1 Local Action Plan -2019 SOUTH WEST Background The south west area covers Cornwall, Devon, Somerset, Wiltshire, Gloucestershire, Bristol and Bath Map As of 31 st December 2016 there were around 270 people known to be living with MND, five MND Association branches, five MND Association groups, a MND Association care centre and MND Care Network in the area. The number in the population is around five million. Overall the region is relatively affluent but there are marked areas of deprivation, dense urban population and rural isolation. This plan was developed following: discussions with regional staff and key stakeholders (branches, groups and NHS/social care colleagues) Analysis of information both internal within the Association and external It considers how well services currently match up to the strategic goals of the Association, and identifies a number of priorities for action from 1 January to 31 December 2019. 2 What are we going to do? How will we do it? Who will be involved? RDP Outcome 1: Intelligence gathering, needs assessment and analysis Gather and analyse data from the Transforming MND Care audit for care centres/networks and for other services not funded by the Association Support care centres/networks to complete the audit as part of their contractual obligations Support at least two other teams/services to complete the audit each year CCC/CNCs Timeframe December Outcome Activity 1a, 1b Gather and analyse data from other sources in relation to regional services Use data from the Improving MND Care surveys (one of pwmnd and one for carers) to highlight shortfalls 2018 1c, 1d, 1e Prioritise available data and intelligence to inform local and national campaigning, where appropriate Develop the south west campaigns forum, to be held three times annually. Encouraging collaboration and joint working between regional staff and agreeing ways of using local data to inform local campaigning. CM 1f, 1g, 1h Forum to identify issues of campaigning priority from mounting evidence of problems being experienced by plw MND and their carers, and strengthen the links between the and campaigns contacts. Existing areas for discussion: Wheelchair provision in Devon Timely access to equipment Timely access to diagnostic tests in Somerset Access to paid carers Changes to MND or specialist posts Follow up post MND Charter adoption relationships between branches/groups/campaigns Contacts and key decision makers in the local area. RDP Outcome 2: Driving service improvement CM with CCs

3 Create and implement improvement plans following the use of the Transforming MND Care audit Work with the Peninsula MND Care Network team to implement the improvement plan, as required Work alongside at least one other team/service in Cornwall, Devon and Somerset each year who have undertaken the audit, to implement their improvement plan December December 2a, 2b Work with the Bristol MND Care Centre team to implement the improvement plan, as required Work alongside at least one other team/service each year in Bristol and West area who have undertaken the audit, to implement their improvement plan December December Use influencing and negotiation to improve health and social care services not funded by the Association where there are identified shortfalls Use LDT meetings to help identify shortfalls, and work with regional staff and campaigns contacts to influence better provision Target commissioners where there appear to be fundamental shortfalls in service provision /CCs 2018 and ongoing 2c RDP Outcome 3: Protecting and extending care and support Manage the contract and relationships Regional staff to attend, and actively participate in, care with care centres and networks centre/network steering group meetings Support and develop MDTs Support and further develop the MDT in Gloucestershire Ensure each MDT meeting has updated terms of reference RDM/ every 6 months 2018 2018 3a 3b Monitor the co-ordination of care in Somerset through responses to the next Improving MND Care survey 2018 Increase the number of referrals to the Association Monitor existing referral pathways, analyse where the number pwmnd known to the Association does not correlate to expected prevalence and take action where appropriate. 3c

4 Improve referral pathway to the Association in Somerset and monitor numbers of referrals Proactively encourage and monitor hip numbers Standardise referral pathways to AVs, introducing the AVC role where appropriate / 2018 Facilitate the delivery of a programme of regional masterclasses Promote the regional masterclass on CI/FTD in Taunton in April April 3d Contribute to the development of the ongoing masterclass programme, ensuring these are promoted throughout the south west to key HSCPs. Provide training and development opportunities for HSCPs according to local needs and priorities Support the Peninsula Care Network team to plan and deliver a study day for HSCPs annually Support the Bristol MND Care Centre team to plan and deliver a study day for HSCPs annually. In this will focus on respiratory care and be planned jointly between the coordinator and Bath MND nurse 3e Improve the knowledge of acute hospital staff in Cornwall. Promote education opportunities for HSCPs to ensure there is good succession planning CNC/ Deliver annual study opportunities in Gloucestershire, working with the specialist nurse Continue the Special Interest Group in Exeter /CNC Develop Special Interest Groups in Cornwall and Somerset 2018 /CNC Recruitment, retention and succession Develop and refresh the volunteer recruitment plan VDC planning of all regionally based volunteers Further develop and support branches/groups in: VDC 3f

5 North Devon Plymouth Torbay Bristol/Bath/West Wiltshire South Wiltshire S&E Somerset Cornwall Introduce new approaches to the support of male carers postbereavement, eg. recruitment of male volunteers VDC Increase cross branch/group working and communication to improve awareness, support and fundraising, and use our volunteer resource more effectively All of LDT Provide support and learning and development opportunities for all regionally-based volunteers Deliver AV development and training, as agreed by the Care Volunteering sub-group. To include at least one AV development day per year 3g Hold development events, and offer other development opportunities, for other volunteers across the region VDC Offer at least one branch/group working together workshop to provide an opportunity for branches/groups to share ideas and good practice in relation to fundraising, support and care, campaigning, awareness. Regional staff team 2018 Promote the Champion the Charter campaign as a means to influence local authorities and open doors for service development Develop and deliver local campaigns where needed Increase the number of councils that have adopted the MND Charter in the south west from two to five (uplift of three) Following adoption, encourage an ongoing relationship between the Campaigns Contact, Branch/Group and the council, to aid monitoring and suggested improvements to the services they offer for plwmnd and their carers. Use local data to inform priorities for local campaigning CM with CC s CM/ with CCs 3h 3i

6 Support the delivery of national campaigns Build and strengthen relationships between campaigns contacts and in order to better inform localised campaigning efforts and provide evidence Continue to promote the Champion the Charter initiative Support national and localised engagement of Financial Support (working title) campaign when launched in. 3j Promote and increase numbers in the Campaigns Network In future years, to be advised of the national campaigns and promote as appropriate. Promote the campaigns network at meetings with volunteers and key stakeholders to increase the number signed up to the network from 440 to 493 across the Southern region uplift of 12% Year-onyear increase 3k Other volunteers (eg. AVs) to promote the Campaigns Network role when in contact with pwmnd and their families /AVs Increase the number of campaigns contacts in the south west from three to four in, and with increases in subsequent years VDC and CM Year-onyear increase Provide awareness and training to branches, groups and other volunteers to improve knowledge and skills Increase the number of branches and Eight of twenty-nine branches across the Southern region to groups involved in campaigning be fully engaged in campaigning activities RDP Outcome 4: Innovation and new work Develop new volunteering roles Implement the volunteer recruitment plan for the south west, introducing the new roles where identified they are needed CM, VDC and CC s CM and CC s VDC 4a Develop new care and support services Introduce new care volunteer roles, where needed VDC 4b Develop and deliver projects funded by Ice Bucket Challenge monies Evaluate the Swindon post, and subject to positive evaluation, develop a business case for the continuation of funding June 4c Evaluate the Bath post, and subject to positive evaluation, develop a business case for the continuation of funding June 2018

Penhaligon s Trust) Map provision of services by CRUSE and other bereavement organisations and develop local relationships where this will improve the services for bereaved carers, C&YP manager, regional staff 2018 4f 5a Focus on the whole family approach as subject for AV learning and development in Form and manage relationships, partnerships and alliances and work effectively with them Develop relationships with hospices and other organisations that also provide support for carers, children and young people, and in bereavement / 5b RDP Outcome 6: Raising awareness, promotion and marketing

8 Promote and market our resources and services to pwmnd/families/carers Actively engage in MND Awareness month in June each year, sharing plans, and promoting activities 6a Promote and market our online and other educational resources, events and activities to HSCPs Ensure use of social media to promote events and resources Promote the use of the Professionals Forum Ensure website is updated with local events / CNC/CCC 6b Raise awareness of MND and promote the work of the MND Association with key regional stakeholders Promote NICE guideline NG42 with HSCPs, with pwmnd and carers Identify opportunities for raising awareness to celebrate the 40 th anniversary of the founding of the Association member 2019 6c Promote research opportunities for pwmnd Actively promote research opportunities to pwmnd in the south west through branch and group newsletters and social media / CNC/CCC/ 6d Include research updates in MND study days were appropriate / CNC/CCC Ensure the regional conferences each year include a presentation on research RDM Promote the MND Association research portfolio Actively promote the MND register once it is fully-developed amongst HSCPs and pwmnd CNC/CCC/ 2018/9 6e Promote the research portfolio to potential funders/ donors CNC/CCC/ Work alongside the national communications team to identify and respond to regional media opportunities Suggest suitable case studies for personal stories Respond the other media opportunities on an ad-hoc basis 6f
