ENVIROMENTAL AND SOCIAL HEALTH DETERMINANTS IN ZANZIBAR

INTRODUCTION Zanzibar part of the United Republic of Tanzania Comprises of 2 main Islands Unguja and Pemba which covers an area of 2328 sq.km.

HEALTH STATUS. Zanzibar population is 984,625 (2002 census 50% below 50 years Annual growth rate of 3.1%) Population density 400/sq.km. Life expectancy 48 years Infant mortality 83/1000 Under 5 mortality 166/1000

Maternal mortality 377/100000 live births 33%live in urban areas Malnutrition rate among children 30% by 2000 70% of children between 12 to 23 month have received complete immunization before 1st birthday

HEALTH SERVICES INFRASTRUCTURE. 100% live within 10 km of Health care facilities and 95% within 5Km 127 Primary Health Care Units,4 Primary Health Care Centres,3regional Hospitals in Pemba with capacity of 80 to 100 beds an 400 bed referral hospital in Zanzibar Town.

HEALTH SERVICES INFRASTRUCTURE contd.. 100 private clinics, 3 private hospitals, 66 pharmacies and 206 OTC shops Population per physician ratio is estimated at about 11,000

CONSTRAINTS. Despite this impressive status health delivery in Zanzibar is not optimum due to a number of contextual problems Downturn of the economy made it difficult for the state to adequately finance social service sector including health Increased population growth rates and resurgence of diseases like HIV/AIDS significantly increased the demand for services beyond what public health infrastructure can provide Barrier to access to services are. socioeconomic, gender and low health knowledge.

CONSTRAINTS. Contd.. Reduction in Direct Donor support About 50% of the Population lives below poverty line. Per capita income is 250 USD with a lot of disparity and inequality between Unguja & Pemba and between rural and urban areas. Health expenditure per capita is 5.9 USD.

INTERVENTIONS. POLICY ENVIRONMENT Vision 2020 with an overall objective of eradicating poverty. To raise health and Nutritional standards of the community in general and specifically for women and children. To give priority on child immunization To channel more resources for preventive services. To provide special emphasis on the war against the spread of HIV/AIDS

POLICY ENVIRONMENT contd.. Zanzibar Poverty reduction plan Provides measure to decrease income poverty and to increase access to social services and to contain extreme vulnerability. High priorities sectors for ZPRP are Education, Health Water & sanitation, Environment and Good governance The over aching objective is to develop health sector with a capacity to provide basic health care services to the poor in both urban and rural areas

POLICY ENVIRONMENT AT SECTOR LEVEL Zanzibar s Health Policy To have equal opportunity of access of basic and essential quality health care To increase the focus on financing the overall health care system To use cost sharing as a means of providing health care services while protecting the rights of the poor and disadvantaged To ensure the disadvantaged and other vulnerable groups are protected and their special needs catered for. Zanzibar Population Policy for sustainable development Overall goal of achieving improved quality of life and standard of living of all Zanzibaris through better population management and development process. Targets. Improve health facilities from 90% within 5km to 90% within 4 km by 2010 Reduce HIV prevalence of 0.6% by 50% by 2010 Reduce malnutrition rate of 31% by 50% by 2010 Improve exposure to family planning education from existing 60% in 200 to 90% by 2010

Environment Formulation of environmental guidelines for various aspects of natural resources management, training of environmental personnel, educate Public on the need to integrate environmental protection process in their daily activities. Prepare National Environmental Management and Action plans Success has been achieved in combating land degradation and biodiversity conservation

People with Disability Zanzibar has greatly changed its perception of disability from charity /welfare perspective to recognition of the rights and contribution made by people with disability in the society

ACHIEVEMENTS Education: Gross Enrollment Rate has increased from 98.1% in 2002 to 99.1 in 2003 and 100% in 2005 Pupils Teacher ratio has improved to 33:1 Teachers Trained 922 in 2001,1455 in 2003. Literacy rate is 73. 4% (2002)

ACHIEVEMENTS contd.. Water and sanitation Water Policy approved by Cabinet in 2003 More than 60% of rural and urban population is now enjoying clean and safe water Communication More than 130 kilometers of tarmac road constructed in Unguja and Pemba in the last five years to ease transport and hence access to health care

WAY FORWARD. There is a need to conduct scientific study to evaluate the social determinants of Health and their impact on delivery of Health services in Zanzibar. Conduct demographic, health and labour force surveys Convert the many policies and plan into reality, this will require political commitment, increase level of awareness and drastic improvement in the economy with more close partnership with donor community and community involvement in planing health care facilities
