However, conventional ERP requires considerable time and is relatively expensive, making it difficult to administer this type of treatment to all OCD patients in general outpatient programs. Therefore, this study endorses self-erp in which general outpatient programs deliver psychological education to OCD patients so that they can work on ERP by themselves. Methods: The medical records of all OCD patients who came to the first author's outpatient clinic from 2004 to 2009 were retrospectively examined. This examination investigated the patients' clinical characteristics, the content of their treatment, and the changes in their symptoms. Assessment of their degree of improvement used the Clinical Global Impression Improvement Scale and the Global Assessment of Functioning. Results: Forty percent of the patients were able to handle working on self-erp, and half of them showed adequate improvement. In particular, the self-erp rate for patients who had not received any prior OCD treatments was quite high at 57.5%, and 78.2% of them showed adequate improvement. Conclusion: The results suggest that teaching self-erp to OCD patients is advantageous and particularly good with patients undergoing treatment for the first time. Key words: obsessive-compulsive disorder, exposure and response prevention, cognitive-behavioral therapy, psychoeducation, self-treatment Introduction bsessive-compulsive disorder (OCD) is a mental O disorder characterized by obsessions and compulsive behaviors (compulsions). Obsessions are exaggerated doubts and thoughts that persistently intrude a person's consciousness against their will, causing mental or emotional pain. Compulsions are exaggerated ritualistic behaviors that a person performs to escape from this pain. Many patients realize that their own obsessions and compulsions are exaggerated and irrational. They desire to stop worrying; however, their obsessions continue to arise in their mind. Thus, they are unable to avoid engaging in their compulsive behaviors. Some patients involve family members by asking for their cooperation (involvement tendencies). Although the compulsions temporarily release the patient from pain, they activate a neural circuit that becomes the foundation for OCD, thereby reinforcing the obsessions and inducing the patient toward still more severe compulsions that require even greater efforts to resist. In this manner, a vicious cycle of mutually reinforcing obsessions and compulsions develops. This cycle is the mechanism that maintains and intensifies OCD. Psychoeducation for self-erp for OCD patients appear while another mental disorder is progressing or vice versa. There are also cases wherein a person begins by carefully examining things after having made a large mistake in everyday life, and this habit gradually escalates into an abnormal condition that hinders the person's daily living activities. If the individual exacerbating factors existed independently, they could be easily solvable problems, ordinary aspects of life that do not require a solution, or even desirable. However, once they become included in the vicious OCD cycle, these factors mutually interact to produce negative impacts and become difficult problems. Therefore, an essential part of treatment for OCD involves stopping the vicious OCD cycle. The treatment methods that have proven effective for OCD are pharmacotherapy, which primarily comprises serotonin reuptake inhibitors (SRIs), and cognitivebehavioral therapy (CBT), which primarily comprises exposure and response prevention (ERP). ERP, as a type of CBT technique, involves combining methods from ERP therapies. Exposure therapy means exposing a patient to the stimulus that causes obsessions, so that, as a result, the patient continues to feel the resulting pain; and then, response prevention therapy involves teaching patients to control themselves and refrain from performing the behaviors to avoid or reduce the pain. The anti-compulsive effects of ERP and SRI are apparent as both treatments reduce the abnormalities found in brain function imaging. 4,7 In instances when both SRI and ERP are utilized, they usually have complementary and synergistic effects, and the risk of a recurrence of the disorder after the completion of an SRI regimen is reduced. 15-18 Therefore, SRI and ERP are recommended in conjunction. If significant risks are associated with the use of pharmaceuticals (e.g., if the patient is a child or a pregnant woman), ERP therapy is the first choice. The conventional ERP (conv-erp) used in numerous previous studies involves a therapist working with a patient on ERP. 18-20 However, this process requires considerable time; therefore, each therapist can only take on a limited number of patients. For this reason, as well as cost considerations, many OCD patients do not have the opportunity to undertake ERP therapy. Since completing training in ERP for OCD patients in 1998, the first author has combined pharmacotherapy, usually comprising SRIs, with ERP or has used ERP by itself. However, the author no longer has sufficient time to deliver ERP to all patients requesting it because of an increase in the number of patients. As an alternative, the author gives psychological education (Table1) to introduce ERP to all applicants. Within this psychological education, instructions are given on ways to conduct ERP by oneself (self-erp). Then, if a patient proves unable to successfully undertake self-erp, conv-erp is proposed as the next step. This two-step approach to treatment is referred to as "ERP in steps." In the past, only a small number of research reports have addressed psychological education and ERP in steps, 21-23 and, as far as the author has been able to determine, there are no studies with a sufficient sample size in which the clinical practice involved a single clinician treating OCD patients as the attending physician. This study examines the effect of incorporating psychological education and ERP in steps in the OCD treatment. Table 1. My obsessions and compulsions are related to a biochemical imbalance in my brain." 3) Refocus You can learn to ignore or to work around them by refocusing your attention on another behavior and doing something useful and positive. 4) Revalue You will come to see intrusive OCD symptoms as the useless garbage they really are. 127

Subjects and Methods Subjects The subjects for this study were all OCD patients treated by the first author from April 2004 to March 2009 after they were first examined at the Kitasato University East Hospital's Psychiatric Outpatient Program. I provided treatment to the patients by combining the previously described ERP in steps with drug treatment involving SRIs. In cases where the patient was a minor or pregnant, or in which drug treatment was not used for other reasons, the treatment employed only ERP in steps. The first step of the ERP in steps was psychological education in which guidance on self-erp was provided. There are a number of different self-erp educational materials, but I felt that the materials by Schwartz, 21 which explain brain function and other issues in a thorough and easily understood manner, were the most appropriate for psychological education. Therefore, I implemented these materials using the outline presented in Table 1, which I created on the basis of the materials, session by session in ordinary one-on-one treatment. In cases when patients could not perform self-erp even after undergoing psychological education, I considered the reasons it was not possible for them to do so (e.g., coexistence of depression) and implemented the necessary measures (e.g., adjusting the depression medication or cognitive intervention). If it was still not possible for the patient to perform the self- ERP, I suggested employing traditional ERP, and in the event that consent was obtained, I did so. In traditional ERP, the clinician makes an ERP task list with the patient, chooses tasks with an appropriate difficulty level with the patient, and performs the ERP tasks with the patient. In the event that an SRI was administered for 12 weeks in accordance with the standard OCD drug treatment algorithm, with the dosage gradually increased from a small amount to a sufficient dosage, but there was no effect, the SRI was exchanged for another. In the event that partial effects were obtained from the SRI, a small quantity of antipsychotic medication or mood-stabilizing medication was added. Retrospective investigation of patient's epidemiological data and treatment selection Medical records created by the first author were used to retrospectively examine the following information: gender, age at initial medical exam, age of onset of the disorder, type of compulsion, awareness of irrationality, presence of involvement tendencies, chief complaint, negative family factors, any concurrent diseases, last medical examination, and number of medical 128 examinations during the study period, content of treatment (any SRIs being taken, self-erp, or conv-erp), and condition at final medical examination during the study period (completed or stopped treatment, hospitalized, continuing with outpatient services, or changed physicians). Treatment outcome We determined that more than 12 weeks were required to achieve therapeutic efficacy, the changes in psychological, social, and occupational functioning of patients who came to the hospital for more than 12 weeks were assessed using the Global Assessment of Functioning (GAF). The GAF assesses a person's comprehensive functioning in addition to improvement in a person's symptoms by quantifying global functioning on a scale of 0 to 100, where higher scores indicate better functioning. Patients whose average scores increased by 10 or more points were categorized as having "improved functioning." Furthermore, the extent of improvement in the symptoms was assessed using the Clinical Global Impression Improvement Scale (CGI-I). This scale has the following seven levels: "clearly worse," "moderately worse," "slightly worse," "no change," "slightly better," "moderately better," and "clearly better." Patients under the last two categories ("moderately better" and "clearly better") were considered to have experienced "adequate improvement" of symptoms. Data analyses A comparison was performed on the epidemiological data and clinical characteristics of all subjects and patients who continued treatment for 12 weeks or more (the outcome-surveyed group). Also, patients who continued treatment for 12 weeks or more were separated into 3 groups, patients who carried out self-erp, patients who did not carry out self-erp but did carry out traditional ERP, and patients who did not carry out either, and a comparison of the epidemiological data and clinical characteristics was performed for each group. Next, the outcome-surveyed group was separated into 2 groups, patients who underwent treatment for the first time (the first-time treatment group) and patients who were visiting other hospitals for OCD treatment but transferred to this hospital (the hospital-transfer group), and a comparison of the nature of the treatment and the outcome was performed. Also, a comparison of clinical characteristics and treatment processes was performed on patients who obtained sufficient improvement in symptoms and patients who did not obtain sufficient improvement. Finally, regarding "lack of awareness of irrationality"

Psychoeducation for self-erp for OCD patients and "involvement tendencies," clinical characteristics that are known to make treatment difficult, in both the firsttime treatment group and the hospital-transfer group, a comparison of treatment outcomes was performed on patients in three groups: patients with one of the characteristics, patients with both characteristics, and patients with neither characteristic. Because this study was not carried out in accordance with a research design for verifying treatment effects, a verification of effects using statistical analysis was not performed. Also, this is a retrospective investigation of medical records, and informed consent from patients was not obtained, but the information collected was obtained in the course of treatment and did not include the patients' names or information that could be used to identify them; therefore, the Kitasato University Ethics Committee determined that deliberations were unnecessary. Results Target group characteristics The target group was comprised of 214 people (93 males and 121 females). Most of the males (56, 60.2%) experienced the onset of the disorder while they were still minors, whereas most of the females (79, 65.3%) had the onset of the disorder when they were adults. Washing and checking were among the most widely acknowledged compulsive symptoms, and more than 70% of the patients were aware of their irrationality. Approximately 50% of the patients had involvement tendencies, and the mental disorder was a negative factor for the families in approximately 30% of the cases. Nearly 50% of the patients were considered to have other concurrent mental disorders. These statistics coincided with the general characteristics of OCD patients as frequently reported in previous studies (Table 2). Moreover, 160 (74.8%) of the 214 patients had a history of being treated for OCD. Of these 160, 101 (63.1%) had changed physicians based on the patient's desire to become even healthier, be examined by a specialist, and/ or undertake ERP. Fifty-five patients came to the hospital for fewer than 12 weeks. Thirty-four (63.6%) of these patients transferred to other physicians because they were introduced to a doctor who was more conveniently located, 6 patients were hospitalized, and 15 decided to stop the treatment. The numbers of the patients' outpatient clinic visits prior to quitting the treatment was: 1 visit (6 patients), 2 visits (4 patients), 3 visits (2 patients), 5 visits (1 patient), and 6 visits (2 patients). All of these patients dropped out at an early stage of the treatment. Removing these 55 patients from the sample left 159 patients (68 males, 91 females) who continued coming to the hospital for 12 weeks or longer, and their basic attributes did not indicate any deviation when compared with the entire target group of 214 patients (Table 2); e.g., the percentage of patients with a prior history of Table 2. Psychoeducation for self-erp for OCD patients OCD treatment remained unchanged. Performance of ERP and clinical characteristics Among the 159 patients in the treatment group, 64 patients were able to perform self-erp, and the ratio of patients who were able to perform self-erp was higher among men than that among women (37 of 68, 54.4% and 27 of 91, 29.7%, respectively). Also, developmental disorders, which are relatively common among male OCD patients, were prevalent in the self-erp group. Of the remaining 95 patients, 77 performed traditional ERP, and the ratio of patients who were able to perform self-erp was high among both men and women (27 of 31, 87.1% and 50 of 64, 78.1%, respectively). Anxiety disorders and mood disorders, which are relatively prevalent among female OCD patients, were slightly more prevalent in the traditional ERP group. There were 18 patients who were unable to perform either self-erp or traditional ERP, most of whom were hospital-transfer patients (17, 94.4%), and among whom the age of onset of symptoms was somewhat lower. For these patients, it was somewhat more likely that the disorder was a negative factor for the family, as well. And there was a high proportion of these patients who lacked awareness of irrationality. These patients' GAF scores were also lower. There were 87 patients with involvement tendencies; and among those, as few as 22 (34.4%) were able to perform self-erp; whereas among the remaining 65 patients, 51 (78.5%) were able to carry out traditional ERP. The treatment outcomes for the 159 patients in the treatment group The treatment outcomes of all 159 patients who came to the hospital for 12 weeks or longer are presented in Table 3. Shishikura, et al. judged as having improved functioning (i.e., a gain of at least 10 points) on the GAF (Table 3). An improvement of at least 10 points in the GAF score was observed for 61 (85.9%) of the 71 patients whose symptoms improved and for 27 (30.7%) of the 88 patients whose symptoms did not improve. In a very few cases (4, 2.5%), "slight worsening," "moderate worsening," or "clearly worsening" was observed on the CGI-I, and the drop in GAF score for all 4 patients was less than 10 points. Thirty-three patients who continued in the study ended their treatment during the study period, and 14 of them did so because they were satisfied with the improvement in their symptoms. The remaining 19 patients decided to stop the treatment even though they had not been observed as having adequate improvement. Eight patients were hospitalized, and 17 patients are continuing to visit the hospital for treatment as of this writing. The remaining 101 patients transferred to other physicians: some returned to the original medical institution that had referred them to this program, and others were introduced to an institution where their follow-up care would be more convenient (Table 3). History of OCD treatment and outcome A comparison was performed on the treatment outcomes of 119 patients who transferred hospitals after undergoing treatment for OCD at other hospitals and failing to obtain sufficient improvement (hospital-transfer treatment group) and 40 patients who underwent treatment for the first time (first-time treatment patients) (Figure1). The ideal treatment of jointly using ERP and pharmacotherapy was introduced to 97 patients (81.5%) of the transfer group and 34 patients (85%) of the firsttime treatment group. This latter group had a particularly large percentage of patients whose symptoms showed adequate improvement (24 of 34, 70.6%). In the hospitaltransfer group, 41 (42.3%) of the 97 patients who received the combined therapy indicated adequate improvement in their symptoms or a considerably lower percentage than the first-time treatment group. The number of patients who were able to handle working on self-erp (regardless of whether or not they were also utilizing pharmacotherapy) included 41 people (34.5%) from the hospital-transfer group; of these, 15 (36.6%) saw adequate improvement in their symptoms (Figure 2). Psychoeducation for self-erp for OCD patients (57.5%) and 18 of these 23 (78.3%) had adequate improvement. In the hospital-transfer group, 61 (78.2%) of the 78 patients who were unable to undertake self- ERP received conv-erp. Twenty-nine (47.5%) of these 61 patients were considered to have improved adequately, representing a higher percentage than did those with self- ERP. In the first-time treatment group, 16 (94.1%) of the 17 patients who were unable to undertake self-erp received conv-erp, and 9 (56.3%) of these 16 patients were considered to have improved adequately. Comparison of patients whose symptoms improved and those whose symptoms did not improve There were 71 cases of improvement (the improvement group) and 88 cases of non-improvement (the nonimprovement group) according to the CGI-I. The clinical characteristics and treatment progress of the two groups were compared (Table 4). There were conspicuous differences between these two groups. The nonimprovement group had a larger percentage of patients with a previous history of being treated, patients with poor awareness of their irrationality, patients with involvement tendencies, negative family factors, and instances of concurrent mood and anxiety disorders. Treatment outcomes for patients with multiple clinical characteristics whose impact upon treatment outcome is unclear Treatment outcomes and the results of an investigation of the extent to which a lack of awareness of irrationality and involvement tendencies coexisted in the same patient are presented in Figure 2. Most patients who lacked awareness of irrationality had involvement tendencies. Among patients with involvement tendencies, a large number of patients obtained positive results by performing traditional ERP rather than self-erp, including both hospital transfer patients and first-time treatment patients. Discussion Regarding the clinical significance of psychological education on self-erp 1. Impact upon treatment Most prior studies report that approximately 25% of OCD patients were unable to perform ERP, 24 but in the present study only 18 of 159 (11.3%) outcomesurveyed patients were unable to perform either self- ERP or traditional ERP. Therefore, offering psychological education on self-erp seems to have not only given patients the opportunity to carry out ERP themselves but to have helped them continue the 133 ERP. Also, 131 (>80%) of outcome-surveyed patients were able to continue the ideal OCD treatment of ERP combined with SRI, and psychological education on self-erp deepened understanding of treatment overall, not only of ERP but also of drug treatment, and seems to have assisted with involvement in treatment. 2. Impact upon improvement of symptoms Among the treatment group in the present study, 71 patients (44.7%) obtained sufficient improvement in symptoms according to the CGI-I. A comparison with prior studies that used the CGI-I to investigate the effects of ERP 25 shows that the treatment was not particularly effective. The main reason for this seems to be that the patients of the present study included many patients who transferred from other hospitals. When hospital-transfer patients who were deemed to have "minimally improved" under the CGI-I are included as well, based on the fact that about half of the patients experienced alleviation of symptoms, and that it is possible that some patients transferred to this hospital having already experienced some improvement in their symptoms relative to the time of the diagnosis at the previous hospital due to treatment at the previous hospital, it may be appropriate to say that some patients had "moderate improvement" under the CGI-I when their symptoms at the time of the diagnosis at the previous hospital and at the time of the final diagnosis at this hospital are compared. It seems very likely that psychological education on self-erp had some degree of significance for hospital-transfer patients. Also, a high proportion of OCD patients undergoing treatment for the first time were able to perform self-erp and to obtain sufficient effects. Psychological education on self-erp seems to have been highly significant for patients undergoing treatment for the first time, suggesting that it is necessary to disseminate psychological education on self-erp to primary medical institutions. 3. Significance in promoting mental health In cases of usage of both ERP and SRIs, usage of ERP alone, and usage of SRIs alone, some patients experienced an increase in GAF scores despite no improvement in symptoms. In other words, some patients experienced an improvement in overall functionality in daily life and social life regardless of the fact that they performed compulsive behaviors due to anxiety and obsessions. This seems to suggest that psychological education may be useful in promoting mental health, and it seems possible that it could also be applied beneficially to psychological welfare and educational settings.

Shishikura, et al. Future challenges Among patients who did not experience sufficient improvement, there was a tendency to have insufficient awareness of irrationality and involvement tendencies. It has recently been reported that interventions combining ERP with cognitive treatment are more effective in treating OCD than ERP alone. 25 Therefore, when providing psychological education to patients with an insufficient awareness of irrationality, it is preferable to develop methods and content based on cognitive treatment. Also, because many patients with involvement tendencies obtained positive results from traditional ERP although few patients were able to perform self-erp, it is preferable to prioritize traditional ERP.
