Hillingdon Carers Delivery Plan, -17 (Young carers)

Objective 1: Finding hidden young carers KPIs: (1) No. of schools engaging with outreach programme (2) No. of new young carers identified by schools (3) No. of new young carer referrals across all projects Review current process and collate a new series of tools to use as part of outreach programme. Continue inputting into central outreach planning table. Target: 5 primary schools, 5 secondary schools plus N-Hance target of 16 sessions per year. Identify a lead member of staff for outreach and education to better coordinate activity Review and strengthen outreach and education programme and identify new routes to identification of young carers Engage a wide range of partners to develop a Think Young Carer campaign Set ourselves an annual target for numbers of new young carers identified Collect data on referrals and what prompted young carers to seek our support and use to target activity Develop a stronger education programme for schools and colleges Review good practice and research Target Boards of Governors and develop compelling key messages to engage schools in the identification and support of young carers. Target: 20 new referrals in 12 months To plan a targeted campaign with a range of partners As per SLAs: YC 90 new referrals annually YC Plus 33 annually N-Hance 60 over 3 years Target: 143 new young carers annually Assess the effectiveness of outreach using charity log statistics and then target further outreach to maximise new referrals. Target: cleanse charity log data by 15 Review current processes and presentations and develop a Schools Information Pack N-Hance to initiate a school/college programme Target: at least 2 schools and 1 college. Visit other Young Carers services. Network Attend conferences/events/training 2017 Ongoing

Objective 3: Extending our range of services KPIs: (1) No. of young carers regularly attending weekly Monday Club (2) Project plan for complex families developed (3) No. volunteer/sessional workers supporting the team Develop our approach to working with complex families and offer whole-family packages of support Review complex family research report before setting targets (due October ) Target: working group held to discuss finding of report and agree future actions to implement Nov Develop our support to young adult carers through the new H-HANCE supported transition project As per Lloyds funding and Carers Trust Bid (see separate Project Delivery Plan) 2017 Grow a pool of sessional workers to enable expansion and contraction of services according to demand. The team needs extra support at differing times of the year, particularly school holidays. Target: To recruit, train and support one additional sessional worker this year Increase frequency and geographical spread of our services. Targets: Increase frequency of Northwood Monday Club to weekly from Sept 15 Grow membership of this club in the north to 15 regulars by 16 Sept Monday Clubs now running weekly (Sept 15)

Objective 4: Supporting young carers to look after their KPIs: (1) No. of volunteers supporting the team own physical and mental health (2) No. of young carers accessing training workshops (3) No. of schools receiving presentations (4) No. of new referrals from schools Continue to develop Activ8 and extend the offer to include healthy eating/cooking Continue with current level of service. Target: To seek to obtain independent funding to provide an all-age carer health programme to include exercise and activities by 2017 2017 Recruit a further 8 volunteers by summer 2017 and develop training and support offered to volunteers Work with schools to promote a better understanding of the needs of young carers and to support young carers to function within the system Promote access to a good range of life skills and health information through regular activities and a programme of workshops Through work with complex families support young carers and provide an advocacy function for them with existing statutory services To further develop a volunteer package this includes training. Target: To recruit two new volunteers by 16 To plan and initiate a more coordinated school s outreach programme. Objective 5: Helping young carers to manage their caring situations KPIs: (1) No. of families accessing drop-in sessions and whole family activities (2) Nos. of young carers accessing training Extend our workshop programme to give young carers new skills and coping strategies Grow our whole-family programme and provide more outreach and drop-in sessions for parents of young carers such as free legal advice Give young carers time out, new experiences and social lives outside of caring role Provide whole family activities so that young carers can enjoy social activities outside of caring as part of normal family life Encourage peer-to-peer discussions so that young carers get support from each other as well as staff Maintain current levels of service and continue to develop programme of workshops Targets: For all N-Hance participants to attend at least 3 workshops each by Jun For all young carers to have access to have access to a programme of at least 6 ageappropriate workshops by Jun Review complex family research report before setting all targets (due October ) However: Target: to provide 1 Family Drop in session in and at least 2 sessions in. Drop-in session to include emotional support with a trained counsellor, legal advice, activities and complimentary therapies. Continuation of current service, provision of trips, clubs, residential breaks and school holiday programme. Target: to extend provision by increasing frequency of Monday Clubs to weekly from Sept Seek funding for additional family outings. Obtain feedback from families to evidence the need for family social activities. Target: 30 families to access support and activities by Sept 16 Provide opportunities for young carers to meet socially and promote informal discussions, sharing of ideas, coping strategies etc. Jun Jun Oct On-going September On-going

Cross-cutting objective 2: The young carer s voice KPIs: (1) Nos. of young carers involved in consultation activities (2) Membership/frequency of meetings of new YC Steering Group (3) Nos. of young carers accessing information or giving feedback via social media Co-design school holiday programmes with young carers so they have a say about activities on offer Explore the use of apps to get young carer feedback on the impact of our services Continue with current processes of obtaining informal feedback and promoting discussion around planning at clubs and activities. Target: Pilot the use of social media as a means of consulting young carers To research how other young carers services use social media Target: carry out a period of research between Sept- 15. Agree pilot for Jan 16 Ongoing Recruit and launch a steering group specifically to develop our support to young carers Increase our use of social media and text alerts to advertise consultation activities for those carers that would like to get involved To hold a series of meetings to recruit relevant participants to a new steering group for young carer services (Sept-15) Target: To hold an inaugural meeting In January 16 with a small core group of 5 participants. Targets: To pilot a texting service for notification of trip details to young carers parents To produce a new design Young Carers newsletter. To use Facebook as a means of sharing information and obtaining feedback. Jan
