Primary Care, CDM and Public Health HEALTHY AGING RESOURCE TEAM Programs, Educational Presentations and Focus Clinics The Healthy Aging Resource Team is a team of Public Health Nurses and Occupational Therapists working in your community. We provide health and wellness programs, presentations, and clinics focusing on a number of key topics related to healthy aging. Our target audience is primarily the older adult population. Listed below are the programs, presentations and focused clinics we currently have available. DEFINITIONS Presentation: Focused clinic: Consultation: Program: an educational session which includes a power point presentation with group activities, brainstorming and problem-solving, questions and answers. includes an educational presentation related to a specific topic, plus self-assessment of associated risk factors, and individual follow-up as required; assessment may be done in a group setting or as a consultation. private, 1:1 assessment conducted by the Public Health Nurse or Occupational Therapist, either on the same day as the focused clinic or at another arranged date and time. a series of consecutive 2 2 ½ hour sessions, running once per week for 2 to 6 weeks, depending on the program. TOPIC: ARTHRITIS Arthritis Clinic TYPE: Focused clinic LENGTH: 1-1.5 hour presentation with 15-30 minute consultation PRESENTER: Public Health Nurse Learn about the nature of arthritis Explore how you can live well with the disease Have an individual assessment with follow-up as needed Referral to occupational therapist for individual assessment and consultations. ADVERTISEMENT: Do you or someone you know have arthritis? Attending this focus clinic will help you gain a better understanding of the different types of arthritis, as well what you can do to live well with Arthritis. TOPIC: BONE HEALTH Stand Up to Osteoporosis LENGTH: 1to 1 ½ hours (Part 1 = 30 minutes; Part 2 = 1 hour) Have increased awareness of the osteoporosis disease process Be aware of risk factors for bone loss Learn how to prevent and treat osteoporosis Be able to recognize that it is never too late to take steps to strengthen bones and prevent fractures ADVERTISMENT: Could you be at risk for osteoporosis (bone disease)? Have you been diagnosed with osteoporosis? Stand up to osteoporosis by taking steps now to reduce your risk for broken bones! TOPIC: CANCER Common Cancers in Canadian Older Adults 2010 TYPE: Presentation LENGTH: 1-1 ½ hours Learn what the most common cancers are people over 50 years of age Recognize the early warning signs and risk factors for each type of cancer Learn what the test and routine screens are for each type Learn what self-care actions you can take to reduce your risks ADVERTISEMENT: Are you or someone you love at risk for cancer? Come learn what the most common types of cancers are in people 50 and older, the routine tests and screens, and how you can reduce your risk for developing cancer. TOPIC: CHRONIC DISEASE SELF-MANAGEMENT TYPE: LENGTH: Better Choices Better Health Program - fun, practical, free 6 week program to help you live with chronic conditions 2 ½ hour sessions, once per week for 6 consecutive weeks; 16 participants maximum per class PRESENTERS: 2 Co-facilitators Learn to overcome daily challenges, and maintain an active, fulfilling life Learn healthier ways to live, gain confidence and motivation Build a toolbox full of information, strategies and techniques to help yours ADVERTISMENT: BETTER CHOICES BETTER HEALTH is a fun and practical workshop which can help people with chronic conditions overcome daily challenges, and maintain an active, fulfilling life. Participants (which include any support people who will be accompanying the individual with chronic disease) learn about healthier ways to live, gain confidence and motivation to manage their health, and feel more positive about their lives. They are provided with information, strategies and techniques, so they will have the tools they need to help themselves. TOPIC: DIABETES Diabetic Foot Clinic LENGTH: 1 1 ½ hour presentation with 15-30 minute consultation PRESENTER: Public Health Nurse Learn how to best care for your at risk diabetic feet Consultation includes neuropathy screening, foot assessment and treatment recommendations Referral to occupational therapist for individual assessment and consultations. ADVERTISEMENT: Do you have diabetes? Do you know how diabetes can affect your feet? This focus clinic will provide the most up to date information on the daily care of your feet and includes an individual foot assessment with treatment recommendations. PRESENTERS: 2 Co-facilitators Learn about the nature of diabetes and explore ways to take care of yourself in order to enjoy a better quality of life! Lower your HbA1c with Type 2 diabetes, and reduce your diabetes symptoms Build a toolbox full of information, strategies and techniques to help yourself manage your diabetes and enjoy a better quality of life ADVERTISEMENT: The BETTER CHOICES BETTER HEALTH - Diabetes Self- Management Program was developed at Stanford University Patient Education Research Center and was found to lower the HbA1c of people with type 2 diabetes as well as decrease their diabetes related symptoms. The goal of the program is to help participants (which include any support people who will be accompanying the individual with chronic disease) learn more about diabetes, and explore ways to take care of themselves. TOPIC: EYE HEALTH Eyes and Aging LENGTH: 1-1 ½ hour PRESENTERS: Public Health Nurses Learn about the most common diseases affecting eyes with advancing age Learn best practice recommendations for treatment and screening for these common diseases Learn the difference between an ophthalmologist, optometrist and optician Learn what you can do to be safe with low vision. ADVERTISEMENT: What can I expect from my eyes as I age? What can I do about it? Who can help me with my eye concerns? This focus clinic covers the most common diseases with aging, as well as normal age related vision changes and what you can do to be safe with low vision. The second part provides vision screening and can help you figure out how to get the help you need. TOPIC: FALLS PREVENTION Steady As You Go [SAYGO] TYPE: Program a free interactive, 2 part program - presentation and individual risk assessment; handbook and exercise video provided LENGTH: two, 2-hour sessions, 3-4 weeks apart [and Senior Facilitator when available] Learn about the risk factors for falling as we age Assess your individual fall risks Learn ways to reduce your risk for falling BTheroux - AHS Seniors Edmonton Zone -revised 19June2012 4

Come join the program and learn how to assess and reduce your risk for falling. Finding Balance LENGTH: 1-1 ½ hours Learn about the nature of adult falls Explore ways to reduce risks for falling as we age ADVERTISEMENT: Falling is not a normal part of aging. Do you want to live a healthy and independent life? Come learn positive steps that you can take to minimize your risks and prevent a fall before it happens. TOPIC: HEALTHY EATING Eat Well Live Well LENGTH: 1-1 ½ hour presentation with 10-15 minute consultation Acknowledge the benefits of healthy eating and active living in your life. Become aware of risk factors for poor nutrition and its effects on your life. Gain skills to monitor your nutritional health over time. Become familiar with recommendations for healthy eating and active living as we age. Learn where to get additional information about healthy eating and active living. Our nurses will help you to identify areas where you can improve your health by making realistic changes in your life. TOPIC: HEALTHY AGING Your Inner Fountain of Youth - designed for the middle old to oldest old population LENGTH: 1-1 ½ hours Learn that there are positive changes associated with aging Learn ways to develop resiliency the key to successful aging. Explore the effect of aging using a wholistic approach, and learn ways to help you maximize your golden years. Healthy Aging the young old LENGTH: 1-1 ½ hours presentation Learn about the 3 components of ourselves: mind, body and spirit Develop an awareness of how each contributes to healthy aging Learn ways to maintain physical and mental health, avoid disorders, remain active and independent ADVERTISEMENT: What is really important to you as you age? TOPIC: HEART HEALTH Hypertension and Stroke LENGTH: 1-1 ½ hour presentation with 10-15 minute consultation PRESENTER: Public Health Nurse [with assistance from another PHN] Learn what high blood pressure and stroke are Learn about the risk factors for developing high blood pressure Gain strategies to decrease your risk for high blood pressure and stroke Consultation includes blood pressure assessment, referral and follow-up as needed ADVERTISEMENT: Do you want to know what those blood pressure numbers mean and what can you do to help keep your blood pressure in a healthy range? Learn more about reducing your heart and stroke risks. TOPIC: INFECTIOUS DISEASES Preparing for Influenza and Cold Season LENGTH: 1-1 ½ hours Learn the difference between bacteria and viruses Learn the nature of common infectious diseases Explore ways to prevent the spread of diseases Learn proper hand washing technique Learn self-care should you get sick ADVERTISEMENT: Influenza and the common cold are infectious diseases easily spread from person to person. We can stop the spread! Not all bugs need drugs! Come learn practical ways to prevent spreading these diseases, take care of yourself, and your love ones should you become ill. ADVERTISEMENT: A memory enhancement program for adults, age 50 and over, who are interested in addressing their concerns around memory in relation to normal aging. Classes are held once a week for six weeks, 2 hours each week. Workshops cover topics on how memory works, normal changes in memory due to aging, memory boosting strategies, memory training techniques to help with remembering, and lifestyle factors that affect memory such as nutrition, physical activities, sleep, medication and stress. Improving Your Memory LENGTH: 1-1 ½ hour PRESENTERS: Public Health Nurses Learn how our memory works Discuss factors which affect memory as we age Learn strategies to maximize your memory ADVERTISEMENT: Are you concerned about your memory? Do you think you are the only one that forgets? Come and learn how your memory works. Explore various strategies that will help you to maintain optimal brain functioning. We will also briefly examine causes of memory decline and age related changes. TOPIC: MENTAL HEALTH Beating the Blues Understanding and Preventing Depression LENGTH 1-1 ½ hours Be able to describe what depression is, Understand how depression differs from dementia and delirium, Learn the common symptoms, causes and risk factors Learn how to prevent depression Learn the treatment options and strategies for living a healthy life. ADVERTISEMENT: Commonly, people associate depression with feeling down or blue. Everyone feels down every so often, but is usually temporary. Depression is when these feelings last for a long period of time and begin to interfere with your everyday life. Come and learn about depression in older adults, the symptoms, causes, and risk factors. Explore ways to prevent and treat depression, and live a healthy life. Become aware of stress and how it affects you Understand healthy lifestyle options that help manage stress Learn relaxation techniques and other ways to manage stress ADVERTISEMENT: Feeling overwhelmed? Unable to find time for yourself? Come and learn about how stress affects you, how to achieve balance in your life and cope with everyday stressors. Discover relaxation techniques and other ways to enjoy life. Classes are held once for 2 hours a week for 5 weeks. What s Bugging You? LENGTH: 1-1 ½ hours PRESENTERS: Public Health Nurses Learn what stress is, and how it affects you Learn ways to recognize when you are stressed Discover practical ways to effectively manage your stress ADVERTISEMENT: We all have experienced stress. Sometimes it is good, and other times we feel out of control. Come learn how to recognize when you are stressed. Explore practical ways of effectively taking control and managing the stress in your life. TOPIC: MOBILITY Walker Clinic LENGTH: 1 1 ½ hour presentation with 15-30 consultation PRESENTER: Occupational Therapists Understand different types of mobility aids Learn how to use mobility aids safely and effectively Spot maintenance, repairs and potential replacement Receive on-site maintenance & repairs Have individual follow-up as needed ADVERTISEMENT: Does your walker need maintenance, repairs or replacement? Do you use your walker safely? This focus clinic includes a 1hour presentation about use and care of mobility aids and a 1:1 follow-up with FREE onsite repairs and co-ordination of off site repairs and/or replacement if required. This focus clinic starts with a 1 1.5 hour presentation on the most up to date information about your body s pain system, and what you can do to manage your pain. The second part is a private consultation with an occupational therapist and a registered nurse to help you make a plan to manage your pain effectively.
