Stage II - Skin blisters or forms an open sore. The area around the sore may be red and irritated. Stage III - The skin breakdown now looks like a crater where there is damage to the tissue below the skin. Stage IV - The pressure ulcer has become so deep there is damage to the muscle and bone, and sometimes tendons and joints. 6

Injury and Poisoning Open Wounds (870-897) Superficial Injury (910-919) Contusion with Intact Skin Surface (920-924) Burns (940-949) Site Burns Severity (degree) of burn Total Body Surface Area 7
