The Centers for Medicre nd Medicid Services (CMS) nd the Ntionl Center for Helth Sttistics (NCHS), two deprtments within the U.S. Federl Government s Deprtment of Helth nd Humn Services (DHHS) provide the following guidelines for coding nd reporting using the Interntionl Clssifiction of Diseses, 9th Revision, Clinicl Modifiction (ICD-9-CM).
