Colorado Exemptions OFFICIAL MEDICAL EXEMPTION OFFICIAL NON- MEDICAL EXEMPTION
Official Medical Exemption www.colorado.gov/vaccineexemption Students with medical exemptions signed by a MD, DO, APN or delegated PA need to submit the form only once, unless the student s information changes. Official Medical Exemptions can be claimed by submitting the signed form to CDPHE for inclusion in CIIS or by submitting the form directly to the student s school or child care. Check with student s school or child care to determine whether they require a paper copy of the exemption form. Exemptions submitted online for inclusion in CIIS are confidential, and available only to authorized persons.
Official Non-Medical Exemptions www.colorado.gov/vaccineexemption Official Non-Medical Exemptions: Students in K 12 th grade must submit exemptions annually (exemptions expire June 30th each year) Students in child care/preschool must submit at 2, 4, 6, 12 & 18 months of age (exemptions expire when the next vaccines are due ) and then when the child enrolls in kindergarten. Non-medical exemptions can be claimed by submitting the online form to CDPHE for inclusion in the immunization registry, or by submitting the form directly to the school or child care. Exemptions submitted online for inclusion in CIIS are confidential, and available only to authorized persons.

Immunization Reporting (take away points) THANKS TO ALL WHO COLLECTED, ORGANIZED AND REPORTED DATA DATA WILL BE AVAILABLE ONLINE BY THE END OF APRIL NON- REPORTING SITES WILL BE LISTED AS SUCH WHEN CDPHE PUBLISHES DATA CIIS AND REQUIRED REPORTING ARE TWO SEPARATE PIECES REQUIRED REPORTING IS ANNUAL, DUE DECEMBER 1 ST OF EVERY YEAR (NO MID- YEAR REPORTS OR EDITS/UPDATES)
Immunization Reporting THERE WILL BE OPPORTUNITIES TO PROVIDE FEEDBACK AND SUGGESTIONS FOR IMPROVEMENTS TO THE REPORTING FORM AND PROCESS IMMUNIZATION MESSAGING OCCURS THROUGH THE SCHOOL NURSE LIST SERVE AND THE CHILD CARE HEALTH CONSULTANT LIST SERVE SEVERAL SCHOOLS DID NOT RECEIVE CONFIRMATION EMAILS EVEN THOUGH THEY SUBMITTED SUCCESSFULLY.
