Have you ever worn a N95 respirator?... 2. Do you currently smoke tobacco or have you smoked tobacco in the last month?... For questions 3-9, if answers are, UABH PLHCP should clarify with the follow-up questions to assist in reviewing the IRUF so medical consultation is compliant with OSHA standards. UABH PLHCP will email all IRUFs to OHS EH. 3. Have you ever had any of the following conditions? A. Seizures (fits)... Follow-up questions: When were you diagnosed with seizures? When was your last seizure? If you have triggers for seizures, what are the triggers? What medications do you take? When was your last visit with your neurologist? Action: UABH PLHCP clears employee for FT if no seizure within one year. Otherwise, refer to OHS EH. B. Diabetes (sugar disease).. Follow-up questions: What was your last A1c? (Preferably within 6 months) When checking glucose routinely, what does it run? (average, peaks, lows) Does MD think that your diabetes is under good control? Action: UABH PLHCP can clear employee for FT even if employee does not know A1c or average glucose. UABH PLHCP advises the employee of the importance of this information from their MD. UABH PLHCP can clear employee for FT as long as A1c is not greater than 9 or average glucose is not greater than 200. Otherwise, refer employee to OHS EH. C. Allergic reactions that interfere with your breathing..... Follow-up questions: Was this asthma related, nasal congestion related, or anaphylactic reaction related? Has this reaction occurred while wearing a N95 respirator? Action: UABH PLHCP can clear employee for FT if employee has an allergic reaction to bee stings, medications, or food allergies. Otherwise, refer employee to OHS EH.

D. Claustrophobia....... Follow-up questions: Does this happen with crowds or in closed spaces like an elevator? How severe is it? Has it happened while wearing a N95 respirator? Have you ever taken medication or seen a MD for it? Action: UABH PLHCP can clear employee for FT as long as employee s claustrophobia is not related to N95 respirator and is not taking medication for claustrophobia. Otherwise, refer employee to OHS EH. E. Trouble smelling odors........ Follow-up questions: Verify that it is smelling versus tasting difficulties. Does it occur with strong odors? Has it interfered with N95 fit testing in past? Note: In a few instances may affect employee s ability to taste. Action: UABH PLHCP can clear employee for FT. 4. Have you ever had any of the following pulmonary or lung problems? If there are yes answers, UABH PHLCP will clarify with employee if the medical condition is ongoing or has resolved with no further follow-up indicated. Action: UAB PLHCP can clear employee for FT if employee had a medical condition in the past that resolved with no further follow-up. If employee has a current ongoing medical condition, then refer to OHS EH. A. Asbestosis..... Action: UABH PLHCP can clear employee for FT if asbestosis has been resolved with no further B. Asthma.. Follow-up questions: Are you currently seeing a pulmonologist or primary MD? Has the employee had a spirometry test within the last year? If so, have employee bring a copy of results to OHS EH appointment. Action: UABH PLHCP can clear employee for FT if asthma has resolved with no further C. Chronic bronchitis. Action: UABH PLHCP can clear employee for FT if chronic bronchitis has resolved with no further D. Emphysema.. Action: UABH PLHCP can clear employee for FT if emphysema has resolved with no further E. Pneumonia.... Action: UABH PLHCP can clear employee for FT if pneumonia has resolved with no further F. Tuberculosis. Action: UABH PLHCP can clear employee for FT if tuberculosis has resolved with no further G. Silicosis. Action: UABH PLHCP can clear employee for FT if silicosis has resolved with no further H. Pneumothorax (collapsed lung).... Action: UABH PLHCP can clear employee for FT if pneumothorax has resolved with no further I. Lung cancer.. Action: UABH PLHCP can clear employee for FT if lung cancer has resolved with no further J. Broken ribs.... Action: UABH PLHCP can clear employee for FT if broken ribs have resolved with no further

K. Any chest injuries or surgeries Action: UABH PLHCP can clear employee for FT if chest injuries/surgeries have resolved with no further follow-up indicated. Otherwise, refer to OHS EH and include related information. L. Any other lung problem that you ve been told about... Action: UABH PLHCP can clear employee for FT if lung problem has resolved with no further 5. Do you currently have any of the following symptoms of pulmonary or lung illness? A. Shortness of breath... B. Shortness of breath when walking fast on level ground or up a slight hill or incline C. Shortness of breath when walking with other people at an ordinary pace on level ground D. Have to stop for breath when walking at your own pace on level ground E. Shortness of breath when washing or dressing yourself. F. Shortness of breath that interferes with your job.. G. Coughing that produces phlegm (thick sputum) H. Coughing that wakes you early in the morning. I. Coughing that occurs mostly when you are lying down J. Coughing up blood in the last month... K. Wheezing. L. Wheezing that interferes with your job... M. Chest pain when you breathe deeply. N. Any other symptoms that you think may be related to lung problems Action: UABH PLHCP refers employee to OHS EH if answers yes to any of the above (question 5 A-N). 6. Have you ever had any of the following cardiovascular or heart problems? A. Heart attack.... B. Stroke.. C. Angina.... D. Heart failure... E. Swelling in your legs or feet. Follow-up questions: Is the swelling ongoing and all the time? Is it related to BP or heart failure? Action: UABH PLHCP can clear employee for FT if employee responds no to both questions. Otherwise, refer to OHS EH. F. Heart arrhythmia (heart beating irregularly).. Follow-up questions: Do you currently have symptoms of shortness of breath, chest pain, or anxiety with your arrhythmias? How frequent? How long do episodes last? Any triggers for arrhythmias? (Such as caffeine, situations, meds) What do you have to do to stop it? Action: UABH PLHCP can clear employee for FT if employee has no SOB, chest pain, or anxiety with arrhythmia. Otherwise, refer to OHS EH.

G. High blood pressure. Follow-up questions: What medications do you take? Is your blood pressure well controlled? Do you see MD regularly for HBP? Action: UABH PLHCP can clear employee for FT if BP is less than 160/100. Otherwise, if BP is greater than 160/100, then UABH PLCHP has employee obtain 3 documented BP readings (1 BP reading/ week). UABH PLCHP emails BP readings to OHS EH for medical clearance. H. Any other heart problem that you ve been told about... Follow-up questions: Do you have any symptoms? (Shortness of breath or chest pain) Action: UABH PLHCP can clear employee for FT if employee has no SOB or chest pain with heart problem. Otherwise, refer to OHS EH. 7. Have you ever had any of the following cardiovascular or heart symptoms? A. Frequent pain or tightness in your chest.. B. Pain or tightness in your chest during physical activity.. C. Pain or tightness in your chest that interferes with your job.. D. In the past two years, have you noticed your heart skipping or missing a beat. Follow-up questions: Do you currently have symptoms of shortness of breath, chest pain, or anxiety with arrhythmias? How frequent? How long do episodes last? Any triggers for arrhythmias? (Such as caffeine, situations, meds) What do you have to do to stop it? Action: UABH PLHCP can clear employee for FT if employee has no SOB, chest pain, or anxiety with arrhythmia. Otherwise, refer to OHS EH. E. Heartburn or indigestion that is not related to eating.. Follow-up questions: Clarify with employee that question asked not related to eating. Does this currently happen after meals? How often? Does this happen when lying down? Do antacids or acid reducers reduce the symptoms? How soon after taking antacid or acid reducer do you get relief? Action: UABH PLHCP can clear employee for FT if employee has symptoms with heartburn, indigestion, or reflux related to eating. Otherwise, if related to angina, refer to OHS EH. F. Any other symptoms that you think may be related to heart or circulation problems... 8. Do you currently take medication for any of the following problems? A. Breathing or lung problems.. B. Heart trouble... C. Blood pressure... Follow-up questions: If not answered from above: What medications do you take? Is your blood pressure well controlled? Do you see your MD regularly for HBP? Action: UABH PLHCP can clear employee for FT if BP is less than 160/100. Otherwise, if BP is greater than 160/100, then UABH PLCHP has employee obtain 3 documented BP readings (1 BP reading/ week). UABH PLCHP emails BP readings to OHS EH for medical clearance.

If you have used a respirator, have you ever had any of the following problems while wearing a N95 respirator? A. I have never used a respirator (go to Question 10).. B. Eye irritation.... C. Skin allergies or rashes. D. Anxiety.. E. General weakness or fatigue... F. Any other problem that interferes with your use of a respirator..... Action: UABH PLHCP refers employee to OHS EH if answers yes to any of the above (question 9 B-F). 10. Will you be wearing a full-face piece respirator OR a self contained breathing apparatus (SCBA)? If, UAB Hospital N95 Respirator Program will refer employee for further evaluation.. 11. Would you like to talk to the health care professional who will review your answers on this form?...
