improving the care and quality of life for people with diabetes MEDICATION ASSISTANCE PROGRAM APPLICATION & INSTRUCTIONS Program Details and Application Instructions: The Diabetes Foundation (DFI) supports people throughout New Jersey who are unable to pay for diabetes medications and/or supplies due to financial hardship. Assistance is for one time only and will be provided for up to eight weeks to bridge the gap until a long-term solution can be obtained. In addition, the DFI Support Services Team will provide follow-up services to assist program participants with solutions for medication compliance, diabetes care, and affordable resources for obtaining their prescriptions in the future. To Be Considered, Applications Require The Following: A completed application with signatures, submitted by a Referrer (i.e. physician, nurse, social worker, case worker etc.). Applications received directly from applicants will not be accepted. A document displaying proof of address (copy of driver s license, utility bill, phone bill, invoice etc.). Applicants must be residents of the state of New Jersey in order to receive assistance. Prescriptions for each item requested must be submitted with the application. A plan for long-term medication assistance- (i.e. applications submitted to government insurance programs, drug company assistance programs, etc.- see page 5 for further examples). **Incomplete applications will be retained until the required documents have been provided Upon review and processing of the application, confirmation will be sent to the Referrer, indicating the approval status. This form will also indicate if certain medications and/or supplies are being substituted or cannot be provided by the DFI. Once approved, medications and supplies are delivered or shipped directly to the patient by our partner pharmacy within 2-3 business days. Shipments can also be sent to the medical facility of choice, if preferred. Please contact our office if the applicant has an emergency situation and will need their prescription delivery expedited. THIS PROGRAM PROVIDES THE FOLLOWING MEDICATIONS & SUPPLIES: Please request items from the following list only. Others may be provided on a case-by-case basis, when possible. If our organization cannot provide a certain item, we may be able to help to obtain it from another source. Please contact us. If the applicant is unable to travel to these stores, we will consider providing these medications on a case-by-case basis. Recommendations: In an effort to help the DFI provide assistance in an affordable manner: Please consider using the most affordable brands of insulin, supplies, and testing products. This will also benefit applicants who will not qualify for long-term assistance programs and will be cash-paying once they finish their initial supply of prescriptions from the foundation. Affordable brands include, but are not limited to: ReliOn/Novolin insulin (WalMart), Humulin insulin, Arkray Glucocard Vital testing supplies (ShopRite and other pharmacies), ReliOn testing supplies (WalMart). In some cases, insulin vials are more cost-effective than insulin pens.
