Airway Pressures PIP Pr Pplat PEEP 1) Peak Inspiratory Pressure (PIP). Time 2) Plateau Pressure (Pplat). 3) Mean Airway Pressure. 4) Positive End Expiratory Pressure (PEEP).

PEEP Advantages: - Recruits collapsed alveoli. - Improve O2 by FRC. - Shear forces. - WOB in Auto PEEP. - bleeding from mucosa. Disadvantages: - COP. -Barotrauma ( > 15 cmh2o) - ICP. Indications: - To improve oxygenation. - To bleeding.

Pulmonary Graphics Flow- Time Waveforms Constant Decelerating Accelerating Sinusoidal -No difference exist in terms of gas exchange or work of breathing. Constant Flow results in better distribution of VT in conditions with low and unequal lung compliance, e.g. lobar Pneumonia. Decelerating Pattern is beneficial in obstructive airway diseases. Accelerating and Sinusoidal Patterns are beneficial in ARDS.

If an imaginary line is drawn down the middle of the loop, the area to the right represents insp. resistance and the area to the left represents exp. resistance.

HOME MESSAGE It s not the ventilator mode that makes a difference It s the skills of the clinician. Ventilator waveforms analysis is an integral component in the management of a mechanically ventilated patient.

HOME MESSAGE Develop a habit of looking at the right waveform for the given mode of patient ventilation. Always look at the inspiratory and expiratory components of the flowtime waveform. Change the scale or speed of the waveform for better interpretation.
