HEDIS 2014 MQIC MEASURES SUMMARY LISTING FOR ANNUAL PERFORMANCE REPORTING DIABETES 1. Comprehensive Diabetes Care (CDC): Percentage of members 18-75 years of age with diabetes (type 1 and type 2) who had each of the following: a. HbA1c Testing b. HbA1c Poor Control (>9.0%) c. HbA1c Control (<8.0%) d. HbA1c Control (<7.0%) for a selected population (Additional exclusion criteria are required for this indicator) e. Eye Exam (retinal) performed f. LDL-C Screening g. LDL-C Control (<100 mg/dl) h. Medical Attention for Nephropathy i. Blood Pressure Control (<140/80 mm Hg) j. Blood Pressure Control (<140/90 mm Hg) Non-HEDIS Measures: 1. Statin Therapy for Persons with Diabetes Members ages 40-75 with at least one statin prescription filled during the measurement year. ASTHMA 1. Use of Appropriate Medications for People with Asthma (ASM): Percent members 5-64 years of age who were identified as having persistent asthma and who were appropriately prescribed medication during the measurement year. a. 5-11 years b. 12-18 years c. 19-50 years d. 51-64 years e. Total (sum of the four numerators divided by sum of the four denominators) Non-HEDIS Measures: 1. Periodic Assessment (5-64 Years Old) Members who had at least one preventive/ambulatory health services visit within the measurement year with a PCP, pulmonologist, or allergist. 2. Emergency Department Visits per 1000 Members with Asthma and Being Treated for a Primary Diagnosis of Asthma (5-64 Years Old) 1

TOBACCO CONTROL 1. Medical Assistance With Smoking and Tobacco Use Cessation (CAHPS ) (MSC): The following three components assess different facets of providing medical assistance with smoking and tobacco use cessation: a. Advising Smokers and Tobacco Users to Quit b. Discussing Cessation Medications c. Discussing Cessation Strategies 1. Percent Current Smokers 18 years and older DEPRESSION 1. Antidepressant Medication Management (AMM): Percent members 18 years of age and older with a diagnosis of major depression and were treated with antidepressant medication, and who remained on an antidepressant medication treatment. Two rates are reported: a. Effective Acute Phase Treatment. Remained on an antidepressant medication for at least 84 days (12 weeks). b. Effective Continuation Phase Treatment. Remained on an antidepressant medication for at least 180 days (6 months). SUBSTANCE USE DISORDERS 1. Initiation and Engagement of Alcohol and Other Drug Dependence Treatment (IET): Percent of adolescent and adult members with a new episode of alcohol or other drug (AOD) dependence who received the following: a. Initiation of AOD Treatment b. Engagement of AOD Treatment HYPERCHOLESTEROLEMIA 1. Cholesterol Management for Patients with Cardiovascular Conditions (CMC): Percent members 18-75 years of age who were discharged alive for AMI, CABG or PCI from in the year prior to the measurement year, or who had a diagnosis of IVD during the measurement year and the year prior to the measurement year, who had each of the following during the measurement year: a. LDL-C Screening b. LDL-C Control (<100 mg/dl) 1. Statin Therapy for Persons with Cardiovascular Conditions Members with at least one statin prescription filled during the measurement year. 2

HYPERTENSION 1. Controlling High Blood Pressure (CBP): Percent members 18-85 years of age who had a diagnosis of HTN and whose BP was adequately controlled (<140/90 mm Hg) during the measurement year. 1. Percent Members with any Diagnosis of Hypertension ACUTE PHARYNGITIS 1. Appropriate Testing for Children With Pharyngitis (CWP): Percent children 2-18 years of age diagnosed with pharyngitis, dispensed an antibiotic and received a group A streptococcus test for the episode. ROUTINE PRENATAL AND POSTNATAL CARE 1. Prenatal and Postpartum Care (PPC): The percentage of deliveries of live births between November 6 of the year prior to the measurement year and November 5 of the measurement year. For these women, the measure assesses the following facets of prenatal and postpartum care: a. Timeliness of Prenatal Care: percent deliveries that received a prenatal care visit as a member of the organization in the first trimester or within 42 days of enrollment in the organization. b. Postpartum Care: percent deliveries that had a postpartum visit on or between 21 and 56 days after delivery. CHRONIC KIDNEY DISEASE 1. Percent members in HEDIS Diabetes or Hypertension populations who had a serum creatinine test (necessary for GFR) MANAGEMENT OF ACUTE LOW BACK PAIN 1. Use of Imaging Studies for Low Back Pain (LBP): Percent members with a primary diagnosis of low back pain who did not have an imaging study (plain X-ray, MRI, CT scan) within 28 days of diagnosis. 3

MANAGEMENT OF UNCOMPLICATED ACUTE BRONCHITIS IN ADULTS 1. Avoidance of Antibiotic Treatment in Adults with Acute Bronchitis (AAB): Percent of adults 18-64 years of age with a diagnosis of acute bronchitis who were not dispensed an antibiotic prescription. ADULT PREVENTIVE SERVICES (AGES 18 65+) 1. Colorectal Cancer Screening (COL) Percent members 50-75 years of age who had appropriate screening for colorectal cancer. 2. Breast Cancer Screening (BCS) Percent women 50-74 years of age who had a mammogram to screen for breast cancer. 3. Cervical Cancer Screening (CCS)* (not reported in 2014) Percent women 21-64 years of age who were screened for cervical cancer using either of the following criteria: a. Women age 21-64 who had cervical cytology performed every 3 years. b. Women age 30-64 who had cervical cytology/human papillomavirus (HPV) co-testing performed every 5 years. *Due to significant specification changes, NCQA will not publically report this measure for HEDIS 2014. 4. Chlamydia Screening in Women (21-24 years) (CHL) Percent women 21-24 years of age who were identified as sexually active and who had at least one test for chlamydia during the measurement year. 5. Flu Vaccinations for Adults Ages 18-64 (CAHPS ) (FVA) Percent members 18-64 years of age who received an influenza vaccination between July 1 of the measurement year and the date when the CAHPS survey was completed. 6. Flu Vaccinations for Adults Ages 65 and Older (CAHPS ) (FVO) [Medicare] Percent members 65 years of age and older who received an influenza vaccination between July 1 of the measurement year and the date when the Medicare CAHPS survey was completed. 7. Pneumonia Vaccination Status for Older Adults (CAHPS ) (PNU) [Medicare] Percentage members 65 years of age and older who have ever received a pneumococcal vaccine. 8. Aspirin Use and Discussion (CAHPS ) (ASP) a. Aspirin Use: A rolling average represents the percentage of members who are currently taking aspirin. 1. Women 56-79 years of age with at least two risk factors for cardiovascular disease 2. Men 46-65 years of age with at least one risk factor for cardiovascular disease 3. Men 66-79 years of age, regardless of risk factors b. Discussing Aspirin Risks and Benefits: A rolling average represents the percentage of members who discussed the risks and benefits of using aspirin with a doctor or other health care provider. 1. Women 56-79 years of age 2. Men 46-79 years of age 4

PREVENTIVE SERVICES FOR INFANTS AND CHILDREN (BIRTH 24 MONTHS) 1. Childhood Immunization Status (CIS): Percent of children two years of age who had four DTaP, three IPV, one MMR, three Hib, three Hepatitis B, one VZV, four Pneumococcal conjugate, one Hepatitis A, two or three Rotavirus (number varies based on which vaccine is given), and two Influenza vaccines by their second birthday. The measure calculates a rate for each vaccine and nine separate combination rates, see Table 1. Well-Child Visits in the First 15 Months of Life (W15): Percent members who turned 15 months old during the measurement year and who had six or more well-child visits with a PCP during their first 15 months of life. 3. Lead Screening in Children (LSC) [Medicaid] Percent of children 2 years of age who had one or more capillary or venous lead blood test for lead poisoning by their second birthday. -- PREVENTIVE SERVICES FOR CHILDREN AND ADOLESCENTS (AGES 2-21) 1. Well-Child Visits in the Third, Fourth, Fifth and Sixth Years of Life (W34): Percent members 3-6 years of age who had one or more well-child visits with a PCP during the measurement year. 2. Adolescent Well-Care Visits (AWC): Percent enrolled members 12-21 years of age who had at least one comprehensive wellcare visit with a PCP or an OB/GYN practitioner during the measurement year. 3. Chlamydia Screening in Women (16 20 years) (CHL): Percentage of women 16-20 years of age who were identified as sexually active and who had at least one test for chlamydia during the measurement year. 4. Immunizations for Adolescents (IMA): Percent of adolescents 13 years of age who had one dose of meningococcal vaccine and one tetanus, diphtheria toxoids and acellular pertussis vaccine (Tdap) or one tetanus, diphtheria toxoids vaccine (Td) by their 13 th birthday. This measure calculates a rate for each vaccine and one combination rate. 5. Human Papillomavirus Vaccine for Female Adolescents (HPV): Percent of female adolescents 13 years of age who had three doses of the human papillomavirus (HPV) vaccine by their 13 th birthday. 5

ADULT OBESITY 1. Adult BMI Assessment (ABA): Percent members 18-74 years of age who had an outpatient visit and whose body mass index (BMI) was documented during the measurement year or the year prior to the measurement year. CHILDHOOD OBESITY 1. Weight Assessment and Counseling for Nutrition and Physical Activity for Children/Adolescents (WCC): Percent members 3-17 years of age who had an outpatient visit with a PCP or OB/GYN and who had evidence of BMI percentile documentation*, counseling for nutrition, and counseling for physical activity during the measurement year. a. 3-11 Years Old b. 12-17 Years Old c. Total (sum of the two age stratifications) *This measure evaluates whether BMI percentile is assessed rather than an absolute BMI value. OSTEOPOROSIS 1. Osteoporosis Management in Women Who Had a Fracture (OMW) [Medicare] Percent women 67 years of age and older who suffered a fracture and who had either a bone mineral density (BMD) test or prescription for a drug to treat or prevent osteoporosis in the six months after the fracture. MQIC GUIDELINES THAT DO NOT HAVE MEASURES AT THIS TIME: 1. Adolescent Health Risk Behavior Assessment 2. Adults with Systolic Heart Failure 3. Advance Care Planning 4. In Office Use of Sedation 5.
