In order to avoid unnecessary delays in the processing of your Privileging Application, please read these instructions carefully. 3. Professional or Read-Only Groups interpreting imaging procedures at a physician office or hospital must complete Section II in addition to all other relevant sections. 4. If the location does NOT provide or interpret imaging services, fill in the practice name, address, Tax Identification Number, and Highmark billing provider number in Section I-A and check NO in the Practice Information Section (I) that asks: Does this practice perform and/or interpret diagnostic imaging studies? 5. B. Radiology Over-Read List the name, and for the individual physician or group of radiologists that performs radiological interpretation services for the practice listed in Section I. This location has an established quality control program. 4. This location has an established radiation safety program and ALARA program. 5. This location is staffed by a board certified radiologist during all business hours. 6. If utilizing equipment producing ionizing radiation, this location has a current (within 3 years) letter of state inspection, or calibration report, or physicist s report. 7. This location prepares and provides a written report for each imaging study performed within 10 business days from the date of service to the ordering physician. 8. If performing mammography, this location has a current MQSA certificate. 9. If performing bone densitometry, this location employs at least one physician and one technologist who: Are certified by the International Society for Clinical Densitometry (ISCD), or Are in the process of becoming certified, or Have not begun the certification process 10. If performing nuclear cardiology, PET and/or any nuclear medicine procedures, this location has a current radioactive materials license. If YES, please provide a copy. 11. If performing PET, this location: Is accredited by the American College of Radiology (ACR) or the Intersocietal Commission for the Accreditation of Nuclear Labs (ICAEL), or 12. If performing nuclear cardiology, this location s imaging system has the capability of assessing both myocardial perfusion and contractile function (ejection fraction and regional wall motion). 13. If performing cardiac stress studies, the studies are performed under the direct supervision of a licensed physician who has a current Advanced Cardiac Life Support (ACLS) certification. 14. If performing nuclear cardiology, this location employs at least one physician who: Is certified by the Certification Board of Nuclear Cardiology (CBNC), or Is sitting for the next scheduled CBNC examination, or Meets the American College of Cardiology/American Society of Nuclear Cardiology COCATS training guidelines level 2, or Is board certified in diagnostic radiology or nuclear medicine, or None of the above 15. If performing nuclear cardiology, this location: Is accredited in nuclear cardiology by the American College of Radiology (ACR) or the Intersocietal Commission for the Accreditation of Nuclear Labs (ICAEL), or 16. If performing MRI, this location: Is accredited by the American College of Radiology (ACR), or 17. If performing MRI/CT, this location offers MRI/CT diagnostic imaging services for a minimum of 40 hours per week. 18. If performing MRI/CT, this location is staffed on-site during the hours outlined in the hours of operation requirement by a credentialed radiologist who has a current Advanced Cardiac Life Support (ACLS) or Advanced Radiology Life Support (ARLS) certification. -Utilizes Teleradiology (must complete teleradiology application) 19. If performing CTA, this location utilizes a: 40 Slice Scanner 64 Slice Scanner Other 20. If performing CTA, the radiologist meets the Qualifications of Personnel outlined in the ACR Clinical Statement on Noninvasive Cardiac Imaging for Cardiac CT. 21. If performing general ultrasound, studies are performed under the personal supervision of a physician credentialed in diagnostic radiology. 22. If performing general ultrasound, this location employs a technologist or sonographer certified by the ARDMS or ARRT. 23. If performing obstetrical ultrasound, this location: Is accredited by the American College of Radiology (ACR) or the American Institute of Ultrasound in Medicine (AIUM), or 24. If performing obstetrical ultrasound, this location: Employs a perinatologist 25. If performing gynecological ultrasound, this location: Is accredited by the American College of Radiology (ACR) or the American Institute of Ultrasound in Medicine (AIUM), or 26. If performing urological ultrasound, the studies are performed under the personal supervision of a credentialed radiologist or urologist. 27. If performing urological ultrasound, this location employs a technologist or sonographer certified by the ARDMS or ARRT. 28. If performing breast ultrasound, this location: Is accredited in breast ultrasound by the American College of Radiology (ACR), or Is accredited in breast ultrasound by the American Institute of Ultrasound in Medicine (AIUM), or Is accredited in breast ultrasound by the American Society of Breast Surgeons (ASBS), or 29. If performing cardiac ultrasound (echocardiography), this location: Is accredited in cardiovascular ultrasound by the Intersocietal Commission for the Accreditation of Echocardiography Labs (ICAEL), or 30. If performing cardiac ultrasound (echocardiography), the system has Color Flow Doppler capability. 31. If performing peripheral vascular ultrasound, this location: Is accredited in peripheral vascular ultrasound by the American College of Radiology (ACR) or the Intersocietal Commission for the Accreditation of Vascular Labs (ICAVL), or 32. V. DECLARATION AND RELEASE I hereby declare the above information to be true, accurate and complete. I authorize Highmark and National Imaging Associates, Inc., to verify any of the information given. I understand and agree to comply with the imaging requirements established by Highmark. I agree to produce documentation supporting current compliance with the above requirements, if requested. I acknowledge a continuing obligation to supplement the answers contained herein and agree to report any changes in this information within fifteen (15) days of the change coming to my attention. I hereby acknowledge that providers/practices that are determined to have misrepresented information on their Privileging Application, failed to comply with imaging requirements established by Highmark, or have failed to provide imaging services and/or procedures consistent with the proper practice of medicine, and in accordance with the rules and regulations of the applicable federal, state and professional licensure boards and agencies, may be subject to termination of imaging privileges by Highmark. I hereby release from any and all liability Highmark, its subsidiaries and affiliates, and National Imaging Associates, Inc., its employees, and agents fro relying on this information.
