioners. Acupuncture/Moxibusion: I understand that acupuncture is performed by the insertion of needles through the skin or by the application of heat to the skin (or both) at certain points on or near the surface of the body in an attempt to treat bodily dysfunction or diseases, to modify or prevent pain perception, and to normalize the body s physiological functions. I am aware that certain adverse side effects may result. These could include, but are not limited to local bruising, minor bleeding, fainting, pain or discomfort, and the possible aggravation of symptoms existing prior to acupuncture treatment. I understand that no guarantees concerning its use and effects are given to me and that I am free to stop acupuncture treatment at any time. Cupping: I understand that if I receive cupping as part of therapy, there is a risk of temporary bruising, reddening of the skin or blistering from its use. I understand that I may refuse this therapy. Chinese Herbs: I understand that substances from the Oriental Materia Medica may be recommended to me to treat bodily dysfunction or diseases, to modify or prevent pain perception and to normalize the body s physiological functions. I understand that I am not required to take these substances but must follow directions for administration and dosage if I do decide to take them. I am aware that certain adverse side effects may result from taking these substances. These could include but are not limited to, changes in bowel movement, abdominal pain or discomfort and the possible aggravation of symptoms existing prior to herbal treatment. Should I experience any problems, which I associate with these substances, I should suspend taking them and call COA at 541-330-8298 as soon as possible. Acupressure/Tui-Na Massage: I understand that I may also be given acupressure/tui-na massage as part of my treatment to modify or prevent pain perception and to normalize the body s physiological functions. I am aware that certain adverse side effects may result from this treatment. These could include but are not limited to bruising, sore muscles or aches and the possible aggravation of symptoms existing prior to treatment. I understand that I may stop the treatment if it is too uncomfortable. Laser Therapy: I understand that I may also be given Laser Therapy and there is a risk of reddening of the area or possible aggravation of symptoms existing prior to treatment. I understand that I may refuse this treatment. Electro-Acupuncture: I understand that I may be asked to have electro-acupuncture administered with the acupuncture. I am aware that certain adverse side effects may result. These may include but are not limited to electrical shock, pain or discomfort and the possible aggravation of symptoms existing prior to treatment. I understand that I may refuse this treatment. I understand that there may be other treatment alternatives, including treatment offered by a licensed physician. Central Oregon Acupuncture (COA) Bend & Redmond Fee Schedule Effective 2/24/14 The fees for acupuncture treatment vary depending upon the procedures that are performed. The following is intended simply as a guideline to help you understand the billing process. The average amount charged for a New Patient Office Visit with Acupuncture treatment is $190 and the average amount charged for a Return Patient Office Visit with Acupuncture Treatment is $175. Additional fees are charged for Infrared Heat Therapy, Laser Therapy, Electrical Stimulation, Manual Therapy and Massage Therapy. These charges apply to all patients except those utilizing the Discount for Payment in Full at the Time of Service. See below for details. The patient is responsible to obtain information from their insurance company regarding Acupuncture and/or Alternative Care Benefits. Benefit information quoted to you or COA from the insurance company is not a guarantee of payment and you are responsible for all charges. COA contracts with a business that specializes in billing insurance and processing claims. If your insurance covers Acupuncture, COA will have them bill your Insurance. Your co-pay or co-insurance amounts along with any remaining deductible are due at the time of service. If you are meeting your Deductible we collect $160 for your first appointment and $120 for follow-up appointments until your Deductible is met. You may also be billed for charges that are not covered by your insurance. Discount for Payment in Full at Time of Service COA offers a discount for payment in full at the time of service. This allows us to keep our costs down and pass the savings on to you. The discounted amounts have no procedure or diagnosis codes and cannot be billed to your insurance. Upon request COA will provide you with a receipt showing the date of visit and payment amount. Some FSA s (Flexible Spending Account) and HSA s (Health Savings Account) will accept the receipt and provide reimbursement. The following are the prices for payment in full at the time of service: New Patient - $110: Includes Initial Consultation and treatment with the Acupuncturist. May also include Electrical Stimulation, Infrared Heat, Laser Therapy, Manual Therapy and Massage Therapy Return Patient - $75: Includes follow-up Consultation and treatment with the Acupuncturist. May also include Electrical Stimulation, Infrared Heat, Laser Therapy, Manual Therapy and Massage Therapy Senior New Patient - $83: Age 65+ or on Medicare. Includes Initial Consultation and treatment with the Acupuncturist. May also include Electrical Stimulation, Infrared Heat, Laser Therapy, Manual Therapy and Massage Therapy Senior Return Patient - $63: Includes follow-up Consultation and treatment with the Acupuncturist.
