PAGE 2 PAGE 3 Ethics and Mental Health Law in the Treatment of Individuals with Co -Occurring Disorders Presented by: Catherine Burns, Ph.D. and Sheryl Bellman, M.A., M.S., LCMHC, LADC Treatment provision for clients with co -occurring needs has become increasingly complex in the 21 st century. Participants in this workshop will review the main tenets of The Health Insurance Portability and A ccountability Act as well as 42 CFR Part 2 that comprise the cornerstones of clinical practice for health, mental health and substance abuse programs. A model of ethical decision making will be reviewed and applied to case studies. Ethical issues pertaining to the provision of emergency mental health care and crisis services will be reviewed with the goal of expanding participants understanding of current laws and practice. Finally, basic issues of boundaries, dual relationships and other challenges that can result in compromised care and professional discipline will be explored (e.g., sexual relationships, negligence, fraudulent acts, and compromised capacity or competence to provide treatment). Participants should expect a combination of lecture and group activity through -out this day-long workshop. Cath Burns, Ph.D., Licensed Psychologist Doctorate, Licensed School Psychologist is the Director of Evaluation and Outcomes for MHSAS at HowardCenter with 17 years working within Vermont s community mental health system, and an additional 10 years working in public schools. Cath has provided training around the State for practitioners in community mental health and education, as well as has been an instructor at Johnson State College, the University of Vermont and in the Program in Community Mental Health for graduates and undergraduates studying development, ethics, and research methods and statistics. She earned a Ph.D. in Developmental Psychology and a Master s degree in Psychology from the University of Vermont, and a Master s degree in Educational Psychology from the University of Colorado at Boulder. Sheryl Bellman, M.A., M.S., LCMHC, LADC has worked in the mental health field for over 40 years, starting at Manhattan State Hospital in New York City and then at two Community Mental Health Agencies in Vermont. Sheryl has been at HowardCenter for 37 years and brings with her a wealth of experience. She has been the Director of Crisis Services since 1992, and has worked to create an array of services that are effective yet respectful, compassionate and responsive to individuals when they are most vulnerable. She strives to be a strong advocate and is passionate about changing the system in which we serve mental health patients. September 19, 2014 9 a.m. 4:30 p.m. 6 (six) Continuing Substance Abuse 101 Presented by: Joe Lasek, M.D. And Jennifer Spagnuolo, MS-LCMHC, LADC The use of drugs and alcohol in our society has reached epidemic proportions over recent decades. Standard of care has moved to co-occurring treatment of mental health and substance abuse problems, and knowledge of how to best help and treat clients with substance use issues is beneficial to all clinicians. This seminar will provide a basic knowledge of addiction issues as well as established and evidence -based models of treatment. Participants will gain knowledge in: diagnosing clients using the DSM -V; the effects of substances on the body; medication -assisted treatments; assessing signs of intoxication; determining and working with a client s stage of change; determining most appropriate level of care using A SA M criteria; and the psychotherapeutic techniques to use in session. Trauma Therapy for Co-occurring Disorders Presented by: Kerry Stout, LISCW, LADC and Leslie Ferrer, LICSW, LADC The purpose of this workshop is to provide clinicians with a de -mystified, strength based and evidenced based, holistic approach to working with clients with a history of trauma and co -occurring disorders. Through this workshop, clinicians will increase their skills, knowledge of interventions, self care and confidence in working with clients who struggle and live with the impact of trauma in their lives, and deepen their understanding of counter -transferential issues and self management skills. At the end of this workshop participants will to be able to: identify, define and diagnose PTSD; describe the impact of traumatic stress; demonstrate a rudimentary understanding of brain functioning as it relates to traumatic stress; understand traumatic stress from biochemical, psychological, social and spiritual perspectives; understand the effect and interrelationship of PTSD and substance use and other self harming behaviors; understand and be able to apply current cognitive, behavioral, body based and mindful interventions to stabilize and alleviate the negative consequences of PTSD; and help their clients develop skills to manage and decrease the impact of trauma. Crimonogenic Thinking Presented by: Bob Wolford, M.S.W., LICSW As service providers, we provide treatment for criminal justice involved people (CJIP). This training will introduce treatme nt professionals to cutting-edge information about the current state of evidence that informs effective criminal justice treatments. This training covers four important and interrelated topics: 1.) What is Evidence-Based Practice? This section de-mystifies what is meant by the term evidence-based practice, and applies the framework to effective CJIP treatments. It will examine necessary adaptations of a model that evolved primarily for healthcare purposes. This section also discusses salient ethical considerations in providing treatment to CJIP, including informed consent and confidentiality. 2.) Overarching principles of effective CJIP treatment. This section develops a working definition of CJIP treatment, including reviewing the well-known risk, needs, and responsivity principles and their application to clinical practices, the philosophy of harm reduction, and the importance of strength-based assessment and interventions. 3.) Common Therapeutic Factors: What generally works to treat people who are CJIP involved? 4.) Specific Evidence-Based Modalities for CJIP. This section will review specific modalities for treating clients affected by addiction and mental health problems, domestic violence perpetrators, and adult female offenders. Joe Lasek, M.D. has been a staff psychiatrist and Associate Medical Director for Mental Health & Substance Abuse Services at HowardCenter since 2005. During that time, he has also been a Clinical Assistant Professor for the UVM College of Medicine where he has taught and supervised medical students and psychiatric residents. Since March 2008, he has served as adjunct faculty at Southern New Hampshire University where he teaches psychopharmacology to Masters level students in the clinical mental health counseling program. Jennifer Spagnuolo, MS-LCMHC, LADC, is a Senior Clinician at Pine Street Counseling Services, the adult outpatient division of HowardCenter. Jennifer has been a therapist in Vermont for nine years. She has worked in crisis services, pr ivate pract ice and community mental health providing various outpatient services to people of all ages. Jennifer recently completed a two-year study on Post-Traumatic Stress Disorder and substance abuse through Dartmouth Psychiatric Research Center where she was a site coordinator as well as a clinician. At HowardCenter, she supervises other clinicians, facilitates an Intensive Outpatient substance abuse group and has an individual caseload for mental health and substance abuse. Kerry Stout LISCW, LADC discovered her passion for working with survivors of trauma during her internship at a hospital based crime victims treatment center during her graduate social work program in New York City, after working for many years with the homeless. She remained on staff as a therapist and coordinator of the sexual assault forensic examiner program in the emergency departments of the hospital, working not only with rape survivors, but survivors of all types of crime from many countries who found themselves struggling with the impact it had in their lives. She has continued to maintain this as her primary clinical focus after her move to Burlington while working as program coordinator at Pine St. Counseling Services, and bringing a trauma informed approach to those struggling with mental health and addiction problems. Leslie Ferrer, LICSW, LADC is currently the Assistant Director at the Chittenden Clinic. Leslie is a clinical social worker and substance abuse counselor who is passionate about getting to know people, their life stories and struggles, and drawing on their strengths along with using knowledge from the best that science has to offer in our field to help people become their highest selves, attain their goals, and move beyond barriers. Leslie has had numerous clinical and leadership positions in the field working at Early Intervention with kids ages birth to 3, school social work, adult crisis services, residential substance abuse treatment, substance abuse treatment in a women s correctional facility, private practice, outpatient services, and medical social work. Bob Wolford, M.S.W., LICSW Since late 2002, Bob has been providing treatment, supervision, consultation and leadership for program development for people at all points of the Sequential Intercept Model. The SIM, model developed by SAMHSA's GAINS Center for Behavioral Health and Justice Transformation, conceptualizes how people move through criminal justice system in predictable ways, and envisions a series of points of interception at which an intervention can be made to prevent individuals from entering or penetrating deeper into the criminal justice system. These programs have included pre-trial mental health and substance abuse screenings, behavioral health treatment courts, facility-based, and reentry services. In this workshop, we will combine the perspective of a psychotherapeutic and medical perspective in the approach to anxiety disorders. We will cover the epidemiology and assessment of Obsessive Compulsive Disorder, Panic Disorder, and Social Phobia. We will review effective psychosocial treatments and pharmacological treatments in the context of co-occurring substance use problems. We will wrap-up with a focus on commonalities in psychosocial treatments for anxiety disorders along with a discussion of the special issues surrounding benzodiazepine use in treatment of anxiety disorders. Assessment, Treatment Formulation & Planning for the Effective Treatment of Co -occurring Disorders Presented by : Catherine Burns, Ph.D. and Leslie Ferrer, LICSW, LADC Clients with co-occurring disorders often have very complex presentations requiring thoughtful treatment formulation. Effective assessment is critical to treatment formulation and subsequent treatment planning. In this workshop, participants will review the common approaches to assessment and how these efforts inform the identification of strengths and needs of clients and inform our diagnostic decisions. Using this information, models of treatment formulation and planning will be reviewed with application to case studies. Finally, exercises to inform how to write specific treatment goals, and how to monitor progress over the course of treatment will be presented. Presented by: Sandra Steingard, M.D. and Daniel Baslock, LISCW, AAP In this training we will review current models for defining and understanding psychosis. We will review the traditional diagnostic classification, examine its limitations, and review alternative formulations. Discuss current data on prognosis and discuss how treatment may help or hinder outcome. We will review current thinking on etiology of psychosis including genetic, biological and environmental risk factors. Discuss the role of pharmacologic and non-pharmacologic treatments including cognitive behavioral therapy, wellness management, open dialogue and hearing voices groups.
