Summa Health Bariatric Care Center It s no secret that a growing number of Americans are overweight, obese or morbidly obese. In fact, obesity currently affects about one-third of adults in the U.S. and more than half of all American adults are overweight. In 2013, the American Medical Association finally declared obesity to be a chronic disease. While no one fully understands the exact cause of obesity, we do know that being overweight, obese or morbidly obese is: Caused by many factors, including food choices, lifestyle and genetics A complex disease that requires skill and knowledge to manage effectively A sensitive condition that requires the care of an attentive and compassionate team Obesity is now ranked as the secondhighest cause of preventable deaths in the country with approximately 300,000 people dying each year from obesity-related illnesses such as: Type 2 Diabetes High blood pressure Obstructive Sleep Apnea (OSA) Gastroesophageal Reflux Disease (GERD) High cholesterol Arthritis Joint problems Our program s comprehensive approach to nonsurgical weight loss is based upon scientific research about weight loss and years of experience gained from working with patients just like you. We will help you reach your weight management goals by teaching you how to make better choices about eating and exercise, and by working with you to address those underlying behaviors which are preventing you from reaching your goals. Comprehensive, Physician-Driven, Team-Based Physician Evaluation Because obesity is complicated, the solution is not simple. The first step is an evaluation with one of our physicians who specializes in Obesity Medicine. We have been providing weight loss management services at Summa since 2004 and our physicians are committed to helping patients lose weight safely and effectively. During your first visit, your physician reviews your medical history to determine if: Our nonsurgical weight management program is right for you Any testing is needed before approving your participation in the program Behavioral Evaluation All patients considering enrollment in our nonsurgical weight management program undergo a behavioral assessment following the physician evaluation. An individual evaluation is conducted by one of our clinical psychologists who specialize in eating behaviors to determine whether there are any underlying eating behaviors contributing to your weight problem. Once you have received medical clearance from the program s physician, and you have completed the Behavioral Evaluation, you are ready to enroll in the program. Our multidisciplinary team uses a collaborative approach to help you take those first steps toward redesigning your eating and exercise behaviors. Each team member works closely with you and with our physician to help you meet your goals. At the Bariatric Care Center, you receive support and guidance from a team that includes: Physician Physician Our board-certified obesity medicine specialist prescribes an eating and exercise plan for all patients who enroll in our program. He leads a team of experts that helps you lose weight safely and effectively. Ken Wells, M.D., Certified Diplomate ABOM Medical Director Clinical Psychologists Clinical Psychologist Our psychologists specialize in weight management and are here to help you recognize behavior patterns and modify them to help you lose weight through a tailored and individualized approach. They will work with you to identify triggers that lead to overeating and teach you tools to help you cope with depression, anxiety and stress. Jennifer Duncan, Psy.D. Registered Dietitians Christina Rummell, Ph.D. Registered Dietitian Our dietitians also specialize in weight management. They work closely with each patient to review the eating plan and to make sure you feel comfortable shopping for and preparing your meals. Hanna Koscinski, RDN, LD Tess Lockhart, RD, LD Clinical Exercise Specialist Clinical Exercise Specialist Our clinical exercise specialist is certified by the American College of Sports Medicine (ACSM) and explains the role that exercise plays in successful weight management. Program Overview Because weight gain happens over time, weight loss takes time as well. Summa Health s nonsurgical weight management program covers a twoyear period of time. The initial intensive phase lasts for six months, and then follow-up continues for an additional 18 months at a decreasing frequency to ensure you reach your goals. Healthy weight management begins with reaching your goal weight, and then continues with maintenance of that weight. Many people have cycled through weight loss and regain multiple times throughout their life. You will continue to see us for lifetime followup to prevent you from regaining weight you worked so hard to lose. Upon enrollment in the program, you will be scheduled for: Lifestyle Change Kick-off Workshop This 4-hour workshop includes: An Exercise Foundations Class A Nutrition Foundations Class A Behavioral Foundations Class Nutritional Monitoring Upon program enrollment, you will also receive an order for bloodwork. This bloodwork will help the physician plan the best approach for you to lose weight. Physician Follow-Up You also will be scheduled for physician follow-up visits. Our physician monitors progress toward your weight loss goals and makes adjustments to the eating and exercise plans as needed for continued weight loss. Patients are seen monthly (1 visit each month for five months) during the first six months. Upon completion of the initial, intensive phase, patients have physician visits at nine months, 12 months, 15 months, 18 months and 24 months. Patients who are struggling with weight management may be seen more frequently. All patients are scheduled for lifelong follow-up care to maintain the weight loss that they achieve. Upon completion of the Lifestyle Change Kick-off Workshop, patients engage in a series of three intensive outpatient weight loss group sessions with the psychologist to focus on the critical behavioral aspects of weight management. Putting It All Together Successful weight loss is best achieved with a plan. Our specialists provide you with a plan for eating, exercise and behavior changes. The Eating Plan Making the right choices about what to eat and when to eat are two of the biggest success factors in managing your weight. Our program helps you learn how to make the right choices while still providing enough variety in your diet so it s easy to stick to the plan. Some things you should know include: We do not use any pre-packaged specialty foods that must be purchased. We do not prescribe medications to assist with weight loss. Our team has extensive experience working with patients who struggle with weight loss. All patients are provided with a structured eating plan, as well as suggestions on how to tailor the eating plan to suit their preferences and lifestyle. The Exercise Plan Regular exercise is a critical part of a successful weight loss program and of lifelong maintenance of a healthy weight. Our clinical exercise specialist helps each program participant create a personalized exercise plan that will help meet his or her weight management goals. An exercise program will only work if you feel it is one you can and will follow. You should know that an effective exercise plan: Uses a combination of aerobic and strength training activities. Requires you to commit to a specific exercise plan during your Exercise Foundations Class. Is reviewed at each visit during the program to ensure you are following the program you chose. Our physician will adjust the program as needed. The Behavior Plan Changing your behavior when it comes to eating and exercise is the key to success in any weight loss plan. The degree to which you change your behavior is directly related to the amount of weight loss you can expect to achieve. Turning these changes into lasting habits is the real key to maintaining a healthy weight. Three important things to know about our behavior plan are: All visits take place in the privacy of the Bariatric Care Center. The evaluation with the clinical psychologist is extremely important, as underlying disordered eating behaviors can be at the root of whether success or failure can be expected from weight loss efforts. That s why we use a team of experts to help our patients transition to a healthier lifestyle. Outcomes Patients who have fully participated in our program have achieved a weight loss of as much as five to 20 pounds per month. Success is directly related to how closely you follow the recommended program. If you follow the program, you will lose the weight. Lifelong Follow-Up Obesity is now a chronic disease and therefore is best managed by physician treatment. Patients of our nonsurgical weight management program are seen at least annually for the rest of their life to ensure they maintain their weight loss and their health. Costs and Locations The Cost Some expenses related to this program are non-covered by all insurance plans. These expenses are the patient s responsibility and include costs for the following: Program administration fee, which includes the Lifestyle Change Kick-off Workshop, three additional Intensive Outpatient Weight Loss Groups and all program materials. This fee is due at the time of enrollment. Co-insurances, deductibles and co-payments for physician and psychologist visits as determined by your insurance company. All physician and individual psychologist visits are billed to your insurance. Most insurance companies cover both if you have any medical or psychological conditions caused by excess weight. Examples of these are high blood pressure, diabetes, reflux disease, high cholesterol, heartburn, sleep apnea, back or joint pain, fatigue, depression, anxiety and binge eating. For patients who do not have medical insurance or whose insurance will not cover physician visits for weight loss, we do offer private pay rates.
