All systematic reviews are not metaanalyses! it is always appropriate and desirable to systematically review a body of data, but it may sometimes be inappropriate, or even misleading, to statistically pool results from separate studies. Indeed, it is our impression that reviewers often find it hard to resist the temptation of combining studies even when such meta-analysis is questionable or clearly inappropriate. Egger et al. When can meta-analyses mislead? When a meta-analysis is done outside of a systematic review When poor quality studies are included or when quality issues are ignored When small and inconclusive studies are included When inadequate attention is given to heterogeneity Indiscriminate data aggregation can lead to inaccurate conclusions When reporting biases are a problem Publication bias Time lag bias Duplicate publication bias Language bias Outcome reporting bias Egger M et al. Challenges in diagnostic meta-analyses Search terms are not well defined Simple pooling may not be meaningful Two effect measures that are correlated Cut-points are a big issue Need more complex analyses Bivariate random effects regression Hierarchical summary ROC models Software options were limited, but better now MetaDiSc Stata (metand command) RevMan No easy way to assess potential for publication bias Publication bias is likely to be a bigger problem with Dx studies Meta-analyses cannot overcome the problem of: Poor quality diagnostic studies and poorly reported studies

There are 50+ systematic reviews on TB tests, but almost all focus on sensitivity and specificity (accuracy) 25
