Grant Prpsal Willamette Dental Grup (WDG) was awarded $117,024 as part f Trillium s Transfrmatin Grant Prgram: The Chrnic Cnditin Dental Management Prgram (CCDM) will manage dental disease in Trillium members with high r extreme dental risk due t tbacc use r diabetes diagnses using evidence-based dentistry. This prject presents a unique pprtunity t serve members in a truly transfrmative manner by strengthening linkages between medical and dental care, and experimenting with alternative payment methdlgy.
Prject Cmpnents: Funding was used fr: Persnnel train and imbed in ffice care crdinatrs knwn as Care Advcates Clinical prducts Evidence Based Dentistry prducts nt cvered under OHP: 1)Chlrhexidine an anti-bacterial muth rinse 2)Clinpr a prescriptin-strength fluride tthpaste 3)Xylitl a natural sugar substitute Shared savings fr members wh shw imprvement in their ral and verall health Administratin prgram peratins cst (mnitring, reprting, infrmatin systems, utreach materials, etc.) Only $52,918.22 was used due t the limited number f eligible members available fr this prgram.
Care Advcates: The WDG Care Crdinatr Rle WDG embedded care crdinatrs within ur dental hmes. These Care Advcates were identified and selected frm current FTE psitins that had custmer service and patient engagement as central tenets f their rle. Each Care Advcate received speciallydesigned WDG training predicated n whle persn health and behaviral/mtivatin techniques. A Care Advcate s respnsibilities include the fllwing: Prvide extensive in-ffice educatin s patients fully understand their treatment Make fllw-up calls with patients Develp relatinships which lead t higher patient engagement Cntact patients t encurage participatin Make persnal reminder calls Inspire the entire ffice team t rally arund crdinated care Fllw up with patients as needed t ensure cmpliance Create linkages between medical, dental, mental as necessary Track cntact with patients

Peple with peridntal disease are almst twice as likely t have heart disease. Peple with diabetes are mre likely t have gum disease, making it difficult t stabilize bld sugar levels. Treating gum disease can have a psitive impact n bld glucse. Pregnant wmen wh maintain gd ral health see a psitive impact n their health and their babies health. Patients taking asthma medicatin may be at risk f dental caries, dental ersin, peridntal diseases and ral candidiasis, s thse n medicatin shuld receive preventive dental care. Treating gum disease reduces inflammatin, which can reduce the risk f kidney failure. Peridntal disease may increase the risk fr respiratry disease, including COPD.
