Clinic Fundraiser Toolkit 01 Overview As part of the clinic experience, team members are asked to participate in fundraising. In addition to raising crucial dollars to support OneSight s efforts to end the global vision care crisis, fundraising is a great way to unite the clinic team toward our common goal even before the first patient walks through the door. We believe that every dollar makes an impact, and strongly encourage every clinic participant to set up a fundraising page. While fundraising goals are a personal decision, the suggested minimum fundraising goal per person for 2017 is $500. 02 Setting Up Your Page Follow the steps below to set up your personal clinic fundraising page. 04 Tips and FAQ Use these tips to maximize your clinic fundraising. We d love to hear your ideas too - email your own fundraising tips to Meghan Fischer at mfischer2@onesight.org Before the clinic Get a head start fundraising pages can be set up as early as the beginning of the year. Set your page up early and give your networks plenty of time to donate. Personalize your fundraising page with your own photos, stories and reasons for supporting OneSight. If you are able, make a donation to your own campaign. Donating to your own campaign shows your commitment to the cause and can inspire others to be involved. Send emails to family and friends inviting them to support your campaign. Consider sharing a video from OneSight s library to help introduce them to the cause. Post regularly about your clinic and campaign on social media be sure to link to your fundraising page. Consider hosting an event or fundraiser where proceeds will benefit your campaign. Ideas include: Cook dinner for family and friends. Share your clinic experience and invite them to donate. Hold a raffle or split the pot and donate the proceeds to your campaign Hold a garage sale and donate proceeds to your campaign In lieu of presents ask for donations for your birthday Offer a service in exchange for donations, like desk cleaning or car washes During your clinic Share real time updates and patient stories with your networks via social media and email After your clinic Thank those who donated to your campaign and share an update on how the clinic went donors love to hear how their contributions make an impact! Bonus points for including photos or patient stories! Reach out to your networks again to provide an update on the clinic and make a final request for donations. pages will remain open until the end of the year, so there s plenty of time to hit your goal.

04 Tips and FAQ Have a question not listed here? Reach out to Meghan Fischer at mfischer2@onesight.org Frequently Asked Questions How do I access my clinic team fundraising page? Log in to your MyOnesight account at my.onesight.org and scroll down to your Clinics section. Click on the tile with your clinic team name to access your team fundraising page. How do I create my individual clinic fundraising page? From your team fundraising page, click on Join Team button to create your individual clinic fundraising page. You ll be walked through a step-by-step process to set a goal, choose copy, pick images, etc. Once your page is complete you ll be able to share on social media or via email with a unique URL link for you to send to your friends and family. Is there a minimum amount of money to raise in order to participate in the clinic? Each team has a fundraising goal based on number of team members. While fundraising is not mandatory, we strongly encourage every clinic team member to participate and the suggested minimum fundraising goal per person is $500. How is the team s fundraising goal determined? Team goals are determined by the number of participants per clinic. They are calculated using the baseline of $500 per team member How long is my fundraising page live? Your page is live from OneDay until 12/31/2017 Am I able to use existing fundraising pages for my new clinic? While you can have multiple campaigns created at one time, OneSight requires that you create a separate individual campaign specific to your Clinic team. This is the only way that funds raised can be attributed to your team s overall fundraising goal. How do I change my fundraising goal? Log in to your fundraising page via MyOnesight dashboard and go to Edit Your Page. Update the amount in the Goal box. You also have the ability to edit your personal message and change/add photos.

04 Tips and FAQ Have a question not listed here? Reach out to Meghan Fischer at mfischer2@onesight.org Frequently Asked Questions (continued) How do I remove or add a donor s name (and/or their gift amount) in the scrolling My Donors section? When a donation is made, the donor has the choice to include their name and/or gift amount in this section. Once the donor has made this decision, unfortunately, the donation platform doesn t allow the user to change it. I made an online donation and reside outside of the United States (US). Am I able to receive a tax receipt specific to the country I reside in? Unfortunately, no. Since the donation was made to a US charity, we are only legally allowed to provide a tax deductible ID that s applicable to the US. Any donors outside the US are advised to consult with their tax advisor. If you reside in Canada, you do have the option to submit check(s) to Dino Riccardi at driccard@luxotticaretail.com who will in turn issue Canadian tax deductible receipt(s). Please follow the below steps: 1. Gather the following information and send via excel document. For example, like the one below. https://docs.google.com/spreadsheets/d/1syup87j7xrdadid3ia7i83qptbl0sk deeoptt4bhwtg/edit#gid=0 Donor s first and last name Address Province Donation amount Check # Donation attribution (insert link to fundraising page you d like the donations to be credited to) 2. Mail check(s) to: Luxottica Retail - Canada 2000 Argentia Road Plaza 2, Suite 101 Mississauga, ON L5N 1V8 Attn: Dino Riccardi 3. Email (or mail) Dino the OneSight Canada Tax-Deductible file. **OneSight will make sure that all donations will be attributed to the fundraising pages of your choosing.

04 Tips and FAQ Have a question not listed here? Reach out to Meghan Fischer at mfischer2@onesight.org Frequently Asked Questions (continued) I m a clinic participant that resides outside of the US, do I use the fundraising platform via MyOneSight with my fellow team members? All clinic participants are required to use the fundraising platform accessible via the MyOnesight dashboard, with the exception of clinic participants residing in Australia or New Zealand, who are instructed to use EveryDay Hero fundraising platform. Please refer to the OneSight online resource center for detailed instructions for how to setup your fundraising page. Any questions? Contact onesightinfo@onesight.org.au. I have a donor that contributed to my online fundraising page by sending a check. How do I ensure that OneSight receives this donation and it is reflected on my page? OneSight prefers to receive donations by credit card, however, we understand that some folks feel more comfortable making donations through other methods. If a donor would like to donate via personal check or money order they can mail directly to the PO Box below. Do not mail cash. Send checks to and make payable to OneSight : OneSight P.O. Box 637509 Cincinnati, OH 45263-7509 On the check s note field, in the lower left corner of the check, specify the clinic participant and clinic name. For example John Smith-India Clinic I made a donation via check. Do I receive a receipt? Yes. Once a check is mailed-in and clears OneSight s lock box, OneSight generates a hard copy thank you letter that is sent to the address listed on the check which serves as your receipt. Are online donations made at my clinic location secure? Yes. OneSight s online fundraising platform is a safe, secure site no matter where you are in the world when making your online donation. I live outside of the United States. Will my credit card work? OneSight s online fundraising platform accepts all major credit cards, including MasterCard, Visa, AMEX and Discover from more than 100 countries. The bank that issues the card will convert your donation to US dollars and may charge the cardholder a foreign transaction fee. Have additional questions? Email mfischer2@onesight.org.

05 Social Media Guidelines Social media is a great way to spread the word about OneSight and raise funds to support your clinic campaign. Use these tips to make the most of your social networks!

05 Sample Social Media Posts Feeling stuck? Try one of these sample posts. Before your clinic Hi friends and family! I'm going to Tanzania and I need your help. This time I'm not traveling to lie on beaches or see the sights. I'm traveling with OneSight to provide vision care and glasses to those without access and we anticipate seeing over 1,000 patients of all ages. I would love your support through thoughts, prayers, and donations. Thanks for helping me help the world see! #OneSight [Link to fundraising page] Later this month I am headed to Bakersfield, CA to participate in a OneSight clinic. While there, I will help to provide eye exams and glasses to people who otherwise lack access to vision care. Please consider a donation to this important cause. #helptheworldsee #OneSight [Link to fundraising page] I ve been chosen to represent #TeamLuxottica as a 2017 OneSight team member in Dallas. 1.1 billion people don t have access to the vision care they need, and it s an honor to be part of the solution. Can t wait! #helptheworldsee [Link to fundraising page] Next week I will travel to Peru to provide eye exams and glasses to people without access to vision care. A donation of just $30 can help provide eye exams and glasses to a person in need. [Link to fundraising page] During your clinic Today we met Rose, a 15-year-old who kept to herself at school because she couldn t see the blackboard. Now she has access to vision care, a new pair of glasses and the confidence to participate. Donations make a difference! [Link to fundraising page] #OneSight #helptheworldsee The OneSight Vision Clinic in Vietnam has officially kicked off. #TeamLuxottica professionals and our local partner [INSERT NAME] have helped make this possible. #OneSight [Link to fundraising page] Post clinic In October, I was blessed to travel to Brazil with a very special group of people. We provided eye exams to over 3,800 people and cut over 1,500 pairs of glasses on site, in just 8 days. It was AMAZING! Our team has raised $12,699 so far, and we need help to reach our $19,000 goal. Please consider making a donation to support this important cause. Our trip has ended but the fight for access to global vision care is going strong!
