As an organization, Lahey Health embraced the American Hospital Association s call to action for all hospitals to be leaders in creating a culture of health and for healthcare organizations to serve as role models for their patients. We recognize that in order to be a leading organization in creating such a culture, we are dependent upon the choices that each one of us makes every day for our own personal health and well being. We support each other along the way with our collective resources. This is how we all will continue our journey to Live Better. This newsletter provides details about our 2016 Live Better wellness program. Your privacy is important to us. That is why the wellness program will continue to be completely confidential. No individual health data will be shared with Lahey Health. Complete details of the Lahey Health benefit plans are included in the official plan documents. If there is any difference between the information presented in this newsletter and the official plan documents, the plan documents will govern. This statement does not constitute an employment contract, nor does it provide a guarantee of future employment. How to earn the 2016 wellness incentive To earn the 2016 wellness incentive, colleagues and spouses may choose to complete some or all of the steps below, with each step earning a portion of the incentive. If all three steps are completed, the total available incentive is earned. Step 1: Know your numbers. Participate in our free, onsite biometric screenings to help identify key health measures. Step 2: Take the On-Line Health Questionnaire (HQ). Colleagues and spouses who take the HQ will receive a personalized health profile. Step 3: Meet three or more of the health measures. More details about these measures are on the following pages. Step Earn Incentive to be applied to your 2016 Lahey Health medical plan premium 1. Colleague participates in the biometric screening...$100 2. Colleague completes the health questionnaire...$100 3. Colleague meets three or more health measures... $200 Colleague total available if all three steps are completed.... $400 1. Spouse participates in the biometric screening...$50 2. Spouse completes the health questionnaire...$50 3. Spouse meets three or more health measures...$100 Spouse total available if all three steps are completed... $200 Potential Grand total for colleague and spouse.... $600 What do the measures mean to you? If you and/or your spouse participated in our biometric screenings last year, compare your results to these health measures. If you and/or your spouse are already meeting at least three out of the five, congratulations! Continue to stay on track for similar results this year to meet step 3. If you and/or your spouse have room to improve, now is the time to learn how you can positively work to improve your health by making healthy lifestyle changes. We know success does not necessarily happen overnight and that achieving some of these health measures will require significant lifestyle changes for some colleagues and their spouses. That is why the health measures you choose to focus on are up to you. Successful achievement of the health measures will be determined with your biometric screening results this fall. Health Measures The health measures outlined below will mean different things for different colleagues. Some of us already meet the measures today, so efforts will continue to focus on maintaining good habits. Others might consider making certain changes in nutrition or physical activity. Wellness is personal how you embrace this opportunity to Live Better is completely up to you. Our health measures are established by our clinical leaders. They take into account national clinical changes along with what we learned about the collective Lahey Health colleague community from previous screenings (remember, we do not have access to individual data). The collective data showed these are important risk areas for our colleagues. Biometrics Health Measures (Measured During Fall 2014 Biometric Screening) Body Mass Index (BMI) BMI range: 19 through 25, or If you participated in the fall 2014 biometric screening and: Your BMI was greater than 25, up to and including 26, and in 2015, you have a BMI between 19 and 25; or Your BMI was greater than 26, up to and including 31, and in 2015, you have at least a one-point drop from your fall 2014 biometric screening result; or Your BMI was greater than 31, and in 2015, you have at least a two-point drop from your fall 2014 biometric screening result. Blood Pressure If you are under age 60: Systolic (top number) less than 140 AND Diastolic (bottom number) less than 90 Non-fasting Cholesterol If you are age 60 or over: Systolic (top number) less than 150 AND Diastolic (bottom number) less than 90 By having your cholesterol taken at the screening, you meet this health measure. You are encouraged to speak with your physician about this result. Non-fasting Glucose (blood sugar) less than 126. Glucose (Blood Sugar) Nicotine Participate in a screening for Nicotine. Live Better: The Lahey Health Wellness Program Questions and Answers We know you may have questions about Live Better and what the health measures mean to you. Here are some answers to get started. What if I did not participate, or my spouse did not participate, in 2014? If you or your spouse did not participate in our wellness program this past fall, you and your spouse still have the opportunity to earn a wellness incentive toward your 2016 annual Lahey Health medical plan premiums. Here is how: 1. Participate in a 2015 onsite biometric screening and complete the confidential online health questionnaire. Later this year, you will receive information with dates, times and locations. Be sure to complete both of these steps by the deadline. 2. Meet three or more out of the five health measures. To meet the BMI health measure, you will need to be in the range of 19 through 25 since you do not have baseline data from the 2014 biometric screening. Get ready by taking advantage of Lahey Health wellness resources during the year, such as an onsite fitness center, Weight Watchers program or attending a Harvard Pilgrim seminar. 6 Live Better: The Lahey Health Wellness Program Summer/Fall 2015
Is there any personalized help available? Starting a big or small lifestyle change can be difficult. Even with all of the great resources available to us, sometimes we need a little extra motivation to take the first steps to Live Better. That s why Lahey Health colleagues and their covered family members enrolled in our medical plans have access to a health coach through Harvard Pilgrim at no cost. This can help you get that boost of support and motivation that you need. Interested? Call Harvard Pilgrim at 877.594.7177 to have a health coach contact you. What do I need to do to complete my health questionnaire (HQ)? You will need the user name and password sent to you in Harvard Pilgrim s summer mailing. Spouses covered under your health plan will receive their own packet with their own user name and password. If your spouse plans to participate in the Live Better wellness program, but isn t covered under your health plan, please have your spouse email us at HPWellness@ harvardpilgrim.org to get a personal user name and password. How else can I use my health questionnaire? Completing your confidential, online health questionnaire (HQ) gives you a handy personalized wellness profile. Do not keep the information to yourself! Be sure to bring a copy to your next doctor s appointment so you can review it with your primary care provider or with a specialist you see for your care. Your results can be a great conversation starter about your health and lifestyle, so that you can work with your provider to make a plan to stay healthy or make improvements.
