This formulary has been developed and reviewed by the HCT Bladder and Bowel Service, in conjunction with East and North Hertfordshire CCG and Herts Valley CCG, for use in the community by community nurses and General Practitioners (GPs). Product choices have been based on experience, clinical effectiveness, patient requirement and cost effectiveness. This formulary will be updated annually to reflect innovations in practice and new, evaluated products. Users of the formulary are advised that: Patient assessment is essential before deciding to catheterise; Alternative management options should be first considered because of the complications associated with indwelling urinary catheters; Product choice should be determined by clinical assessment, however patients/carers should be given the opportunity to make informed choices and be part of the decision-making process; Patients should be regularly reviewed to confirm continued need, and/or assess changes, taking into account new products and developments which may better suit the patient s need, in accordance with the formulary; Requests for non-formulary items must be clinically justifiable and may be challenged; The minimum quantity required to meet patient needs should be ordered to avoid stockpiling. Requesting Prescriptions Prescriptions must be authorised and generated by a GP practice/independent Non-medical Prescriber (NMP) prior to any delivery of appliances to patients. HCT community nursing staff requesting prescriptions to be issued by patients GPs for continence products must use the Incontinence Appliance Request Form (Annex 1). Ordering Supplies Patients must be given the choice of where to obtain their prescribed continence supplies from. One supplier should not be recommended in preference to another. There are two routes of supply: 1. Community Pharmacies prescriptions can be collected by the patient or a patient s representative from the GP surgery and items delivered directly to the patient s home, along with other medication. Stock will be delivered within 24 48 hours in most instances (NB not all pharmacies offer a delivery service to patients). 2. Dispensing Appliance Contractors (DAC) prescriptions can be posted from the GP surgery to the DAC and items delivered directly to the patient s home. Stock is usually delivered within 24-48 hours.
