ABOUT THE MOVEMBER FOUNDATION The Movember Foundation is the leading global organisation committed to changing the face of men s health. The Movember community has raised over 580 million, funding over 900 programmes and projects in 21 countries. This work is saving and improving the lives of men affected by prostate cancer, testicular cancer and poor mental health. The Movember Foundation challenges men to grow moustaches during Movember (formerly known as November), to spark conversation and raise vital funds for its men s health programs. To date, 5 million moustaches have been grown worldwide, but we won't stop growing as long as serious men s health issues exist. JUNE 2015 PAGE 3 OF 13

DESCRIPTION With this funding announcement, the Movember Foundation invites applications for high quality, internationally recognised teams of investigators to complete a high impact prostate cancer research project. The project may be a biomedical, translational, clinical, or health services project that leads to one of more of the following: avoidance of unnecessary treatment interventions that reduce or cure side effects of the disease or its treatment non or minimally invasive tests to monitor prostate cancer and its progress accelerate discoveries that lead to interception of lethal disease tests, treatments and interventions that cure or slow progression of lethal disease the development and evaluation of clinical interventions a reduction in variation of clinical quality through the clinical use of patient reported outcome measures a better understanding of morbidity and mortality associated with prostate cancer diagnosis through epidemiological studies that describe treatment paradigms, complications and results of prostate cancer management at a national level The proposed research must have significant impact on the lives of men diagnosed with prostate cancer by addressing one or more of the above Movember Foundation s strategic priorities for the disease. Funds available in this competition are to support collaborative, multi-disciplinary, multi-institutional teams that address important research questions in prostate cancer in Denmark. The Movember Foundation will provide up to 200,000 in total for up to 3 years. The full 200,000 should be expended over a minimum of two years, but not more than three years. The total number of projects that will be supported will be one (1). The project emphasis is on the production of new knowledge, and the translation and transfer of research findings to clinical practice to optimise impact. Proposals that represent active, meaningful partnerships between research teams based in clinical institutions in Denmark are required. JUNE 2015 PAGE 4 OF 13

ELIGIBILITY The project team must include: A single Principal Investigator who is an independent investigator; Co-applicants (Co-Principal Investigators and Co-Investigators) who are also independent investigators; The Principal Investigator, and Co-applicants must be formally affiliated with eligible institutions. The Principal Investigator will: be responsible for the direction of the proposed activities; assume the administrative and financial oversight for the entire project; assume the financial responsibility for their own portion of the project at their own institution; and, receive all related correspondence from the Movember Foundation. An independent investigator is an individual who: has an academic or research appointment which: o must commence by the effective date of funding; and o allows the individual to pursue the proposed research project, to engage in independent research activities for the entire duration of the funding, to supervise trainees, and to publish the research results; and o obliges the individual to conform to institutional regulations concerning the conduct of research, the supervision of trainees, and the employment conditions of staff paid with Movember Foundation funding. will assume financial responsibility for their portion of the award at their own institution and will be responsible for reporting this information back to the Principal Investigator as well as to the Movember Foundation. Eligible institutions must meet the following requirements: The institution type must fall under one of the following categories: o Danish post-secondary institutions and their affiliated institutions including hospitals and research institutes. o Danish non-governmental, not-for-profit organisations (including community or charitable organisations) with an explicit research or knowledge translation mandate. The institutions must be recognised legal entities. The institutions must have the necessary accounting systems and financial controls in place to manage Movember Foundation funds. The institutions must ensure that the researchers will have the time and infrastructure that will permit the pursuit of the planned research and the freedom to publish the results. In addition, the following special conditions apply: The members of the team may pursue other research in addition to their commitment to this project, but each individual researcher must contribute sufficient time to the team award to ensure the achievement of the project s research aims. If a project seeks to address clinical quality variation, the clinical and patient reported outcome measures should aim to be consistent with the minimum data standards for localised/advanced prostate cancer as defined by ICHOM (www.ichom.org) Collaborators, from outside Denmark, who make a substantial intellectual contribution to the project award, may be listed as team members. Specific and justified requests for funding to be used outside of Denmark will be considered; however, it is anticipated that the great majority of the funds will remain in Denmark. Potential team leaders are encouraged to put their specific questions, in writing and email it to the Movember Foundation contact listed at the end of this document. All questions will be addressed in a timely manner. ALLOWABLE COSTS The following expenditures will be considered eligible for funding received through this funding opportunity: Research operating costs for the proposed collaborative research project, which must be distinct in its objectives from those for which groups members currently receive funding. Costs of data collection, database management, database integration and maintenance. Costs of regional, national or international networking activities, including collaboration, planning, and knowledge exchange activities. Salaries of research assistants, coordinators, technicians, administrative staff, and other personnel who will enhance the collaborative research productivity of the team. Costs involved in linkage with and dissemination and translation of research findings to those who benefit from the new knowledge generated by the team. INELIGIBLE COSTS Institutional overhead costs Tuition and professional membership dues Capital Costs, including costs associated with construction, renovation or rental of offices, laboratories or other supporting facilities. COMMUNICATION REQUIREMENTS Funding recipients are required to acknowledge the Movember Foundation in all communications or publications related to this funding opportunity. The award name must be used in all communications and promotions related to the Movember Award. Recognition guidelines including instructions on the Movember Foundation logo use will be provided to successful applicants as part of the approval package. JUNE 2015 PAGE 6 OF 13

PERFORMANCE MEASUREMENT The Movember Foundation is committed to, collecting, evaluating and disseminating information on the outputs and impacts of the research it funds. This information is an important part of the Foundation s accountability and transparency with its donor community. The team leaders and nominated independent investigator(s) will be required to submit an end of project report 6 months following the end period of the project. The format of the report will be made available to the team leader(s) at the beginning of the project-funding period and can be filled in as the research progresses. In addition, the team leader(s) must contribute to the monitoring, review and evaluation of this funding opportunity by participating in requested evaluation studies, surveys, audits, and workshops as required by the funder for the purposes of collecting information to assess progress and results. The team leader(s) will be required to submit Movember Foundation annual written progress reports, as well as a list of all publications resulting in whole or in part from this funding opportunity. KNOWLEDGE TRANSLATION STRATEGY Knowledge Translation can be defined as the methods for closing the gaps from knowledge to practice. http://www.cmaj.ca/content/181/3-4/165.full The Knowledge Translation Plan should be developed with this definition in mind and must go well beyond traditional dissemination endeavours (i.e. publications, conferences). The team is required to develop a comprehensive knowledge translation plan and demonstrate that a meaningful collaborative research approach is being used. The knowledge translation plan must include evidence that prostate cancer clinical experts, young investigators and end-users are active participants and are engaged in the entire research process. For example, these individuals should shape the research process by collaborating to determine the trials research questions, deciding on the research methodology, interpret the findings, and assist with the dissemination of research results. In addition, the applicants are required to articulate their knowledge sharing/dissemination plan to demonstrate that the results achieved are relevant and useful to end-users. REVIEW PROCESS AND EVALUATION CRITERIA An international peer review panel (external to Denmark) will be convened to evaluate the applications. The panel will be created specifically for this funding opportunity. Panel members will be selected based on their breadth of knowledge and expertise in prostate cancer. Applications will be rated and ranked based on their excellence as judged against the following four evaluation criteria. Because different projects will emphasize different research approaches and knowledge translation strategies, it is understood that reviewers may weight criteria differently from one application to another. Each individual component will be evaluated, as well as the project as a whole. 1) Research Approach 2) Significance/Impact of the Proposal 3) Researchers/Research Team 4) The Research Environment To support the strategic objectives of this funding opportunity, the following factors will be considered: 1) Research Approach a. Coherence of the Research project. b. Evidence of synergy between Investigators. c. The soundness and rigor of the science. d. The extent to which the proposed project brings together researchers from different institutions and disciplines in a meaningful and collaborative way. e. The extent to which the health question, problem, or issue is being approached by the project team through an original, integrated, and feasible research plan, which will generate valid, reliable, and useful knowledge. f. The extent to which the team has a knowledge translation plan for linking with and disseminating research findings to end-user groups who will utilise and apply the results. 2) Significance/Impact of the Proposal a. The extent to which the proposal is advancing knowledge that will lead to significant improvement(s) in the lives of men diagnosed with prostate cancer. b. The translational aspects of the project are clear and the potential to improve outcomes for prostate cancer patients is evident. c. The research question is important and the results from the project will have broad interest and applicability. d. The originality of the hypotheses or questions being addressed by the project are clear and well formulated. e. The extent to which the project challenges existing paradigms and engages in novel concepts, approaches, methodologies or tools. JUNE 2015 PAGE 8 OF 13

3) Researchers/Research Team a. The extent and appropriateness of the collaborative, team-based approach proposed b. The appropriateness and inter-disciplinarity of the team, including the team's leadership and the integration of team members. c. The team leader(s) and supporting team members are appropriately trained and well suited to carry out the work described in the proposal. d. The expertise and experience of the project team leader(s) is complementary and synergistic. e. Senior and junior investigators are engaged as active team members. 4) The Research Environment a. The degree of institutional support for the project team. b. A quality research environment is evident and it will contribute to the probability of success for the project. Applications that do not comply with these instructions may not be accepted for review. By submitting an application, the applicants accept the Terms and Conditions set out below. Applicants are required to submit project information and supporting documentation as follows: a) A project proposal up to a maximum of twenty-five (25) pages (excluding title page (if applicable) table of contents, letters of support, publications, CVs of team members, and letters of collaboration.) The proposal must clearly outline each of the elements below: Project title Lay summary of the Project (up to 250 words) Project rationale, originality and innovation of the project The areas of research the proposed research initiative falls and how the research proposal is relevant to that area The research approach The significance and impact of the proposed project The Team - detailing their background and expertise (2 page limit) A comprehensive knowledge translation strategy An evaluation plan The requested budget, the justification for the budget request and supporting documents Documentation of existing resources and a description of how the new project funds will complement existing project infrastructure/funding are required. In addition to the project proposal, the following information and documents will need to be included. Publications a maximum of fifteen (15) publications relevant to the proposal may be submitted Letters of Support (mandatory - minimum 3) signed letters of support are required from all organizations supporting the proposed project. If applicable, their specific contribution must be clearly indicated in the letter. Letters of Collaboration (mandatory) signed letters of collaboration are required from all collaborators on the project. The necessary required signatures (mandatory) identification of which organization will receive and administer the award, is required. Short form CVs (mandatory) for any applicable team members. To be eligible for review, the full application must adhere to the following instructions for presentation and content. Microsoft Word or PDF format preferred. One (1) page for the table of contents. A4 size (210 X 297mm) Arial font (regular), minimum 11-point; Single-spaced text; 1 " (2.54 cm) margin on all sides of each page; and A header on each page with the Leader's name in top left-hand corner, and the page number in the top right-hand corner. The Movember Foundation may, at its sole discretion, consider for acceptance a response that does not comply with the requirements of this request for applications. 8) The Applicants grants the Movember Foundation, a non-exclusive license to use for the purpose of this application process, any information, processes, sketches, calculations, drawings, or other data or information submitted with or included in, the response submitted by the Applicant. 9) Each Applicant agrees to indemnify the Movember Foundation against third party claims arising out of any use of any proprietary information submitting with or included in, the full application. 10) Should the Applicant find any material discrepancy, error or omission in this call for applications, the applicant must immediately notify the Movember Foundation in writing of the nature of the discrepancy, error or omission. 11) The Applicant and team members of the project acknowledge that their details, including any personal details may be disclosed to third parties including peer reviewers, for the purposes of this application process and any related purposes. VARIATIONS The Movember Foundation may vary the requirements set out in this call for applications and seek further information from the Applicants. THE MOVEMBER FOUNDATION S RIGHTS The Movember Foundation reserves the right to subject the Applicant to a "due diligence" enquiry, which may comprise of: A) Verifying whether the represented resources and skills are actually available; and B) Assessing experience and integrity. The Movember Foundation, at its sole discretion, reserves the right to depart from any method of evaluation set out in this call for proposals. RELIANCE ON INFORMATION The Movember Foundation will rely on information provided by, or on behalf of the Applicants at all stages of the application process. In providing information, Applicants represent to the Movember Foundation that the information is complete and accurate in all material respects, that it is not misleading and that in preparing the information, reasonable skill and care has been exercised by the Applicant and its personnel and acknowledges that the Movember Foundation may rely on that information.
