Dear Prospective or Returning JDRF Youth Ambassador: We would like to invite you to participate in the 2017 JDRF Youth Ambassador Program. This group is made up of children and their families living with type 1 diabetes (T1D), between the ages of 6 and 18 (first grade to senior year in high school), who are interested in becoming involved with the educational and volunteer efforts of the JDRF Greater Fort Worth/Arlington Chapter. This group provides awareness of T1D to our community and helps support the mission of JDRF as we work to fund the research to find better treatments and a cure for T1D. There will be a variety of opportunities for the Youth Ambassadors to volunteer individually and as a group. For our older YAs, these opportunities are a great way to start or continue to build your college resume. For our younger members, the program is a great way to build confidence and meet other people living with T1D while being a part of a group supporting JDRF and T1D research. Our goal for program participants is to feel ownership and pride in their group while continuing the progress of education and fundraising. We are making adjustments to the Youth Ambassador Program in an effort to make it more formalized than it has been in years past. The efforts of the YA program are a tremendous value to the local JDRF community as well as the growth and personal development of our Youth Ambassadors. We hope you will choose to participate in the JDRF Youth Ambassador Program. You have the ability to use your voice to spread diabetes awareness and help JDRF in many ways that only you can! We need your voice, your participation, and your leadership. Enclosed you will find a sheet outlining the responsibilities that you will be asked to fulfill, as well as, a JDRF Youth Ambassador Program application form. REQUIREMENTS/RESPONSIBILITIES 2017 Requirements 1. Have T1D 2. Be willing to share your personal story in front of large groups and the media. 3. Have a positive outlook on life and diabetes 4. Ages 6-18 years old and/or 1 st 12 th grade 5. Obtain parental permission in writing to be a part of the Youth Ambassador Program 6. Commit to participating in the program for 2017 (January 2017 December 2017) 7. Complete a minimum of 20 hours of participation in volunteer service at JDRF events and community outreach programs 8. Sign up to be a JDRF Advocate (advocacy.jdrf.org) 9. Organize your own or join and fundraise for a JDRF One Walk team 10. Start a conversation with your school administration about hosting a JDRF Kids Walk or JDRF Fun Run 11. Arrange for transportation to and from JDRF responsibilities and social activities 12. Must abide by and maintain good School District or school Code of Conduct policies 13. Keep your parents involved Responsibilities (Participation in some activities may be based on age appropriateness) 1. Health education in the community/volunteer efforts for JDRF through: a. Public speaking and presentations to schools, hospitals, companies, and organizations b. Production of radio and TV PSAs c. Health Fair/TypeOneNation Summit participation 2. Perform general public relations activities 3. Help JDRF create and distribute awards, and other special gifts and recognition for our community partners 4. Provide office assistance (assist with art projects, writing notes and invitation mailings, etc.) 5. Participate in Promise Ball activities 6. Participate in Walk activities 7. Participate in Promise to Remember Me Meetings & JDRF Advocacy efforts 8. Make calls to JDRF donors and sponsors to thank them for their support 9. Please write your answers in complete sentences. Your personal statement could include more information about your hobbies and interests, and it should also include at least one experience, activity, or type of personal growth (e.g. become more outgoing, get better at public speaking, etc.) that you want to gain from your time as a JDRF Youth Ambassador. Please also tell us about your current involvement with JDRF.

Code of Conduct As a JDRF Greater Fort Worth/Arlington Chapter Youth Ambassador you are valued and appreciated for your commitment to the organization. Your pledge as an Ambassador comes with accountability. Youth Ambassadors are expected to conduct themselves in a manner that allows the furtherance of the JDRF mission. Treating each other with the respect, being punctual, a willingness to help and learn, a positive attitude and adherence to the volunteer conduct guidelines is expected! For this reason JDRF has listed the following expectations: If any of the following are violated a warning will be issued and an in person meeting with a parent/guardian will be set. 1. Be courteous and respect the opinions of your peers, JDRF staff, donors, and JDRF guests. 2. Absolutely NO verbal abuse or bullying of any kind will be tolerated. 3. Use of electronics (including cell phones) should be extremely limited during Youth Ambassador events. Hand held gaming devices do not have a place at JDRF events and activities. Cell phones are allowed for emergency and communicating with parents/guardians use only CGM usage of cell phones is always allowed. 4. Youth must stay in groups of two or more at all times and you must notify a Youth Ambassador leader of your whereabouts. No one is permitted to go anywhere without a Youth Ambassador lead s approval. 5. No one is permitted to get into a vehicle and travel anywhere with another Youth Ambassador or guest without adult/parent/guardian approval. 6. Punctuality is a high priority. Arrive 10-15 minutes prior to your scheduled time for all JDRF related activities, events, and/or meetings where you are scheduled to appear. 7. Attend all socials and volunteer opportunities where you sign up to be present more than two (2) no shows will constitute as your resignation from the Youth Ambassador Program. Dress Code As a Youth Ambassador, please remember that you are always representing JDRF in the community. The way you present yourself and the way you dress directly reflect on JDRF s reputation and can influence potential and current donors, sponsors, and supporters views of JDRF. You are an extension of JDRF and you represent the organization! We want everyone to dress sensibly for the volunteer duty, social, and activity to avoid injuries and exposure to the sun. 1. All clothing should be of appropriate length and fit (at minimum fingertip length for shorts, skirts, and dresses). 2. No undergarments should be exposed 3. No t-shirts with inappropriate, controversial, obscene, or suggestive sayings 4. No mid-drift tops Page 7 of 8
