Hello Mr/Mrs., This is (interviewer s name) and I am calling on behalf of the Dental Practice-Based Research Network to interview you for the study named Development of a patient-based provider intervention for early tooth decay management. The interview will last about 20 minutes. Is now a good time to participate? Note to the interviewer: If the answer is no, ask When would be a good time to contact you for the interview? and call at a later date. If the answer was yes, continue. The goal of this study is to create patient education materials and a patient satisfaction questionnaire related to non-invasive treatment for early tooth decay. For the purpose of this study, non-invasive treatment is defined as clinical monitoring combined with prevention. Your dentist will not have access to your individual answers. You may refuse to answer any question and we can stop the interview at any time you wish. The interview will be recorded for accuracy. Please let me know at this time if you do not wish to record the interview. Do you have any questions before we begin? I ll begin by asking you a few questions about you and your dental treatment. Please choose the answer which fits best. 8) When was your last visit to the dental office? / / Note to the interviewer: If the participant had visited the dental office after the date the non-invasive treatment was provided, go to question 10, if not, go to question 11. 1

9) Do you recall going to the dental office on the date? (Note to the interviewer: please check all that apply): An examination of your teeth and mouth X-rays Information on how to prevent tooth decay Any other treatment to prevent decay Had your teeth cleaned A dental filling because of active dental decay Any other treatment (specify ) Note to the interviewer: If the participant received a filling because of active dental decay in the same visit, thank and end the interview. The next statements refer to the visit in which you received the brochure. For each statement, please indicate how much you or dis.
