Welcome to SeeYourChart SeeYourChart is set-up through Cancer & Hematology Centers of Western Michigan s office to provide you with access to your personal medical information and helps you to stay connected with your health care team. You will be able to access many different resources all from one, easy to use website. Just signup in our office and sign-in from our homepage (www.chcwm.com) and you will find your medical chart, calendar, journal, and more waiting for you.

It s Easy to Enroll When you sign-up in our office for SeeYourChart, you will receive either an email or a printed form that will provide you with the following details: a PIN number that identifies your oncologist an access code that grants your initial access to SeeYourChart and allows you to create your own Username the registration page: https://secure.seeyourchart.com/newuser.aspx
Your Dashboard The first thing you will see when you log into SeeYourChart is your Dashboard. Your Dashboard displays your About Me section, your most current journal entry, information about upcoming appointments and a quick access menu into personal medical information.
Your Calendar The calendar page of SeeYourChart displays your treatment and appointment schedule. The sample below shows a treatment appointment at 10:30am on Friday, September 12.
Your Medical Information SeeYourChart is meant to bring your medical information to you easily. The records tab on your SeeYourChart can link you to several pages, including: Summary, Vitals, Intake Forms and Demographics.
Your Medications On SeeYourChart you can view a list of the prescribed medications you are taking, as well as add any over the counter medications you happen to take. The medication page allows you to make notes about your medication, track dates, and read the instructions for use.
The SeeYourChart Forum The forums on SeeYourChart are a great place to connect with other patients from around the country. There are forums on various topics relating to cancer and other diseases. If you do not see a discussion topic you want to join, feel free to start a topic of your own that is interesting to you.
COMING SOON The SeeYourChart Mail The SeeYourChart (SYC) messaging system allows you to securely communicate with our office. SYC Mail provides various templates for our collective communication, including options for appointment changes, treatment questions, or billing issues (just to name a few). SYC Mail helps to create a faster and more efficient way to communicate with your healthcare team for non-emergency situations.
The SeeYourChart Journal Your SeeYourChart Journal allows you to write about what s going on in your life or treatment. Add an image to go along with your post, or leave it blank; the choice is yours.
Be Informed In addition to access to your personal medical information, SeeYourChart provides learning tools related to disease within the Be Informed tab. This resource section allows you to watch videos, and read about your disease, side effects, and various health related topics.
