Marco Campello, MA, PT Associate Clinical Director, Occupational and Industrial Orthopaedic Center, Hospital for Joint Diseases, Mt Sinai NYU Health, New York, New York, USA Mr. Campello has been involved in clinical work as well as research on occupational musculoskeletal disorders for the past 10 years and has published a number of articles in this field. He is a PhD candidate in the Program of Ergonomics and Biomechanics at New York University and is currently following up on 90 patients who underwent a multidisciplinary program to establish their retention and recurrence rates. Charles Clark, MD, MPH Visiting Associate Professor at the University of South Florida, Tampa, Florida, USA Dr. Clark graduated from the Johns Hopkins School of Medicine in 1964 and from the University of South Florida College of Public Health in Epidemiology and Biostatistics in 1995. For many years he has been interested in the return to work of patients with psychiatric illnesses and with chronic pain. More recently, he worked in Denver on the rehabilitation and return to work of severely mentally-ill, homeless persons and has authored the book AIDS and the Arrows of Pestilence, a social history of this epidemic disease. 2

Speaker Biographies Deborah V. Dibenedetto, MBA, RN, COHN-S/CM, ABDA President, DV DiBenedetto & Associates, Yonkers, New York, USA Ms. Dibenedetto has her Masters Degree in Business Administration. She, is a registered professional nurse, certified occupational health nurse specialist, a senior disability analyst and diplomat of the American Board of Disability Analysts. Prior to developing her own company, Ms. Dibenedetto was Senior Consultant and Workers Compensation Practice Leader for Watson Wyatt Worldwide, Principal Consultant at Price Waterhouse LLP, and Integrated Benefits Practice Leader/Senior Consultant at Alexander & Alexander Consulting Group. Her business background covers 25 years and includes work in senior management in manufacturing, transportation, and the service industries. Ms. Dibenedetto is president of the American Association of Occupational Health Nurses (AAOHN) and a member of the Society of Human Resources Management, the American Nurses Association, and Case Management Society of America (CMSA). She serves on the editorial board of the Occupational & Environment Medicine (OEM) Report, is the lead author of the OEM Occupational Health & Safety Manual, and she consults, lectures and writes on business and human resources issues. She is currently co-authoring a book on integrated disability case management and is a contributing author for a book on managed care and the CMSA s core curriculum. 3

Ingrid Laux, MD Former Medical Director, United Nations, New York, New York, USA Dr. Laux received her Medical Degree at the University of Innsbruck, Austria, completed her residency in psychiatry in Berlin, and in Public Health and Preventive Medicine with the New York City Health Department, and received her Masters Degree in Public Health at Columbia University. After her training, she worked with the New York City Health Department until 1975 when she joined the United Nations. From 1989 through 1999 she was Director of the Medical Service of the United Nations and in this capacity was responsible for the health and welfare of 35,000 staff members stationed in New York and in the field, as well as for the medical support to all peacekeeping operations. As Medical Director she also was the Medical Advisor to the Board on system-wide compensation claims and Medical Consultant to the Pension Fund of the UN common system. 4

Stuart W. Lewis, MD Senior Advisor, Occupational & Industrial Orthopaedic Institute, Hospital for Joint Diseases, Mt. Sinai NYU Health, New York, New York, USA Dr. Lewis began his medical career in private practice in general surgery. In 1974 he graduated from the Harvard Medical School and held a clinical rotation at the Montreal Neurological Institute at McGill Medical School, as well as a surgical clerkship at The Radcliff Infirmary, Oxford University Medical School. Last year Dr. Lewis joined the Occupational and Industrial Orthopaedic Institute, a division of the Department of Orthopaedic Surgery at the Hospital for Joint Diseases, as part of its multidisciplinary team of clinicians, researchers and educators where he will serve as Senior Advisor. He is also Adjunct Professor of Environmental Medicine at New York University Medical School. From 1988 to 1996, while serving as the Medical Director and Assistant Vice President of Occupational Health Services at the Metropolitan Transit Authority of New York City Transit, Dr. Lewis collaborated closely with the OIOC in producing Medical Standards for the MTA where they are still in use and serve as a model within the transportation industry. During this period, Dr. Lewis co-authored numerous publications, including the award winning paper The Woodbridge Foundation, on predictors of disability in patients with acute low back pain. Dr. Lewis has served as Chairman of the Board for the Bedford Stuyvesant Restoration Corporation, as well as a Director on the Board of the Bedford Stuyvesant Family Health Center. He, was a Director of the Herbert Birch Services and Chairman of the Membership Committee for the Museum of Modern Art: Friends of Education. He served on the President s Committee on Employment of People with Disabilities, and as Chair of the Membership Committee of the New York Occupational Medical Association 5

Chris Main, PhD, FBPsS Professor & Dept Head, Behavioural Medicine, Hope Hospital, Salford, UK Having qualified in Clinical Psychology at the University of Edinburgh in 1974, Dr. Main completed a PhD on Psychological Factors in Back Pain at the University of Glasgow. After completing a scholarship to North America in 1982, he moved to Salford and established the first pain management program (specifically LBP) in the UK. In 1997 he was elected Honorary Professor of Behaviour Medicine at the University of Manchester and Research Director of the Manchester and Salford Pain Centre. He has lectured widely, served on a number of government advisory committees in the UK and New Zealand, and has published extensively on the topic of LBP. Currently he is an Associate Editor of SPINE, Chair of the Educational Committee of The British Chapter of IASP; President of the Society for Back Pain Research; Scientific Chairman (1999-2000) of the International Association for the Study of Lumbar Spine; and has convened the IASP International Task Group on developing a curriculum on pain for psychologists. Barton Margoshes, MD National Medical Director & VP, Liberty Mutual Group, Boston, Massachusset, USA Dr. Margoshes is a Board Certified Internist with extensive experience in lost time and return-to-work issues. While he was the Associate Medical Director of Bell Atlantic, Dr. Margoshes helped re-engineer the disability case management program for occupational and non-occupational injuries and illnesses. In June 1996 he joined Liberty Mutual Group as their Vice President and National Medical Director, with strategic responsibilities for workers compensation and STD/ LTD case management, protocol and guideline development, utilization management, provider network quality and outcomes development. Dr. Margoshes practiced general internal medicine in Richmond, Virginia and has spoken nationwide on reengineering corporate health services, case management and return to work outcomes. 6

Margareta Nordin, DrSci Professor and Director, Occupational & Industrial Orthopaedic Center, Hospital for Joint Diseases Orthopaedic Institute, Mt. Sinai NYU Health, New York, New York, USA Dr. Nordin, is the Director of the Occupational & Industrial Orthopaedic Center at the Hospital for Joint Diseases, Director of the Program of Ergonomics and Biomechanics, and Research Associate Professor at the Department of Environmental Medicine, School of Medicine, New York University. She has written five books in basic biomechanics and occupational orthopaedics and published over 100 peerreviewed papers. Dr. Nordin and her co-workers received The Boeing Award for Excellence in 1980, Tibor and Dorothy Fabian Award in 1987, the Volvo Award in l988, the Woodbridge Award in 1996, and the Laborer s Clinical Research Award, the Arthritis Foundation in 1997. She has been a panel member of the Agency for Health Care Policy and Research for (AHCPR) Clinical Practice Guidelines for Acute Low Back Problems in Adults (1990-1994) and the Quebec Task Force on Whiplash Associated Disorders (WAD) (1992-1995) and the Paris Task Force on Exercises and Low Back Pain (1996-1998). Her interests include motion analysis, dynamic strength and fatigue measurements, and injury mechanisms related to workplace design. The focus of Dr. Nordin s research is prevention of disability related to musculoskeletal injuries and disorders at work. Dr. Nordin was the President of the International Society for the Study of the Lumbar Spine (ISSLS) in 1999-2000. 7

Jeffrey Perry, DO Medical Advisor, Occupational and Industrial Orthopaedic Centre & Physiatrist at the Hospital for Joint Diseases Spine Centre, New York; Director of Pain Management, Center for Women s Health, Darien, Connecticut, USA Dr. Perry received his medical degree from the New York College of Osteopathic Medicine and served a Rotating Internship at the Peninsula General Hospital Medical Centre. He completed his training in physical medicine and rehabilitation as Chief Resident at the Long Island Jewish Medical Centre and as Clinical Assistant Instructor at SUNY Downstate Medical Centre. He was a Fellow of Pain Management in the Department of Anesthesia at the State University of New York Health Science Centre in Brooklyn and has appeared as a medical expert on various radio broadcasts as well as being a frequent guest lecturer for Orthopaedic, Family Practice, and Neurology Board Review courses. Dr. Perry is a specialist in Physical Medicine and Rehabilitation (Physiatry) as well as pain management and provides physiatric care for patients at the Hospital for Joint Diseases Spine Centre and the Occupational and Industrial Orthopaedic Centre. He is currently participating in the National Institute of Health study with respect to operative versus non-operative treatment for back disorders. 8

Mohammed I. Ranavaya, MD, MS, FACPM, FAADEP Professor, Occupational & Environmental Medicine, Marshall University School of Medicine; Director, Appalachian Institute of Occupational & Environmental Medicine, West Virginia, USA Professor Ranavaya is a board certified specialist in occupational and environmental medicine and has performed several thousand independent medical evaluations. He is an internationally recognized expert on AMA Guides and has taught the skills for performing independent medical evaluations to over ten thousand physicians globally as well as how to be more effective in the use of the AMA Guides to the Evaluation of Permanent Impairment. Professor Ranavaya has authored several chapters in various books on Disability Medicine and numerous articles regarding AMA Guides, disability guidelines and independent medical evaluations. He is the Chair of the editorial board of the Medical Disability Advisor, a book on workplace disability duration guidelines for various injuries and diseases, and served on the senior editorial advisory board of the AMA Guides 5th edition and is a contributor to the AMA Guides 5th edition. Professor Ranavaya is the past President of the American Academy of Disability Evaluating Physicians, is one of the founders of the American Board of Independent Medical Examiners (ABIME) and has helped develop the certifying exam for independent medical examiners. 9

Presley Reed, MD Founder & Chairman of the REED GROUP Ltd, Boulder, Colorado, USA Dr. Reed, a corporate consulting physician and boardcertified psychiatrist who has worked in occupational health for fifteen years, is founder and Chairman of Reed Group Ltd. which specializes in medical disability case management programs to help control and reduce disability costs while improving employee performance and company productivity. Reed Group also publishes the MDA for Windows reference software and developed the MDA Case Manager software system. Dr. Reed is a fellow in the American College of Occupational and Environmental Medicine and chairs their Business and Labor Committee. He is also Past- Chairman of the American Board of Independent Medical Examiners and was appointed to the Technical Advisory Panel of the Disability Management Leadership Forum of the Washington Business Group on Health. He authored The Medical Disability Advisor: Workplace Guidelines for Disability Duration (Third Edition, published 1997). Sherri Weiser, PhD Psychologist, Occupational & Industrial Orthopaedic Center, Hospital for Joint Diseases, Mt. Sinai NYU Health, New York, New York, USA A licensed psychologist, Dr. Weiser is Coordinator of Psychological Services at the Occupational & Industrial Orthopaedic Center as well as Assistant Professor in the Program of Ergonomics & Biomechanics at NYU Health. For 15 years she has been involved in clinical work and research on psychosocial factors of LBP. She has published a number of papers and lectured extensively on the subject. 10
