INDICATION (ribociclib) is a prescription medicine used in combination with a certain type of medicine as the first hormonal-based therapy to treat women who have gone through menopause with hormone receptor (HR)-positive, human epidermal growth factor receptor 2 (HER2)-negative breast cancer that has spread to other parts of the body (metastatic). The (ribociclib) FEMARA (letrozole) Co-Pack is a prescription medicine used as the first hormonal based therapy to treat women who have gone through menopause with hormone receptor (HR)-positive, human epidermal growth factor receptor 2 (HER2)-negative breast cancer that has spread to other parts of the body (metastatic). The FEMARA Co-Pack contains 2 different types of medicines: The violet tablet contains the medicine (ribociclib). The yellow tablet contains the medicine FEMARA (letrozole). It is not known if or the FEMARA Co-Pack are safe and effective in children. IMPORTANT SAFETY INFORMATION What is the most important information I should know about or the FEMARA Co-Pack? or the FEMARA Co-Pack may cause serious side effects, including: Heart rhythm problems (QT prolongation). or the FEMARA Co-Pack can cause a heart problem known as QT prolongation. This condition can cause an abnormal heartbeat and may lead to death. Your health care provider should check your heart and do blood tests before and during treatment with or the FEMARA Co-Pack. Tell your health care provider right away if you have a change in your heartbeat (a fast or irregular heartbeat), or if you feel dizzy or faint. Welcome to You ve probably had conversations with your doctor about what it means to have advanced or metastatic breast cancer when breast cancer has spread to other parts of the body. Learning that you have metastatic breast cancer is life-changing. You may feel shocked and overwhelmed. As you process your diagnosis, it s important to remember that you didn t do anything wrong. This is not your fault. Treatment will now be part of your daily routine. That s why your doctor prescribed along with hormone therapy like FEMARA (letrozole) for your specific type of advanced or metastatic breast cancer: hormone receptor (HR)-positive, human epidermal growth factor receptor 2 (HER2)- negative, or HR+, HER2-. is a breakthrough therapy that received approval from the U.S. Food and Drug Administration (FDA) based on the need for more treatments for women living with metastatic breast cancer. Targeted therapy (ribociclib) is a CDK4/6* inhibitor and a targeted therapy for HR+ (ER+ and/or PR+), HER2- metastatic breast cancer. This targeted treatment blocks, or interferes with, substances that play a role in cancer cell growth. Targeted therapies affect cancer cells, but can also affect healthy cells. was researched in a clinical trial that included 668 postmenopausal women who had not yet received treatment for their metastatic breast cancer. Women either took + letrozole or letrozole alone. * CDK4/6 = cyclin-dependent kinase 4 and 6. ER+ = estrogen receptor-positive; PR+ = progesterone receptor-positive. IMPORTANT SAFETY INFORMATION (continued) Liver problems (hepatobiliary toxicity). or the FEMARA Co-Pack can cause serious liver problems. Your health care provider should do blood tests to check your liver before and during treatment with or the FEMARA Co-Pack. Tell your health care provider right away if you get any of the following signs and symptoms of liver problems: yellowing of your skin or the whites of your eyes (jaundice) dark or brown (tea-colored) urine Significantly more effective The combination of and letrozole (a type of hormone therapy) has proven to help delay disease progression. + letrozole reduced the risk of disease progression by 44% vs letrozole alone. In a clinical trial analysis, more than half of women taking + letrozole remained alive and progression free. 2+ years without disease progression Median time women lived without disease progression from the start of treatment 0 months 15 months 30 months Shrinks tumors and letrozole worked together to shrink tumors in women who had measurable tumors. MORE THAN HALF 52.7% of women taking + letrozole had tumors that shrank or disappeared vs 37.1% of women taking letrozole alone.* * 49.6% of women taking + letrozole saw their tumors shrink in size vs 34.7% of those taking letrozole alone. To understand how works with hormone therapy, it s important to also understand the role that hormones play in causing your type of cancer cells to grow and divide. Together, and hormone therapy work to help delay disease progression HR+, HER2- metastatic breast cancer growth Hormones like estrogen activate CDK4/6 proteins causing cell division Treatment with and hormone therapy blocks CDK4/6 activity, helping prevent cell division Hormone therapy, like IMPORTANT SAFETY INFORMATION (continued) Low white blood cell counts (neutropenia). Low white blood cell counts are very FEMARA (letrozole), limits estrogen levels common when taking or the FEMARA Co-Pack and may result in Estrogen CDK4/6 Hormone infections that may be severe. Your health care provider should check your white proteins therapy blood cell counts before and during treatment with or the FEMARA Co-Pack. Tell your health care provider right away if you have signs and symptoms of low white blood cell counts or infections such as fever and chills. Recommended 4-Week Dosing Cycle For the first 3 weeks in a cycle, you ll take your 3 pills in combination with a hormone therapy. The fourth week, you won t take any pills you ll only take your hormone therapy. After the fourth week, your doctor may have you start the same cycle over again, or may adjust your dose if needed. It is important to follow your doctor s directions on how to take. How How to take (ribociclib) is a convenient, once-daily oral tablet that is supplied in an easy-to-use blister pack. Your doctor will prescribe the dose of that s right for you. Take exactly as prescribed. Do not change your dose or stop taking unless your doctor tells you to do so. is taken with hormone therapy: The recommended starting dose of is 600 mg (three 200-mg tablets), taken once daily Ask your doctor about the recommended dose of your hormone therapy Not actual pill sizes. IMPORTANT SAFETY INFORMATION (continued) Your health care provider may tell you to decrease your dose, temporarily stop, or completely stop taking if you develop certain serious side effects during treatment with or the FEMARA Co-Pack. 600 mg (three 200-mg tablets) once daily for 3 weeks, followed by 1 week without taking Hormone therapy (eg, letrozole) Follow your doctor s recommended dosing schedule Treatment is taken once daily during that week of the cycle. Dosing Instructions Take the recommended dose of (ribociclib) once each day at about the same time, preferably in the morning You can take either with food or without food Swallow tablets whole with a glass of water. Do not chew, crush, or split the tablets Do not take any tablets that are broken, cracked, or that look damaged Avoid pomegranate or pomegranate juice, and grapefruit or grapefruit juice while taking If you miss a dose of or vomit after taking a dose of, do not take another dose on that day. Take your next dose at your regular time If you take too much, call your doctor right away or go to the nearest hospital emergency room DOSE ADJUSTMENTS offers dosing flexibility to help your doctor find the dose of that s right for you. Your doctor may lower your dose, interrupt your dose, or stop treatment with to help you manage side effects. dosing can easily be adjusted by your doctor to fit your needs. If your doctor decides that you need a lower dose, you won t need to obtain a new prescription to adjust your dose that month. The number of 200-mg tablets may be reduced from 3 tablets to 2 tablets to 1 tablet, according to your doctor s instructions. IMPORTANT SAFETY INFORMATION (continued) What should I tell my health care provider before taking or the FEMARA Co-Pack? Before you take or the FEMARA (letrozole) Co-Pack, tell your health care provider about all of your medical conditions, including if you: have any heart problems, including heart failure, irregular heartbeats, and QT prolongation have ever had a heart attack have a slow heartbeat (bradycardia) have problems with the amount of potassium, calcium, phosphorus, or magnesium in your blood have fever, chills, or any other signs or symptoms of infection have liver problems are pregnant, or plan to become pregnant. or the FEMARA Co-Pack can harm your unborn baby If you are able to become pregnant, your health care provider should do a pregnancy test before you start treatment with or the FEMARA Co-Pack. Females who are able to become pregnant and who take or the FEMARA Co-Pack should use effective birth control during treatment and for at least 3 weeks after the last dose of or the FEMARA Co-Pack. Talk to your health care provider about birth control methods that may be right for you during this time. If you become pregnant or think you are pregnant, tell your health care provider right away. are breastfeeding or plan to breastfeed. It is not known if or the FEMARA Co-Pack passes into your breast milk. Do not breastfeed during treatment with or the FEMARA Co-Pack and for at least 3 weeks after the last dose of or the FEMARA Co-Pack Tell your health care provider about all of the medicines you take, including prescription and over-the-counter medicines, vitamins, and herbal supplements. or the FEMARA Co-Pack and other medicines may affect each other causing side effects. Know the medicines you take. Keep a list of them to show your health care provider or pharmacist when you get a new medicine. How In a clinical trial with (ribociclib) + letrozole, most side effects were mild to moderate and could be managed by reducing the dosage, interrupting treatment, or stopping treatment. Dose reductions due to side effects occurred in 45% of women taking + letrozole vs 3% of women taking letrozole alone. And 7% of women permanently stopped taking + letrozole due to side effects vs 2% of women taking letrozole alone. General tips for staying healthy while on Wash your hands often Stay hydrated Avoid anyone who is sick Avoid raw food and undercooked meats Avoid hot tubs, ponds, rivers, and lakes Diarrhea IMPORTANT SAFETY INFORMATION (continued) Drink plenty of water, sports drinks, juice, or clear broth Avoid spicy and/or greasy foods, alcohol, coffee, tea, and soda with caffeine Avoid milk and dairy products Eat food high in protein, such as beef, chicken, turkey, eggs, fish, and cheese Eat small amounts of soft, bland food. Summary of Important Information INDICATION (ribociclib) is a prescription medicine used in combination with a certain type of medicine as the first hormonal-based therapy to treat women who have gone through menopause with hormone receptor (HR)-positive, human epidermal growth factor receptor 2 (HER2)-negative breast cancer that has spread to other parts of the body (metastatic). The (ribociclib) FEMARA (letrozole) Co-Pack is a prescription medicine used as the first hormonal based therapy to treat women who have gone through menopause with hormone receptor (HR)-positive, human epidermal growth factor receptor 2 (HER2)-negative breast cancer that has spread to other parts of the body (metastatic). The FEMARA Co-Pack contains 2 different types of medicines: The violet tablet contains the medicine (ribociclib). The yellow tablet contains the medicine FEMARA (letrozole). It is not known if or the FEMARA Co-Pack is safe and effective in children. What is the most important information I should know about or the FEMARA Co-Pack? or the FEMARA Co-Pack may cause serious side effects, including: Heart rhythm problems (QT prolongation). or the FEMARA Co-Pack can cause a heart problem known as QT prolongation. This condition can cause an abnormal heartbeat and may lead to death. Your health care provider should check your heart and do blood tests before and during treatment with or the FEMARA Co-Pack. Tell your health care provider right away if you have a change in your heartbeat (a fast or irregular heartbeat), or if you feel dizzy or faint. Liver problems (hepatobiliary toxicity). or the FEMARA Co-Pack can cause serious liver problems. Your health care provider should do blood tests to check your liver before and during treatment with or the FEMARA Co-Pack. Tell your health care provider right away if you get any of the following signs and symptoms of liver problems: yellowing of your skin or the whites of your eyes (jaundice) dark or brown (tea-colored) urine feeling very tired loss of appetite pain on the right side of your stomach area (abdomen) bleeding or bruising more easily than normal Low white blood cell counts (neutropenia). Low white blood cell counts are very common when taking or the FEMARA Co-Pack and may result in infections that may be severe. Your health care provider should check your white blood cell counts before and during treatment with or the FEMARA Co-Pack. Tell your health care provider right away if you have signs and symptoms of low white blood cell counts or infections such as fever and chills. Your health care provider may tell you to decrease your dose, temporarily stop, or completely stop taking if you develop certain serious side effects during treatment with. Before you take or the FEMARA Co- Pack, tell your health care provider about all of your medical conditions, including if you: have any heart problems, including heart failure, irregular heartbeats, and QT prolongation have ever had a heart attack have a slow heartbeat (bradycardia) have problems with the amount of potassium, calcium, phosphorus, or magnesium in your blood have fever, chills, or any other signs or symptoms of infection have liver problems have any other medical conditions are pregnant, or plan to become pregnant. or the FEMARA Co-Pack can harm your unborn baby If you are able to become pregnant, your health care provider should do a pregnancy test before you start treatment with or the FEMARA Co-Pack. 35 About How

Summary of Important Information (continued) Females who are able to become pregnant and who take (ribociclib) or the FEMARA (letrozole) Co-Pack should use effective birth control during treatment and for at least 3 weeks after the last dose of or the FEMARA Co-Pack. Talk to your health care provider about birth control methods that may be right for you during this time. If you become pregnant or think you are pregnant, tell your health care provider right away. are breastfeeding or plan to breastfeed. It is not known if or the FEMARA Co-Pack passes into your breast milk. Do not breastfeed during treatment with or the FEMARA Co-Pack and for at least 3 weeks after the last dose of or the FEMARA Co-Pack What other medications might interact with or the FEMARA Co-Pack? Tell your health care provider about all of the medicines you take, including prescription and over-the-counter medicines, vitamins, and herbal supplements (especially St. John s wort). or the FEMARA Co-Pack and other medicines may affect each other, causing side effects. Know the medicines you take. Keep a list of them to show your health care provider or pharmacist when you get a new medicine. What should I avoid while taking or the FEMARA Co-Pack? Avoid eating pomegranate or grapefruit and avoid drinking pomegranate or grapefruit juice during treatment with or the FEMARA Co-Pack since these may increase the amount of in your blood. What laboratory tests do I need if I am prescribed or the FEMARA Co-Pack? Your doctor should check your heart rhythm, liver, and blood before you start or the FEMARA Co-Pack and periodically during your treatment with or the FEMARA Co-Pack. Your doctor may eventually stop checking some of these tests. If you are able to become pregnant, your health care provider should do a pregnancy test before you start treatment with or the FEMARA Co-Pack.
