Page 1 of 12 Application Information In order to be considered for acceptance into the AAS Physical Therapist Assistant program, all application and required supplemental documents must be received no later than Friday, October 10, 2014 at 5:00pm. Electronic applications and supplemental documentation, including letters of recommendation and clinical observation forms must be received by the UAA Physical Therapist Assistant program office no later than the application deadline. Letters of recommendation and clinical observations postmarked by the application deadline will not be accepted. Use the application checklist to help ensure you have everything required for application submission. Incomplete applications will not be processed, and applicants will not be notified of incomplete applications or missing documentation. Program Eligibility To qualify as a candidate for the indicated year, applicants must: Be admitted as degree-seeking student at the University of Alaska Anchorage (UAA). Students must have a "C" or better in all completed prerequisite courses or be in progress of all prerequisite coursework by the application deadline. Unless otherwise approved by the PTA faculty advisor, all coursework must be documented on the UAA transcript on or before the application deadline. Have a minimum GPA of 2.5 GPA in all coursework including all coursework from previous colleges and universities. Submit all application materials by the date posted on the PTA program website. Late or incomplete applications will not be considered. To qualify as a candidate for the indicated year, your complete application must contain the following: Completed and submitted Physical Therapist Assistant web application. Completed or in progress prerequisite coursework indicated on the UAA academic transcript Three (3) letters of recommendations on the forms provided completed by non-family members. At least two (2) completed clinical observation forms demonstrating a minimum of 30 hours of observation/work experience in two different clinical settings e.g., outpatient, hospital, inpatient, home health, rehab center, or long term care. Proof of Professional Rescuer CPR certification through the American Red Cross or Basic Life Support (BLS) certification through the American Heart Association. Proof immunizations as instructed on the PTA program website.

State Boards of Physical Therapy have strict guidelines regarding a history of drug/alcohol abuse and prior criminal convictions. Students who have a history of drug/alcohol abuse, a criminal record or emotional/mental illness must disclose this information when applying for licensure/registration/certification and may be deemed ineligible by the State Board of PT for licensure/registration/certification. The following questions are included in the licensure application: 1. Have you been convicted of a crime or are you currently charged with committing a crime? For purposes of this question, crime includes a misdemeanor, felony, or a military offense, including, but not limited to, driving under the influence (DUI) or driving while intoxicated (DWI), driving without a license, reckless driving, or driving with a suspended or revoked license. Convicted includes having been found guilty by verdict of a judge or jury, having entered a plea of guilty, nolo contendere or no contest, or having been given probation, a suspended imposition of sentence, or a fine. 2. Have you ever been convicted of a violation of federal or state narcotic laws? 3. Are you now or have you been in the last five years addicted to, or excessively used, or misused, alcohol, narcotics, barbiturates or habit forming drugs? 4. Are you now or have you been in the last five years treated for bipolar disorder, schizophrenia, paranoia, psychotic disorder, substance abuse, depression (except for situational or reactive depression) or any other mental or emotional illness? A "yes" answer to one of the above questions does not necessarily mean that you will not be eligible for licensure. It is highly recommended that applicants with a "yes" answer to any of the above questions contact the State of Alaska Division of Occupational Licensing, Board of Physical and Occupational Therapy (907) 465-2580 to discuss the potential ramifications on their future eligibility for licensure. Essential Requirements

Page 9 of 12 Essential Functions Successful participation in the PTA Program requires the ability to perform essential functions (with or without reasonable accommodation), which are necessary for safe/ethical delivery of physical therapy. Applicants should consider their ability to complete the tasks below, with or without reasonable accommodation. Reasonable accommodation refers to ways in which the university can assist students with disabilities to accomplish these tasks (for example, providing extra time to complete an examination or enhancing the sound system in a classroom). Reasonable accommodation does not mean that students with disabilities will be exempt from needing to demonstrate competency in certain tasks; it does mean that we will work with students with disabilities to determine whether there are ways that we can assist the student toward completion of the tasks. Prospective students who indicate that they can complete these tasks (with or without reasonable accommodation) are not required to disclose the specifics of their disabilities prior to an admission decision. Prospective students who cannot complete these tasks with or without accommodations are ineligible for consideration for admission. If admitted, it is the responsibility of the student with a disability who wishes reasonable accommodation to request it through the Disability Support Services. An offer of admission may be withdrawn if it becomes apparent that the student cannot complete essential tasks (even with accommodation), that the accommodations needed are not reasonable and would cause undue hardship to the institution, or that fulfilling the functions would create a significant risk of harm to the health or safety of others. Essential Functions for the PTA Student Motor: The student must possess sufficient motor capabilities in order to provide safe and effective physical therapy procedures, including the ability to: Sit or stand for up to 8 hours a day Assist and protect patients who are walking, exercising, or performing other activities Adjust, move, position, and lift patients and equipment, which involves lifting, carrying, pulling and pushing weights up to and including 100 pounds Perform pushing, pulling, bending, twisting, reaching, standing, kneeling, sitting, walking, and crawling Palpate anatomical structures and handle injured body parts without causing injury to the patient Give manual resistance to a patient s limbs or trunk during treatment/exercise Move with adequate agility and speed to ensure patient safety. Walk and balance well enough to help patients walk and transfer with or without equipment, while preventing injury to patient and oneself Provide cardiopulmonary resuscitation (CPR) using guidelines issued by the American Heart Association or American Red Cross Manipulate equipment including adjustment of dials, gauges, small nuts/bolts, and various equipment settings Move about in a clinical setting steadily throughout an 8 hour work day Observational and Sensory: The student must possess sufficient sensory abilities in order to competently assess and monitor patients, including: Sufficient visual ability to: Recognize and interpret facial expressions and body language Identify normal and abnormal postures and patterns of movement

Page 10 of 12 Identify normal and abnormal postures and patterns of movement Read or set parameters on equipment Read small numbers / scales on goniometers, thermometers, sphygmomanometers, etc. Discriminate differences and changes in skin and soft tissue Recognize a patient s physiological status Assess a patient s environment Sufficient auditory ability to: Recognize and respond to verbal communication, auditory timers and equipment alarms in an environment with a moderate level of background noise Effectively use devices for measurement of blood pressure, auscultation of heart and lung sounds Communication: The student must possess sufficient ability to communicate effectively and competently in the English language with others using appropriate verbal and written methods, including the ability to: Read at a level of competency that allows him/her to safely perform the essential functions of an assignment Write in a legible manner Present information about physical therapy procedures and services to patients, family members, and other health care professionals Recognize, interpret, and respond to nonverbal behavior of self and others Interpret and communicate information regarding the status, safety, and rehabilitation of patients Record thoughts legibly and clearly for written assignments and tests Record/document patient care notes and communications in standard medical charts in hospital/ clinical settings in a timely manner and be consistent with the acceptable norms of the clinical settings. Behavior: The student must possess sufficient ability to exercise good judgment, develop therapeutic relationships with patients and others, work in stressful situations, and tolerate close physical contact with patients and co workers, including the ability to: Work with multiple patients and colleagues at the same time Work with others under stressful conditions Recognize and respond appropriately to individuals of all ages, genders and races, and from all socioeconomic, religious, and cultural backgrounds Act in the best interests of patients Behave in an ethically sound, competent, compassionate and professional manner in the classroom and clinic. Critical Thinking: The student must possess sufficient ability to comprehend and process information in a timely manner, including the ability to: Collect and interpret data related to patients and physical therapy services Prioritize multiple tasks, process information, and make decisions Apply knowledge from education to the provision of physical therapy services Observe, measure, and interpret normal and abnormal patient responses to physical therapy interventions, and appropriately modify treatment interventions Act safely and ethically in the physical therapy lab and clinic Cope with the stress of heavy workloads, demanding patients and potentially life threatening clinical situations Recognize and respond appropriately to potentially hazardous situations
