Tutorial Objectives After completing this tutorial, you will be able to: Understand the purpose of the RADAR tool Access and utilize the RADAR Report View and interpret Profile Scores This tutorial will take about 20 minutes to complete. 3
RADAR Report Overview RADAR is a Web based, resident level clinical and predictive risk management tool. RADAR includes: Resident Profile Scores Quality Measures (QM) Alerts Resident historical data presented from MDS history 4

RADAR Benefits: Provides a high level overview of all current residents Identifies residents with clinical and risk concerns Drill down for more details - Risk management - Care planning MDS 3.0 scales replace other required assessments Excellent tool for Administrators and Managers 5
Steps for Using the RADAR Report The following slides will guide you through the steps for using the RADAR Report: Step One: Login to PointRight Step Two: Access the RADAR Report Step Three: Drill down for details 6
Step One: Login to PointRight 1. Go to: www.pointright.com 2. Click Client Login 7

Step One: Login to PointRight (cont.) 4. Enter your Log In information User Name Password The password is case sensitive! 5. Click Log In. 9
Step Two: Access the RADAR Report Roll the mouse icon over the Products tab. The Products drop down menu displays. In the Products drop down menu, click RADAR. The RADAR screen displays. 10
Step Three: Drill down for Details The RADAR Report displays Profile scores so that you see at a glance what occurs with each resident. 11
Step Three: Drill down for Details (cont.) The RADAR Report displays links to the most recent MDS assessment and MDS history. Click on an ARD to view the most recent MDS assessment. Click on a resident s name to view the resident s MDS history. 12

Using the RADAR Report (cont.) Click a column header to sort by that column Click once to view in descending order. Click twice to view in ascending order. The view you select will remain the active view. 14
Profile Scores Profile Scores are based on two categories of MDS scales: Descriptive scales and Predictive scales. 15
Profile Scores (cont.) Descriptive scales identify the condition or outcome, and the severity of the condition. These scales focus on resident characteristics for ADLs, Cognitive Performance, Delirium, Mood, and Pain. 16
17 Profile Scores (cont.) Predictive scales identify specific risk factors that contribute to a resident developing a particular condition or outcome. These scales predict the likelihood of an event within a specific period of time for Falls, Frailty, Hospitalization, and Pressure Ulcers.
Profile Scores (cont.): Risk Profile Probability Tables To see additional information about RADAR s Predictive Scales, please see the RADAR Risk Profile Probability Tables. This information can be viewed and printed from the RADAR section of the Training page on the PointRight website. 18
Profile Scores (cont.) Roll over a Risk Profile name to view information about that scale, including scale range and the high risk/high impairment cut point. Click on a resident s score to view resident history and details. Click a resident s score to drill down 19
Historical Trending & Scale Graphs The Hospitalization Risk scale indicates a resident s likelihood of being rehospitalized. This example indicates that now may be a good time to address changes in the resident s condition in the care plan to prevent rehospitalization. 20

Risk Scale MDS History The MDS Assessment History displays below the trend graph. Roll over any MDS item header to see the coding details. Click on the ARD to see the MDS assessment. Use the scroll bar to view all MDS items 23
QM Alerts The QM column displays the CMS Quality Measures that will potentially trigger on the resident s most recent MDS. Click the QM Alert link to view the triggers. 24
QM Alerts (cont.) The QM triggers for the most recent MDS will appear in a new browser window. NOTE: if the resident is in the Long Stay sample you will need to scroll down to view the triggers. 25
QM Alerts (cont.) Click the Y link to view the resident s MDS History for that measure. MDS History will display in a new browser window. 26
Printing RADAR 1. Roll the mouse icon over the Print tab, and select Print Report from this Page 2. View RADAR as a printable report 3. Click the Print button in the report window to print. 27
Tips for Success Sort by most recent ARD to bring current issues to IDT meetings Helps your team focus on most current clinical issues Use changes in QM Alerts and Profile Scores to guide care planning and risk management decisions Drill down to identify specific risk factors Use Profile Scores to screen residents for rehab/restorative, pain management or palliative care programs 28
