Vol. 34 No. 1 January-February 2011 Chang Gung Medical Journal CONTENTS REVIEW ARTICLES 1 Surface Anthropometric Indices in Obesity-related Metabolic Diseases and Cancers Chao-Yang Yang, PhD; Chien-Yu Peng, MS; Ying-Chieh Liu, PhD; Wen-Zhi Chen, PhD; Wen-Ko Chiou, PhD Obesity-related metabolic syndrome (MetS), cardiovascular diseases (CVD) and cancers such as colon cancer (CCa), prostate cancer (PCa) and breast cancer (BCa) are related to surface anthropometry such as weight, height and waist circumference (WC) to some extent. Generally, WC is accepted in CVD and CCa risk assessment whereas more evidence on WC-CVD, WC-PCa and WC-BCa correlations is needed. The body mass index, which has been widely used as a determinant of obesity, has a strong connection to CCa risk in men and young women but has an inconsistent connection with BCa. 23 Papillary Thyroid Carcinoma with Different Histological Patterns Jen-Der Lin, MD; Chuen Hsueh, MD; Bie-Yu Huang, MD Different histopathological patterns and variants of papillary thyroid carcinoma may influence the prognosis of these patients. The clinical presentation, cancer recurrence, and cancer-specific mortality of the follicular variant, insular pattern, tall cell pattern, diffuse sclerosing type, papillary thyroid carcinoma with Hashimoto s thyroiditis, and multicentric papillary thyroid carcinoma were reviewed. Different histological variants of papillary thyroid carcinoma with other histological patterns are important for predicting cancer recurrence. ORIGINAL ARTICLES 35 Asthma Control in Asthmatic Patients Treated for Lung Cancer Chung-Hsing Hsieh, MD; Chun-Hua Wang, MD; Han-Pin Kuo, MD, PhD; Kang-Yun Lee, MD, PhD Non-small cell lung cancer (NSCLC) and asthma are both Th2-predominant diseases. NSCLC may have an impact on the control of asthma. Thirty-eight asthmatic patients had worsening of their symptoms on asthma controltest (ACT) scores at the time of diagnosis of lung cancer compared to scores in the preceding year. Patients who responded to lung cancer treatment had improved ACT scores. However, patients who did not respond to cancer treatment had even worse ACT scores. Asthmatic patients with acquisition or progression of NSCLC had worsening of their asthma control status. Patients with good responses to cancer treatment had improved asthma control. These observations indicate that the Th2 pathway in lung cancer may be a contributing factor in asthma control, another Th2 predominant disorder. 44 Foot Bacterial Intertrigo Mimicking Interdigital Tinea Pedis Jing-Yi Lin, MD; Yi-Ling Shih, MD; Hsin-Chun Ho, MD Patients with foot intertrigo with a poor response to antifungal or anti-inflammatory treatment from 2004 to 2009 were included in this study. We recorded 32 episodes of foot bacterial intertrigo in 17 patients. The disease was more common in men (82%). The majority of bacterial cultures grew mixed pathogens (93%). Pseudomonas aeruginosa, Enterococcus facealis and Staphylococcus aureus were the most common pathogens. Autoeczematization was present in 50% of the 32 disease episodes. Foot bacterial intertrigo is not a rare condition and can be easily confused with interdigital tinea or eczematous dermatitis. Proper identification of bacterial organisms is critical for early effective antibiotic therapy. 50 A Double-blind, Randomized, Comparative Study to Evaluate the Efficacy and Safety of Zaleplon versus Zolpidem in Shortening Sleep Latency in Primary Insomnia Yu-Shu Huang, MD; Shih-Chieh Hsu, MD; Shen-Ing Liu, MD, PhD; Chih-Ken Chen, MD, PhD This was a randomized, double-blind, active-controlled, double-dummy, comparative study to compare the

effectiveness and safety of zaleplon and zolpidem in primary insomnia. A total of 45 patients completed the study. There was a significant decrease in sleep latency from baseline for patients receiving either zaleplon 10 mg or zolpidem 10 mg. There was no significant difference between zaleplon and zolpidem in the efficacy of reducing sleep latency or adverse effects; however, during this clinical trial there was one lethal event caused by a traffic accident in the zaleplon group. 57 The Role of Consulting Psychiatrists for Obstetric and Gynecologic Inpatients Huang-Li Lin, MD; Hung-Hsueh Chou, MD; Chia-Yih Liu, MD; Shi-Chieh Hsu, MD; Mei-Chun Hsiao, MD; Yeong-Yuh Juang, MD This study investigated the consultation psychiatry service to the Obstetrics and Gynecology Department in a general hospital based on 6-year retrospective review of the medical charts. The most common reasons for referral were depression, past psychiatric history, insomnia and confusion. The psychiatric referral rate of obstetric and gynecological inpatients was relatively low compared with that of other departments. More collaboration and liaison between gynecologists and consultation psychiatrists may provide better care for obstetric and gynecological inpatients. 65 Cytomegalovirus Infection in Non-transplant Patients with Hematologic Neoplasms: A Case Series Hung Chang, MD; Tzung-Chih Tang, MD; Yu-Shin Hung, MD; Tung-Liang Lin, MD; Ming-Chung Kuo, MD; Po-Nan Wang, MD Cytomegalovirus (CMV) infection is uncommon in hematologic patients. We encountered 17 patients with 20 episodes of CMV infection in 6 years. CMV infection occurred at various ages and with a number of underlying hematological diseases. No single laboratory assay was sensitive enough to serve as a screening test. CMV infection were accompanied by various coinfections in 60% of cases. Ganciclovir as the sole antiviral treatment was effective in most infection episodes. Cytotect was provided for one patient. Although most patients initially had good responses to antiviral treatment, only 5 patients survived after short follow-up durations (4-14 months). In an era of intensive immuno-chemotherapy, CMV infection may become a serious threat for hematology patients. Physicians dealing with hematological malignancies should be aware of CMV infection, especially for patients receiving rituximab and steroids. 75 A Comparison of Spring-loaded and Vacuum-assisted Techniques for Stereotactic Breast Biopsy of Impalpable Microcalcification Lesions: Experience at Chang Gung Memorial Hospital at Linkou Pei-Ching Huang, MD; Yun-Chung Cheung, MD; Yung-Feng Lo, MD, PhD; Shin-Cheh Chen, MD; Tzu-Chieh Chao, MD, PhD; Shir-Hwa Ueng, MD; Swei Hsueh, MD; Mei-Hua Chang, BS; Shu-Hang Ng, MD The aim of this study was to assess the diagnostic performance of stereotactic core needle breast biopsy using spring-loaded or vacuum-assisted techniques on impalpable microcalcification lesions in Taiwanese women. We retrospectively reviewed 335 breast stereotactic core needle biopsy procedures for microcalcification lesions from January 1999 to February 2009. The overall accuracy, false negative rate, ductal carcinoma in situ (DCIS) upgrade rate and rate of concordance with stereotactic core needle biopsy were determined. In conclusion, stereotactic core needle biopsy of breast microcalcification lesions is a feasible diagnostic technique in Taiwanese women. The diagnostic performance of the vacuum-assisted technique was better than that of the spring-loaded technique. 84 Surgical Results and Complications of Developmental Dysplasia of the Hip - One Stage Open Reduction and Salter s Osteotomy for Patients between 1 and 3 Years Old Chia-Hsieh Chang, MD; Hsuan-Kai Kao, MD; Wen-E Yang, MD; Chun-Hsiung Shih, MD We reviewed 63 patients with unilateral hip dislocation who had one-stage open reduction and Salter s osteotomy under the age 3 years. After 10 years, good clinical and radiographic results were noted in 89% and 92% of the hips, respectively. Osteonecrosis occurred in 30 hips, of which 14 developed coxa vara in a stable hip. Residual dysplasia was noted in 5 hips, mostly resulting from late-onset coxa valga. These two major complications could limit the longevity of the hip.

93 Role of Magnetic Resonance Imaging and Apparent Diffusion Coefficient at 3T in Distinguishing between Adenocarcinoma of the Uterine Cervix and Endometrium Yu-Ching Lin, MD; Gigin Lin, MD; Yu-Ruei Chen, MD; Tzu-Chen Yen, MD, PhD; Chun-Chieh Wang, MD; Koon-Kwan Ng, MD From 2006 to 2008, 29 patients with uterine adenocarcinoma proven by biopsy underwent magnetic resonance imaging (MRI) study and 22 cervical cancers and 7 endometrial cancers were studied. Distinctive features of adenocarcinoma showed that endometrial cancer is more elongated than cervical cancer and the apparent diffusion coefficient (ADC) value for endometrial cancer (mean 96.9 x 10-5 mm 2 /sec) was higher than for cervical cancer (mean 76.6 x 10-5 mm 2 /sec). MRI is helpful in distinguishing uterine adenocarcinoma originating from the cervix and endometrium. Combination with the tumor s ADC value may strengthen diagnostic confidence. 101 Open and Arthroscopic Surgical Management of Primary Synovial Chondromatosis of the Hip Yi-Hsun Yu, MD; Yi-Sheng Chan, MD; Mel S. Lee, MD, PhD; Hsin-Nung Shih, MD Primary synovial chondromatosis commonly involves large joints such as the knee and hip. The optimal approaches for successful surgical management of symptomatic primary synovial chondromatosis of the hip are controversial. A review of the medical records at our hospital between 2000 and 2005 found that selecting an appropriate surgical approach (open arthrotomy or arthroscopic surgery) by means of identified indications provides a rapid recovery and lower recurrence. CASE REPORTS 109 Successful Medical Treatment of An Implant Periapical Lesion: A Case Report Li-Ching Chang, DDS, MS; Chi-Sen Hsu, MD; Ying-Lin Lee, DDS An implant periapical lesion (IPL) is an infectious-inflammatory alteration surrounding an implant apex. The diagnosis is based on the clinical manifestations and radiological findings, in which a radiotransparency can be seen at the periapical level. It is apparent that an IPL has a multifactorial background, mainly caused by preexisting microbial pathology or surgical trauma during implant surgery. In the literature, treatments for an IPL have all been surgical methods. We present a case of successful treatment of an IPL with medical methods. There were no symptoms or signs of the IPL after functional loading at follow up. 115 Sclerosing Syringoid Carcinoma of the Finger Chuieng-Yi Lu, MD; Shih-Ming Jung, MD; Yu-Te Lin, MD We present a case of sclerosing syringoid carcinoma of the finger, which is seldom reported in the literature. It has a predilection for the head and neck region, but is rare in the extremities. We believe this is the second reported case of sclerosing syringoid carcinoma in the finger. The disease presentation and the difficulties encountered during establishment of the diagnosis and planning of the treatment are discussed. 122 INSTRUCTIONS TO AUTHORS
