Preventive Care Services Preventive care services can help prevent and lower your risk for certain health conditions. They may help your doctor find and treat some health conditions early, when standard treatments are more likely to work. What are preventive services? One example is a flu vaccine. Receiving this vaccine once a year can help lower your risk of the flu. Another example is a blood pressure screening. Over time, this screening may help your doctor diagnose or treat high blood pressure. The preventive services that can help you stay healthier throughout your life include: Health screenings Vaccinations Routine check-ups Education Counseling Making healthy lifestyle choices such as getting the preventive care you need on a regular basis can be vital to your health and well-being. You can use this worksheet to: Understand the most common preventive services that are recommended for your age and gender. Talk with your doctor about what preventive services are right for you. Keep track of which services you have received and when you are next due. Standard Recommendations by Age and Gender Here are some of the most common preventive services recommended for individuals who are healthy, based on age and gender. To see the services recommended for you, first find the tool for your gender, then find your age group. The table for women is on pages 2 5. The table for men is on pages 6 7. Your Unique Preventive Health Needs Preventive health needs can vary from person to person. Many factors will determine the type of preventive care that you need. These can include your age, gender, job, health habits, current health status, family health history, and other risk factors you may have for certain health conditions. You may need more or less preventive services than listed in the table above. Talk with your doctor about the preventive care services that are right for you. He or she may recommend other tests, vaccines, and time frames for you. Your doctor can work with you to create a custom preventive care schedule that fits your needs and helps optimize your health. Keeping Track of your Screenings and Vaccinations It s a good idea to keep track of the screenings and vaccinations your doctor recommends. You can do so using the chart on the next page. List the name of each screening or vaccination you have had or you need. Include the date you received the screening or vaccine. Write down your results and note any follow-up tests you may need. Below is an example of how you can use this chart. On the following page is a blank chart that you can print out or save.
