ALZHEIMER S DISEASE INITIATIVE MEMORY DISORDER CLINICS AND FLORIDA BRAIN BANK 2016-2017 Year End Summary This document was created by the staff of the 15 Memory Disorder Clinics and the Florida Brain Bank on behalf of the State of Florida, Department of Elder Affairs.

INTRODUCTION In 1985, the Florida Legislature launched the Alzheimer s Disease Initiative (ADI), an integrated system of dementia specific services to prepare for the increase in the number of Floridians affected by Alzheimer s disease and related dementias (ADRD). As a direct result of that foresight, today Florida has the infrastructure in place to provide clinical care and support for individuals with ADRD and caregivers, education for families and health providers in training, and research. One of the foundational components of Florida s ADI is a network of 15 statewide Memory Disorder Clinics (MDCs). The MDCs provide comprehensive assessments, diagnostic services, and treatment to individuals who exhibit symptoms of ADRD. The clinics develop training programs and provide education on understanding and providing care to address the challenges of ADRD for family and direct service caregivers, respite care providers, and healthcare professionals in the care of persons with ADRD, and the general public. The MDCs also participate in research toward effective treatments and, ultimately, a cure for Alzheimer s disease. MDCs work closely with the Florida Brain Bank, another component of the ADI, to confirm a clinical diagnosis of ADRD, collaborate in research, and inform patients and families about research opportunities. The intent of the Florida Brain Bank is to ultimately find a cure by collecting and autopsying the brains of deceased patients previously diagnosed with dementia. The state of Florida leads the nation in its longstanding approach to coordinated care for individuals with ADRD. Florida has the second highest number of residents over the age of 65 with Alzheimer s disease in the U.S; approximately 520,000 persons in Florida are living with Alzheimer s disease today, and this figure is estimated to increase to 720,000 by 2025 1. Florida is experiencing a surge in Alzheimer s cases as the population increases and Baby Boomers age; thus, it is our responsibility to be innovative leaders, charting the course for the rest of the nation in our approach to this disease. 2016-2017 State of Florida Memory Disorder Clinics Facts From July 1, 2016 to June 30, 2017, the Memory Disorder Clinics completed the following: The Memory Disorder Clinics in total completed 11,565 comprehensive memory evaluations, saw 5,819 new patients, with 17,685 office visits made by patients and their caregivers. 1,529 free memory screenings were conducted by trained Memory Disorder Clinic staff. 27,598 referrals were made to medical or community services for patients and families who came to the Memory Disorder Clinics. Nearly 11 percent of the patients evaluated for memory loss were age 60 and younger, and 18 percent of these patients were 85 and older. 7,586 family caregivers received educational training from the clinics staff on how to care for a loved one at home who has dementia. Of the new patients evaluated, 11.1 percent are of Hispanic ethnicity and 5.5 percent were Black (or African-American). 5,876 hours of training were provided to 27,503 family caregivers, medical professionals, health students, social service workers, and the general public. An additional 20,325 phone contacts were made to provide information and referrals to community resources that assist individuals affected by dementia. The Memory Clinics followed up with family members upon the cancellation of 254 Silver Alert occurrences to provide education, resources, and referrals to assist the recovered person and in an effort to prevent future reoccurrences. The Department of Elder Affairs provides funding for 15 Memory Disorder Clinics located across Florida to provide statewide coverage. Additionally, the Florida legislature designated Florida Hospital in Orlando as a new Memory Disorder Clinic in the 2017 legislative session. 3

University of South Florida Memory Disorders Clinic 3515 East Fletcher Avenue, MDC 14 Tampa, Florida 33613 Phone: (813) 974-3100 Serving Hillsborough, Hardee, and Manatee Counties Alzheimer s Disease Initiative Memory Disorder Clinics have expert medical teams who provide complete diagnostic evaluations, education for caregivers, health providers and the general public, and participate in research towards effective treatments and an ultimate cure. There are an estimated 53,431 residents with probable Alzheimer s disease in PSA6 (based on the 2015 DOEA county profiles) The USF MDC completed 240 comprehensive memory loss evaluations. The USF MDC evaluated 222 new patients for memory loss. The USF MDC provided training/education to over 858 caregivers and health care workers. The USF MDC provided 281 free community screenings. The USF MDC made over 1,620 referrals to community resources. The USF MDC was a research site for 8 clinical trials and basic scientist conducted 4 federally funded research projects in ADRD. The USF MDC trained one Geriatric Psychiatry Fellows The USF MDC conducted series of Savvy Caregiver Education Group (evidence-based program) for caregivers of people with ADRD. The USF MDC incorporated research findings into presentations, workshops, and trainings. The USF MDC responded to 100% of Silver Alerts within its catchment area. The USF MDC provided counseling to families about safety, driving, resources, and care throughout Hillsborough, Manatee, and Hardee counties. 5

Each one of these individuals touch the lives of multiple friends and family members. 2016-2017 Highlights: University of Florida ADI Memory Disorder Clinic The MDC completed 907 comprehensive clinical evaluations for memory and cognitive impairment. The MDC made approximately 1344 referrals to community programs and family resources. The MDC provided education to approximately 989 individuals, including medical professionals, health students, social service providers, family caregivers, ADI service providers, model day care providers, and members of the general community. The MDC partnered with Al z Place, ElderCare of Alachua County, Elder Options, Alzheimer s Association, and HarborChase of Gainesville to provide a free, public, half-day workshop for caregivers. The MDC trained a behavioral neurology fellow as an academic dementia specialist. The MDC completed 30 publications including chapters, peer-reviewed journal articles, poster presentations at national meetings, and abstracts. The MDC created 6 new educational handouts with information and recommendations for caregivers. The MDC provided training during clinical evaluations and mentored in research 22 physicians, residents, medical students, and doctoral students. The MDC responded to 16 Silver Alerts in the 9-county service area and provided counseling to these families about safety, resources, and dementia care to minimize the likelihood of a reoccurrence. 6

University of Miami Memory Disorder Clinic 1695 N.W. 9 th Avenue, Suite 3204B Miami, Florida 33136 Phone: (305) -355-9065 Serving Miami-Dade and Monroe Counties 2017- University of Miami Memory Disorder Clinic Highlights There are an estimated 55,630 people with probable Alzheimer disease in the University of Miami Memory Disorders Clinic service area, according to the Department of Elder Affairs 2016 Florida County Profiles Completed 570 diagnostic evaluations, and provided 305 free community screenings Provided education & support to 1073 caregivers and families Staff at the UM-MDC provided over 2569 hours of education and training to 2016 individuals, including health care and social service professionals, medical students, and direct care staff Made over 309 referrals to community resources Conducted our Hispanic ADI Educational & Training Seminar, providing education and support to South Florida s Hispanic families dealing with Alzheimer We have enrolled over 1020 participants in various research studies This year we enhanced our educational trainings on Memory and Cognitive Impairment and Family Caregiving. These two topics were presented to 30 community aging adults who are part of the Osher Lifelong Learning Institute (OLLI) at the University of Miami 7

Clinics participate in research studies in order to develop effective treatments and to eventually find a cure. 2016-17 Wien Center for Alzheimer s Disease and Memory Disorders Facts The number of individuals in Miami-Dade County aged 65+ who are estimated to have probable Alzheimer s Disease or a related diagnosis is 53,983; 65,307 individuals are estimated to have a cognitive impairment. Completed 2,382 full diagnostic evaluations and provided 92 free screenings Provided education to 268 caregivers about effective coping mechanisms and behavior management plans. Staff at the Wien Center provided training and education to about 1,454 individuals Made 2,202 referrals to community resources Facilitates support groups in the service area for individuals with Mild Cognitive Impairment, Early Stage Dementia and caregivers or other loved ones Sponsored the 15 th Annual Mild Cognitive Impairment Symposium for 177 physicians and scientists held in January 2017 (http://www.mcisymposium.org/). Participated in over 13 clinical research trials sponsored by pharmaceutical companies and the NIH for the treatment and prevention of Alzheimer s disease Collaborated with the University of Miami, University of Florida, Florida International University and Florida Atlantic University to administer an ongoing Alzheimer s Disease Research Center Grant study awarded on August 15, 2015. Successfully enrolled 257 participants as of June 2017 The Wien Center administers the State of Florida Brain Bank with the collaboration of the Mayo Clinic and the Alzheimer s Disease Resource Center. A total of 54 autopsies were performed, 14 of which were performed on patients enrolled from the Wien Center. 8

Each of these individuals touches the lives of multiple friends and family members. 2016-17 Broward Health North MDC Highlights The Broward Health North Memory Disorder Center (BHNMDC) was established in 1986 and in 1989 it was designated by the State of Florida (430.502) under the Alzheimer s disease Initiative as a State-designated memory disorder center. Staff at BHNMDC provided 436 hours of education in the community to over 2,004 individuals. Our multidisciplinary medical team, including neurologist, psychiatrist and neuropsychologists evaluated 448 patients this year, providing diagnosis, follow-up and recommendations for treatment and care in English and Spanish. Referrals were made to 1837 community resources Provided support to 1,988 caregivers Responded to 19 Silver Alerts in Broward County. First in the Nation to attain Joint Commission Certification for Alzheimer s disease. The Center offers the Care Assistance Program a 16-hour educational program for caregivers held ten times a year. Program speakers discuss the disease process, or behavior strategies, medications, legal issues and resources available in the community. Transportation and care for the Alzheimer s patient are available upon request. 9

Babcock Street, Melbourne, Florida 32901 Sandy Stauffer, Program Manager Phone: (321) 434-7613 2016-2017 Highlights The ECFMDC provided 378 full diagnostic evaluations and testing in partnership with the Health First Aging Services Geriatric medical clinic and the Florida Institute of Technology School of Psychology doctoral students trained to complete neuropsychological exams. The ECFMDC provided 152 free Memory Screens in the community providing information on memory health and resources for next steps if indicated. Utilized bi-lingual Psychometrist in order to provide Memory Screens in Spanish. The ECFMDC made over 3,989 referrals to medical services and community resources. The ECFMDC provided 142 Dementia training programs and educated 4,306 individuals including: >943 family caregivers >2,432 general public >931 healthcare professionals The ECFMDC was one of the first in the nation to offer the evidence based Stress-Busting Program for Family Caregivers of People with Dementia. Which provides family members with education, discussion, and support. Teaching caregivers stress management techniques since many are at greater risk as a caregiver. The ECFMDC offered the MindSet Class a six-week proactive educational series for people with mild cognitive impairment and their family caregivers to learn together. The ECFMDC focused educational programs in the community and at Senior Centers around brain health and wellness with the six-week Total Memory Workout for Seniors series and other brain health initiatives. The ECFMDC responded to 25 Silver Alerts, counseling families about safety, resources, and dementia care to assist them and minimize the likelihood of a reoccurrence. The ECFMDC created a monthly Brain Business Newsletter with helpful tips and other information about our educational programs and resources to family caregivers and professionals. The ECFMDC offered monthly Virtual Dementia Training in the community with partners to increase sensitivity to the needs of people with Alzheimer s Disease and Related Dementia s. 10

Mayo Clinic Memory Disorder Clinic 4500 San Pablo Road Jacksonville, FL 32224 Phone: (904) 953--6523 Serving Baker, Clay, Columbia, Duval, Flagler, Hamilton, Nassau, North Volusia, Putnam, St. Johns and Suwannee counties The Alzheimer s Disease Initiative Memory Disorder Clinics have expert medical teams who provide complete diagnostic evaluations, education for caregivers, health providers and the general public, and participate in research towards effective treatments and an ultimate cure. 2017 Mayo Clinic Memory Disorder Clinic Highlights Multi-disciplinary medical team, including neurologists and neuropsychologists accurately evaluated 236 new patients, providing diagnosis and recommendations for treatment and care. A total of 267 unduplicated persons were seen in the MDC. Staff at Mayo Clinic MDC provided education & support to 846 family caregivers and 613 health care professionals. All patients and families seen were provided information on services for those with Alzheimer s Disease and related disorders available in the State of Florida. All patients and families were provided information on the Silver Alert Program. Provided education and training to over 2,600 individuals including medical professionals, family caregivers and general public. Counseled families on the best practices for providing care across the continuum focusing on safety and quality of life. Made 814 referrals to community resources including ADI and ADRC partners. Continued participation in the Northeast Florida Caregiver Coalition to provide Caring for the Caregiver workshops throughout the service area. Continued an art appreciation program for those with dementia and their primary family caregiver. This is collaboration with The Cummer Museum of Arts and Gardens, Mayo Center for Arts and Humanities and the Mayo MDC. Conducted monthly caregiver support groups, open to the public and with no charge or registration required. Responded to 28 Silver Alerts, counseling families about safety and resources. 11

West Florida Hospital Memory Disorder Clinic 8383 North Davis Highway Pensacola, FL 32514 (850) 494-6497 Serving Escambia, Santa Rosa, Walton, Okaloosa, Holmes, Jackson, Washington, and Bay Counties, with an estimated 19,564 people with probable Alzheimer s disease in the West Florida Hospital MDC service area, according to the DOEA 2016 Florida County Profiles. 2016-17 Highlights: West Florida Hospital Memory Disorder Clinic The MDC evaluated 130 patients for memory loss. The MDC presented education to 1413 individuals about Alzheimer s disease, Silver Alert, Disaster preparedness and caregiver education. The MDC provides the area s only early onset support group. Conducted 64 events/programs to educate about Alzheimer s disease. The MDC provided education, support, and counseling by phone to 2,753 persons. The MDC provided 1016 referrals to community resources. The MDC responded to 13 Silver Alerts, counseling families about safety, resources, and care. 12

Memory Disorder Center 5305 Greenwood Avenue West Palm Beach, Florida 33407 Phone: (321) 841-9700 Serving Palm Beach, Martin, and Okeechobee counties, with an estimated 55,135 people with probable Alzheimer s disease in the St. Mary s MDC service area, according to the DOEA 2015 Florida County Profiles. Each one of these individuals touches the lives of multiple friends and family members. 2016-2017 Highlights: St. Mary s ADI Memory Disorder Clinic The MDC provided education in the community for the professionals, students, caregiver and general public to over 628 individuals. The MDC evaluated over 107 comprehensive memory evaluations and completed 50 free community screenings. The MDC made referrals to 1040 community resources including ADI and ADRC partners. The MDC provided support and education to 233 family caregivers. The MDC responded to multiple Silver Alerts in the areas served, counseling families regarding safety, services and care. The MDC provided MindSet trainings for individuals with Mild Cognitive Impairment and Early Stage Dementia. The MDC provided training for organization planning to provide MindSet trainings. The MDC partnered with students from Florida Atlantic University in their research to develop a self-screening tool for those with early memory loss. Participated in the IDEAS study The MDC incorporated research findings into presentations, workshops, and trainings. The MDC provided 8 outreach events and screenings to low-income and minority areas, exceeding the yearly standard of 4 13

There are an estimated 7,462 people with probable Alzheimer disease in the TMHMDC service area. 2016 Tallahassee Memorial HealthCare Memory Disorder Clinic Highlights MDC completed 1161 total evaluations and 279 Free Memory Screens. MDC made 2,500 referrals to community resources and programs. MDC along with community partners: Dr. Glueckauf, Marghi McClearn (Ageless Grace), and the Alzheimer s Project presented 867.75 hours of education to 4442 participants in our 10 County Service Area and our 14 county Area Agency on Aging (AAA) service area. MDC partnered with Florida State University College of Medicine to assist in recruitment of five research projects addressing caregiver burden and individuals with dementia. 268 Participants were referred to research projects. MDC responded to 2 Silver Alerts, counseling families about safety, resources and care. MDC is working with the DOEA and the DCCI to increase awareness, provide assistance, and continue advocacy throughout the Tallahassee area: This past year we provided trainings at the Kearney Center, TMH, CRMC, Leon Co Sheriff s Office, Firefighters, churches, & senior centers. We also presented at the National Association of Social Workers conference in Orlando and at the Dementia Action Alliance Conference in Atlanta. Collaborated with Florida State University College of Medicine and Alzheimer s Project to hold annual Alzheimer s Conference providing education to over 150 caregivers and professionals. The MDC partnered with Covenant Care and Alzheimer s project to offer Virtual Dementia Tours in the community with partners to increase sensitivity to the needs of people with ADRD. The MDC provided 22 hours of training to 306 ADI providers in our 10-county service area and to our AAA 14 county service area. 14

Support group topics included: Geriatric Polypharmacy, Elder Law, Estate Planning, Coping Strategies, Effective Communication with a patient with Dementia/AD, Caring for Self in Caregiving, Pet Therapy and many more! Responded to 30 Silver Alerts, counseling families about safety, resources and care Met all base contract measures Achieved 16 out of 20 incentive measures for MDC Performance-Based Funding There are an estimated 60,038 people with probable Alzheimer disease in PSA 8-Lee Memorial Health System s service area. Sarasota Memorial Hospital Memory Disorder Clinic Maintain this vital resource for Sarasota, Desoto and Highlands counties Sarasota Memorial s Memory Disorder Clinic (MDC) is part of a Department of Elder Affairs-funded network of Memory Clinics located throughout the state providing critical resources and support to community members with Alzheimer s disease and related dementias. The Sarasota clinic is designated by the state to serve Sarasota, Desoto and Highlands counties, with an estimated 24,146 people with probable Alzheimer s disease in the service area, according to DOEA 2015 Florida County Profiles. 2016-2017 Service Area Highlights: Sarasota Memorial Memory Disorder Clinic The MDC completed 337 full diagnostic evaluations and provided 152 free memory screenings. The MDC provided education to 2,463 individuals and made 1,390 referrals to community resources. The MDC provided three monthly dual support Groups for both patients and care partners to help families with Mild Cognitive Impairment and Early Stage Dementia The MDC partnered with Jewish Family Community Services on a federally funded Alzheimer s Support and Education Network (ASEN) to help those living alone with dementia, those providing care in the community to someone with dementia, or those with developmental disabilities and dementia. The MDC published and distributed the monthly On Your Mind newsletter with helpful tips and information about local educational program. The clinic completed 507 Comprehensive Memory Loss Evaluations. A total of 3764 referrals were made to community resources. A total of 107 training and education programs, and support groups were conducted. The clinic participated in three on-going research projects: The Imaging Dementia- Evidence for Amyloid Scanning (IDEAS Study), DAYBREAK-ALZ: A mild Alzheimer s disease 3-year clinical drug trial, and the RAmP Registry: A registry for Amyloid Positive patients. Facilitated the only Early Stage Alzheimer s disease patient support group for 8 years in Pinellas County. Facilitated two Caregiver Support Groups on a biweekly basis. A 9-year partner with UCLA Center on Aging in Memory Fitness Training. A 9-year partner with DriveABLE: A cognitive driving assessment. Responded to 13 Silver Alerts in our service area. Information was given on Memory Disorder Clinic assessments, community resources, and caregiver support groups. 18
