Use of MedDRA in Special Situations Patrick Revelle MedDRA MSSO 13 March 2012 Overview Existing terms in MedDRA for off-label use, abuse, misuse, overdose, and medication errors Current coding recommendations Proposed changes Upcoming changes in MedDRA Terms Selection: Points to Consider document (MTS:PTC) for MedDRA v15.0 2 1

Off Label Use Terms in MedDRA (1) Version 15.0 3 Off Label Use Terms in MedDRA (2) MedDRA Introductory Guide (v15.0), Appendix B (Concept Descriptions) Off Label Use Off label use is defined as a practice of prescribing pharmaceuticals outside the scope of the drug s approved label, most often concerning the drug s indication. 4 2

Off Label Use MTS:PTC (1) 5 Off Label Use MTS:PTC (2) 6 3

Off Label Use MTS:PTC (3) Off Label Use MTS:PTC (cont) 7 Off Label Use Proposed Changes (1) 8 4

Off Label Use Changes for v15.0 (1) Document will now include an example related to patient population (pediatric) 9 Off Label Use Proposed Changes (2) 10 5

Off Label Use Changes for v15.0 (2) For off label use with AE/AR, preferred option will be changed 11 Overdose Terms in MedDRA (1) 12 6

Overdose Terms in MedDRA (2) MedDRA Introductory Guide (v15.0), Appendix B (Concept Descriptions) Overdose More than medically recommended dose (in quantity and/or concentration) is administered. An excessive dose. 13 Overdose MTS:PTC (1) 14 7

Overdose MTS:PTC (2) No specific changes proposed in new MTS:PTC (v15.0) 15 Misuse Terms in MedDRA Concept has been challenging to define Is all misuse intentional? No wording in current MTS:PTC document addressing misuse 16 8

Misuse Proposed Changes Points to Consider Working Group has postponed addition of wording until there is international agreement on regulatory definition of misuse 17 Abuse Terms in MedDRA (1) 18 9

Abuse Terms in MedDRA (2) 19 Abuse Terms in MedDRA (3) MedDRA Introductory Guide (v15.0) 20 10

Abuse Proposed Changes Points to Consider Working Group has postponed addition of wording until there is international agreement on regulatory definition of abuse 21 Expansion of Medication Error Terms Prior to MedDRA Version 8.0, only one term existed - PT Medication error Medication error section expanded in v8.0 Added HLGT Medication errors in SOC Injury, poisoning and procedural complications HLT Maladministrations HLT Medication errors due to accidental exposures HLT Medication monitoring errors HLT Overdoses HLT Medication errors NEC 22 11

Scope of MedDRA Drug Information Association www.diahome.org 23 Medication Error Definition A preventable event Intentional action is NOT an error Includes intercepted and not intercepted errors, because Patient harm is NOT required Inappropriate use of the medication is NOT required Applies to all stages of medication handling and process 24 12

Concept Descriptions Appendix B in MedDRA Introductory Guide To assist in understanding and appropriate use of medication error and product quality issue terms in regulatory reporting Medication error descriptions developed by FDA and MSSO 25 Medication Error Concept Description Examples Documented hypersensitivity to administered drug: This medication error refers to the situation when a patient is administered a drug that is documented in the patient's medical file to cause a hypersensitivity reaction in the patient. Example: Despite the fact that the patient's medical record indicated "sulfa allergy," the physician prescribed a sulfa antibiotic. Subsequently, the patient took the antibiotic and experienced hives. 26 13

Medication Errors Terms in MedDRA Drug Information Association www.diahome.org 27 Medication Errors MTS:PTC 28 14

Medication Errors Proposed Changes There can be overlap between product quality issues and medication errors An example of both a product quality issue and a medication error is being added to MTS:PTC for v15.0 29 Occupational Exposure Terms in MedDRA 30 15

Occupational Exposure MTS:PTC Not specifically referenced, but covered under accidental exposures 31 Occupational Exposure Proposed Changes Points to Consider Working Group has postponed addition of wording until there is international agreement on regulatory definition of occupational exposure 32 16

Summary There is regulatory interest in special situations Off label use Overdose Misuse Abuse Medication errors Occupational exposure MedDRA covers most such concepts and can expand as needed through Change Requests MTS:PTC also addresses some of these; more may come in the future 33 Thank You 17
