SELECT CLEANSE A DETOXIFICATION POWDER DESIGNED SPECIFICALLY FOR TODAY S CHRONICALLY ILL, CHRONICALLY OVERWEIGHT PATIENT As you all know, there are currently many detoxification powdered products on the market from which you can chose. Why should you preferentially consider our new, updated formulation found in Select Cleanse? There are two reasons: 1. It contains superior versions of the key nutrients you would expect to find in any top-grade detoxification powder. 2. It contains factors that are unique to Select Cleanse that address the unique needs of chronically ill, chronically overweight patients. Since both of these points are extremely important to consider when weighing your decision to purchase Select Cleanse for your patients, I would like to discuss each in detail. Superior versions of key nutrients that are required for optimal detoxification Protein type The protein type in Select Cleanse is exclusively non-gmo yellow pea protein isolate. Furthermore, its amino acid profile is quite favorable in terms of the formation of key detoxification enzymes and factors such as glutathione which lie at the heart of any detoxification process. However, concerning the protein type, what is not in Select Cleanse may be even more important that what is in Select Cleanse. Why? Many detoxification powders on the market contain rice protein. Why is this a June 2015 #279 significant issue? Unacceptably high levels of arsenic are virtually ubiquitous in the world s rice supply today. So that you can have more details on this vitally important issue, I would now like to review some selected quotes from the paper Arsenic in rice: A cause for concern by Hojsak et al (Hojsak I et al. JPGN, Vol. 60, No. 1, pp 142-145, January 2015). The first quote provides some general information about arsenic toxicity: Arsenic is a ubiquitous metalloid that is found in nature in organic and inorganic forms. The harmful effects of arsenic are well established; however toxicity depends on the form (inorganic or organic) and the oxidation state of arsenic compounds. It is generally accepted that inorganic arsenic is much more toxic than organic arsenic. As you will see in the quote below, the reason that organic arsenic is of little concern has to do with how the body handles it: The bioavailability of different forms of arsenic also varies. Most of the organic compounds are found in seafood, and when ingested, they undergo little biotransformation and are excreted almost unchanged. In contrast, consider the following about inorganic arsenic: By contrast, inorganic arsenic, which is largely responsible for the toxic effects, is found mostly in water in specific geographic areas and in rice and rice products. Inorganic arsenic is considered a first-level carcinogen because long-term exposure is associated with an increased risk for various carcinomas including skin, bladder, lung, kidney, liver, and prostate. Furthermore, exposure is also associated with alterations in gastrointestinal, cardiovascular, hematological, pulmonary, neurological, immunological and reproductive/developmental function.

The next quote provides more detail about the relationship between inorganic arsenic and rice: Differentiation between total and inorganic arsenic is important because some foods such as fish and seafood with the highest total arsenic content have very low levels of the toxic inorganic form. In contrast, the concentration of inorganic arsenic is high in rice and can reach 90% of the total arsenic content. The concentration varies according to the soil where the rice was cultivated and the type of rice. The high arsenic concentration in rice compared with that of other grains reflects the anaerobic growing conditions of flooded rice paddies and the unique physiology of the plant that allows it to take up and sequester arsenic from the environment, accumulating it to a greater extent than the soil. The next quote provides still more information on the specifics of arsenic contained in rice: The arsenic content of raw rice varies from 0.1 to 0.4 mg of inorganic arsenic/kg of dry mass. Rice has a much higher arsenic level than that in other grains such as wheat and barley, for which the reported total arsenic content is 0.03 to 0.08 mg/kg. Moreover, the quantity of inorganic arsenic in cereals based on grains other than rice was less than detection levels. The arsenic content in rice varies depending on the type of the rice cultivar, the place where it was cultivated, and how it was processed; brown rice contains higher concentrations than white rice. Moreover, changes in arsenic content may occur during the preparation of food in which the cooking water seems to be of special importance: cooking rice in uncontaminated water can reduce the arsenic content of the rice. What can be stated specifically about rice protein? Hojsak et al point out: One study reported that inorganic arsenic in rice protein-based infant formula was 7.3 times lower than the amount reported by the European Food Safety Authority for infant rice-based products, which is more than 15 times higher than regular cows milk-based formula Therefore, as you can see, while the inorganic arsenic content of rice protein is not as significant as other rice-based products, based on its comparison to cow s milk, it still is of major concern. Because of the above it is our opinion that rice containing substances no longer have a rightful place in any detoxification product or, for that matter, any product in the Moss Nutrition Professional Line. Micronutrients Many micronutrients serve as key cofactors in many detoxification processes. Therefore, particularly given the patient population for which Select Cleanse was primarily intended, we insisted, contrary to what is seen in many detoxification powdered products, on the most bioavailable, easiest to metabolize forms. These include: Riboflavin-5-phosphate Pyridoxal-5-phoshate Folate as (6S)-5-methyltetrahydrofolate (MTHF) Vitamin B12 as methylcobalamin Highly absorbable Albion mineral chelates of the following minerals: calcium, magnesium, zinc, selenium, copper, manganese, chromium, molybdenum, and potassium. Factors that are unique to Select Cleanse that address the unique metabolic imbalances of chronically ill, chronically overweight patients chronic inflammation and insulin resistance The impact of inflammation on detoxification Much of the research upon which the formulation of typical detoxification powdered products is based, is from studies of healthy individuals. For example we all know that glutathione is a key factor in many detoxification processes. Since sulfur is an integral component of glutathione, all detoxification powdered products, including Select Cleanse contain sulfur-based compounds such as methylsulfonylmethane 2

(MSM), N-acetyl cysteine (NAC) and others that have been demonstrated to increase glutathione production. Of course, the implicit assumption is that when these compounds are provided to patients, increases in glutathione will be the inevitable result. Interestingly, this may be true in the healthy, textbook individuals that form the basis of many if not most studies on the subject. But what about your patients who are most likely overweight and chronically ill? As I have demonstrated repeatedly in many of my newsletters and seminars over the years, they are different in many ways compared to the healthy, textbook individuals mentioned above. However, two of the most important ways is that they very likely are experiencing chronic inflammation and insulin resistance. As noted by Robert Grimble in Sulfur amino acids, glutathione, and immune function (Grimble R. in Masella R & Mazza G, Glutathione and Sulfur Amino Acids in Human Health and Disease, John Wiley & Sons, Inc., Hoboken, NJ, pp. 273-288, 2009) inflammation has a profoundly negative impact on sulfur status: The pro-inflammatory cytokines interleukin (IL)-1, IL-6, and tumor necrosis factor alpha (TNFα), have widespread metabolic effects on the body and stimulate the process of inflammation. Many of the signs and symptoms experienced after infection and injury, such as fever, loss of appetite, weight loss, negative nitrogen, sulfur, and mineral balance, and lethargy are caused directly or indirectly by pro-inflammatory cytokines. A good example of this adverse impact on sulfur and glutathione metabolism can be seen with HIV, as noted by Grimble: Infection with human immunodeficiency virus (HIV), however, has been shown to cause substantial excretion of sulfate in the urine during the asymptomatic phase of the disease. The losses reported were equivalent to 10 g of cysteine per day, in contrast to losses of approximately 3 g/d for healthy individuals on a Westernized diet. As cysteine is the precursor for both sulfate and glutathione (GSH) this finding may be linked with the decline in tissue glutathione pools that has been observed in HIV infection. Clearly such a depletion of antioxidant defenses will not be sustainable over a long period. Of course, in most instances, your patients have been chronically ill and chronically inflamed for a long period. Therefore, we strongly believe at Moss Nutrition that an effective powdered detoxification product must compensate for chronic inflammation if is truly going to deliver maximum detoxification benefit in the real life, chronically ill, chronically overweight patients that form the bulk of our practices. To that end, Select Cleanse contains an ingredient that has powerful anti-inflammatory properties. In addition, it contains still another ingredient unique to powdered detoxification products that aids in neutralizing the other metabolic enemy of detoxification, insulin resistance. Before discussing these factors, though, I would like to present additional research which demonstrates in an emphatic fashion how much inflammation can adversely affect detoxification processes. First, in relation to the extremely important Phase I, cytochrome P450 enzymes, Morgan states the following in Regulation of cytochrome P450 by inflammatory mediators: Why and how? (Morgan ET. Drug Metab and Disposition, Vol. 29, No. 3, pp. 207-212, 2001): In humans and animals, infections or inflammatory stimuli cause changes in the activities and expression levels of various forms of P450 in the liver, as well as in extrahepatic tissues such as kidney and brain. In most cases, P450s and their activities are suppressed, but some are unaffected or induced under these conditions. P450 suppression can result in increased clinical toxicity of drugs with a low therapeutic index. Conversely some drugs must be converted to their pharmacologically or toxicologically active metabolites by P450 enzymes, and suppression of their metabolism during an inflammatory response can lead to a reduced therapeutic or toxic effect. 3

What about the impact of inflammation on Phase II, conjugation enzymes? Mimche et al (Mimche SM et al. Hepatic cytochrome P450s, phase II enzymes and nuclear receptors are down regulated in a Th2 environment during Schistosoma mansoni infection, Drug Metab and Disposition, Vol. 42, pp. 1324-140, January 2014) state the following: It has been well documented that infection and inflammation cause impairment of hepatic drug metabolizing enzymes (DME) in animals and humans. This is associated with the down regulation of hepatic P450s, phase II DMEs [including UDPglucuronosyltransferase (UGTs) and sulfotransferase (SULTs), and drug transporters, leading to alterations in pharmacokinetics and drug clearance. The impact of insulin resistance on detoxification As noted by the title of the paper by Engstler et al (Engstler AJ et al, Gut, published online ahead of print, May 25, 2015), Insulin resistance alters hepatic ethanol metabolism; studies in mice and children with non-alcoholic fatty liver disease, insulin resistance can have a profoundly adverse impact on detoxification pathways. Meriva The unique anti-inflammatory ingredient present in Select Cleanse Select Cleanse is unique among powdered detoxification products in that it contains the highly bioavailable curcumin product Meriva Curcumin Phytosome, which, as I have discussed previously, has proven in several studies to have significant anti-inflammatory properties. (Please see Product Review Newsletter #267 for my earlier discussions). Leucine The unique insulin sensitizing ingredient present in Select Cleanse In addition to the leucine found naturally in pea protein, Select Cleanse is unique among many powdered detoxification products in that it contains added leucine. Why is this important in relation to the optimization of insulin metabolism? Van Loon, in his paper Leucine as a pharmaconutrient in health and disease (van Loon LJC. Curr Opin Clin Nutr Metab Care, Vol. 15, pp. 71-77, 2012), states the following: The insulinotropic properties of amino acids, and leucine in particular, may be of relevance as a means to stimulate endogenous insulin release and, as such, to improve postprandial glycemic control in type 2 diabetes patients. Although long-standing type 2 diabetes patients generally show a blunted insulinotropic response to carbohydrate intake, this response can be substantially increased by the co-ingestion of protein and/or amino acids. We feel that in the real world where many of the candidates for detoxification in your practice may be diabetic or have tendencies towards insulin resistance, Select Cleanse should give you a unique head start towards improved detoxification and patient improvement.
