Loban' HIGHER STATE EDUCATIONAL ESTABLISHMENT OF UKRAINE "UKRAINIAN MEDICAL STOMATOLOGICAL ACADEMY", POLTAVA, UKRAINE A B S T R A C T In tr o d u c t io n : The present article deals w ith th e role o f the oral mucosa colonization resistance in th e im p le m e n ta tio n o f th e influence o f individ u a l-typ o lo g ic a l characteristics o f the individual susceptibility to caries and inflam m atory periodontal diseases. T h e a im : to study th e role o f co lo nizatio n resistance o f th e oral mucosa in th e im p le m e n ta tio n o f th e influ ence o f in d ivid u a l-typ o lo g ic a l characteristics o f th e individual susceptibility to caries and inflam m atory periodontal diseases. M a te r ia ls a n d m e th o d s : The studies were conducted on 182 medical students 19-29 years old. All the examinees had a screening assessment o f the oral mucosa colonization resistance by our own m ethod, which received a patent UA 51373. C o n c lu s io n : It is shown th a t psychophysiological features o f healthy individuals and patients w ith dental caries and g in g iv itis d e te rm in e th e level o f resistance to caries and periodontal in fla m m a to ry diseases, deviation o f pe rso nality p rofile tow ards high neuroticism is a risk factor fo r caries and g in g iv itis. It is reported th a t th e highest resistance to caries and in fla m m a to ry periodontal disease had e m o tio n a lly stable introverts, in fla m m a to ry periodontal disease are m ore com m on in e m o tio n a lly unstable introverts. The authors found th a t oral colonization resistance is a predictor o f th e risk o f caries and in fla m m a to ry periodontal diseases. Reducing o f th e oral cavity colonization resistance most commonly diagnosed in em otionally unstable introverts, that correlated w ith their low resistance to inflam m atory periodontal diseases. Wiad Lek 2017, 71, 4, 754-757 INTRODUCTION Preservation of public health and an increase of its level rem ains an urgent problem worldwide. D espite considerable specialization in m edicine, that contributed to intensive development of its individual areas, a search for new solutions to this complex and multifaceted problem is ongoing now [1,2,3]. Active research on the fundamentals of dentistry involving biom edical sciences is essential in terms o f m ounting understanding of the im portance of fundamental science in m edicine [1]. According to W H O, the problems o f caries and periodontal diseases rem ain unresolved by m odern medicine, but their topicality is determ ined by the high demand of the population for effective treatm ent of these diseases. Accumulated experience in recent decades shows that the leading role in the occurrence o f dental caries and periodontal inflam m atory diseases belongs to m icrobial and im mune shifts occurring in the mouth, on the background of genetic predisposition [4,5,6]. Oral cavity represents an ecological system, inhabited by various microorganisms that are combined into a biofilm. Resident microflora as an integral part of this microecosystem, together with the oral mucosa performs barrier-protective function and ensures its colonization resistance. Colonization resistance is one of the mechanisms of local immunity that depends on a combination of factors that prevent the adhesion and growth of bacteria on the mucous membranes. A role in this belongs to the resident microflora, which is an antagonist of bacteria, optional for the same ecological niche, and the epithelial cells of the oral cavity. The antagonistic effect of the normal microflora is carried through a large adhesive and colonizing ability of resident species of microbes, as well as the production of specific substances - bacteriocins and antibiotics that inhibit the growth of pathogenic microorganisms [7,8]. Understanding of the causes of pathological processes is closely linked to obligatory consideration o f the role of the body in causing the disease. The same factor may be pathogenic to one organism and not cause pathological changes in the other. The causative agent does not just act on the body but there is their peculiar interaction. In the occurrence o f the disease the properties of the factor and the whole organism are important. It is believed that 754

The intensity o f changes in the activities o f various organs and systems depends on individual-typological character istics o f the person and his/her tem peram ent. That s w hy a special attention is paid to personal predisposition o f the form ation o f psychosom atic disorders. A great num ber o f works are devoted to the study o f individual typological characteristics o f the patients with dental pathology [1,10,11]. But the role o f colonization resistance o f oral personal predisposition to the develop m ent o f caries and inflam m atory periodontal disease has n o t been studied yet. THE AIM To study the role o f colonization resistance o f the oral m u cosa in the im plem entation o f the influence o f individual -typological characteristics o f the individual susceptibility to caries and inflam m atory periodontal diseases. C RI 2 points indicates overvoltage barera colonization, a quantitative increase in microorganisms, which may include n ot only sym biotic but also pathogenic ones [7,8]. Psychological testing included determ ination extraver sion - introversion and neuroticism level using Eysenck q u estio n n a ire [1 4 ]. In terp reta tio n s o f L.A.U lyan ova s in terp reta tio n th e in d ica to r o f ex tra-in tro v ertiv en ess w ithin 1-13 points indicates introvertiveness, 14-24 points - ekstravertivness. The level o f neuroticism up to 12 points is characterized as low, such individuals are em otionally stable. Em otionally unstable individuals have the level o f neuroticism m ore than 12 points [14]. Statistical analysis o f the results was perform ed using SPSS 17.0 software and M icrosoft Excel 2003. The overall sample was analyzed by p aram etric m ethods after pre lim inary check for norm al distribution using the Kolm ogorov-sm irnov test. The differences betw een the studied param eters were evaluated by Student test. Identified fre quency in individual groups were com pared by y-square to determ ine the reliability o f their differences. RESULTS AND DISCUSSION MATERIALS AND METHODS The studies were conducted on 182 m edical students aged 19-29 years, o f w hich the control group consisted o f 22 people (11 m en, 11 w om en) who had no lesions o f hard tissue o f teeth and periodontal disease. The groups that were studied were form ed on the basis o f dental status: 1-103 people (53 m en, 50 w om en) with the defeat o f hard tissues o f teeth with caries and periodontal intact; 2-57 patients (29 m en, 28 w om en) who were diagnosed with ch ronic catarrhal gingivitis. A conventional dental exam ination was conducted with determ ining D M F index, hygiene index o f G renn-v erm ilion (O H I-S), PM A in the m odification o f S.Parm a, M uhle m ann index, M uhlem ann-saxer index (P B I), interdental H I (H YG ), com prehensive periodontal index (C P I) [12]. All the examinees had a screening assessment o f coloni zation resistance o f the oral m ucosa by our own methods, w hich received a patent UA 51373 [13]. Colonization resis tance index (C R I) 1 point characterizes a high level o f colo nization resistance o f the oral mucosa. C RI 0 point indicates the oppression o f the barrier o f colonization resistance and lowering antagonistic properties o f the norm al m icroflora. According to the results o f clinical exam ination o f patients D M F index in patients with caries was 5,74 ± 0,26, in people suffering from catarrhal gingivitis - 5,79 ± 0,35. In patients with chronic catarrhal gingivitis PM A was 26,5% ± 0,92, and gingival bleeding indices o f M uhlem ann that M uhlem ann-saxer (PB I) - 1,39 ± 0,05 and 0,87 ± 0,03 respectively. Test results using Eysenck s questionnaire m ade it possi ble to identify individuals who were referred to extraverts, introverts, em otionally stable and em otionally unstable in dividuals. Frequency o f psychophysiological characteristics o f patients with different dental status are shown in table I. E x tra in tro v ertiv e n ess is a fa irly stab le p e rso n a lity ch aracteristic. A ccord ing to our data the frequ en cy o f the incid ence trait in the patients w ith gingivitis is n ot significantly different from the control group. In patients suffering from caries the frequency characteristics o f extro- introvertiveness differed from the control group and were characterized by slight predom inance o f extroverts. A lthough we could n o t identify significant differences o f average indicator o f extro- introvertiveness in the groups, they accounted for the control group 13,6 ± 0,77 points, for patients w ith caries - 13,8 ± 0,34 points, for patients w ith gingivitis - 13,9 ± 0,68 points. 755

H.Eysenck found that introverts have a higher level o f activity o f the cerebral cortex. Extroverts com pensate this lack by activation o f add itional m ovem ents, increased attention to external signals, introducing diversity in any m o n o to n o u s situation. In troverts and extro verts have different style o f intellectual activity [15]. By frequency characteristics o f neuroticism study groups differed significantly. If in the control group em otionally stable individuals prevailed by 45,4%. They are ch arac terized by properties such as stability o f sthenic em otions and em otional arousal w hen exposed to various stresses, m aintaining an optim istic m ood, lack o f fear, abashm ent and depression. In the group o f patients w ith caries em o tionally unstable individuals prevailed by 7,8%. As our research has shown, a high level o f neuroticism is a risk factor for the development o f inflam m atory periodon tal diseases. For example, among patients with gingivitis, em otionally labile individuals were by 40,4% m ore frequent. They were characterized by the predom inance o f negative em otions, excitem ent, apathy, exhaustion o f the nervous system, resulting from overly severe or prolonged mental stress, not a full restoration o f balanced forces. Individuals with severe neuroticism were em otionally unstable and they were characterized by predom inantly non willed but em o tional regulation. Individuals with a high level o f neuroticism were significantly more likely (than the control group and the group o f patients with caries) to develop inflam m atory peri odontal disease. Average values o f the level o f neuroticism in the control group amounted to 9,95 ± 0.84 points, in the group o f patients with caries - 12,98 ± 0,41 points (p<0,05), in the group o f patients suffering from gingivitis - 13,60 ± 0,48 points (p <0,05). Taking into acco u n t extro-in trovertiven ess and level o f neu roticism, we d istributed all patients by indidual typological features o f personality. A ccording to Eysenck, em otionally unstable extrovert corresponds to ch oleric tem peram ent, stable extrovert to sanguine, unstable in tro vert to m elancholic and stable introvert to phlegm atic [15]. According to our data, the ratio o f personality types in each o f the treatm ent groups differed significantly (Table II). 756 The study o f the frequency profile o f the control group showed a high resistance to caries and periodontal inflam m atory diseases o f em otionally stable introverts. Gingivitis m ost often develops in em otionally unstable introverts (melancholic). In the group o f patients with caries, caries resistency was inherent to emotionally stable introverts (phlegmatic). The individuals o f choleric, sanguine and m el ancholic temperament types were equally susceptible to caries. The causal lin k betw een psychophysiological ch arac teristics o f an individual and som atic diseases, including dental are mediated, ambiguous and don t fit into a basic scheme. Caries and inflam m atory periodontal diseases are multifactorial diseases, the foundation o f which supports the development o f com plex pathological changes in the m icro ecology o f the oral cavity. Research by dom estic and foreign scientists showed that the basis m icroecological disorders is depression o f colonization resistance [16]. In the case of reducing the num ber o f colonization resistance, the range o f potential pathogens increases and expands largely deter m ines the risk o f development and progression the diseases o f the hard tissues o f teeth, periodontal and oral mucosa [16]. The analysis o f microecological disorders o f the oral cavity in young people showed that cytological smears surveyed in terms o f colonization resistance (CRI) were different. In the control group incidence ofc R I 1 point in cytological smears was 68,2%, whereas C RI 0 points were observed only in 31,8% ofindividuals and CRI 2 points were not recorded. W ith the development o f caries and gingivitis, an increase in individuals with CRI 0 points and 2 points, and respectively, a decrease o f persons with CRI 1 point was observed. In particular, individuals with caries incidence o f CRI 0 points and 2 points were 68,0% in patients with gingivitis - 100%. Frequency characteristics of CRI 1 point for these groups was respectively 32,0% and 0%. The obtained results show a decrease in the colonization resistance barrier o f the oral mucosa with dental caries and gingivitis (Table III). The results suggest that patients with dental caries, which are dom inated by em otio n ally unstable introverts and extroverts in 68,0% o f the people, here was a reduction o f colonization resistance o f the oral m ucosa. The study o f colonization resistance o f the oral m ucosa in the groups o f people that differ in individual-typological characteristics o f the individual, has revealed that m ost low resistance to colonization by pathogens was observed in em otionally unstable introverts (m elan ch olic). Thus, 80,0% o f the em otionally unstable introverts had reduced colonization resistance o f the oral cavity. A m ong em otion ally unstable extroverts the figure was 71.4%, am ong em o tionally stable extroverts - 72,1%, and am ong em otionally stable introverts - 68,6%. The desire to research the causal relationship betw een som atic and psychic factors in som atic clinical diseases, including dental, and to associate them w ith the person ality o f the patient, is called a psychosom atic approach in m od ern m edicine. Systemic clinical manifestations o f dental caries and peri odontal inflam m atory diseases are associated with tension o f regulatory m echanism s. The specific differences in the reactions o f the whole organism m anifestation o f the m ajor dental diseases outline the basic m orphological, functional and physiological com ponents o f each functional type o f constitution [1,17,18]. A ccording to M.I.K lenovskoy [19] em otionally labile introverts (m elancholic) are character ized by the predom inance o f sym pathetic nervous system and the high susceptibility to stress factor. These regulatory m echanism s are likely to affect the functional properties o f the buccal epithelial cells, local im m unity and m icro ecology o f the oral cavity. Thus, th e p ro g n o stic and cu rren t assessm en t o f the em ergence and development o f dental caries and periodon tal inflam m atory diseases, the form ation o f risk groups should be carried out taking into account the individual typological characteristics o f the organism and the level o f colonization resistance o f the oral cavity. CONCLUSIONS Psychophysiological features o f healthy individuals and patients with dental caries and gingivitis determ ine the level o f resistance to caries and periodontal inflam m atory diseases. Deviation o f personality profile towards high neuroticism is a risk factor for caries and gingivitis. The highest resistance to caries and inflam m atory periodontal disease have em otionally stable introverts, inflam m atory p erio dontal disease are m ore com m on in em otionally unstable introverts. O ral colonization resistance is a predictor o f the risk o f caries and inflam m atory periodontal diseases. Reducing o f colonization resistance o f the oral cavity is th e m o st co m m o n in em otio n ally unstable introverts, that correlates w ith their low resistance to inflam m atory periodontal disease.
