UBT147X Thai stamp massage Unit reference number: D/507/5503 Level: 3 Guided Learning (GL) hours: 50 Overview This unit will enable learners to develop the knowledge and skills required to prepare for and provide a Thai stamp massage treatment to clients, whilst following salon requirements. Learners will also develop an understanding of the ingredients used in Thai stamps, the possible contra-indications to the treatment, appropriate aftercare advice for Thai stamp massage treatments and the effects and indications for use. Learning outcomes On completion of this unit, learners will: LO1 Know the salon requirements for Thai stamp massage treatments LO2 Understand the ingredients used, effects and indications for Thai stamp massage treatments LO3 Be able to prepare the client, self and the work area for Thai stamp massage treatments LO4 Be able to provide Thai stamp massage treatments Version 7 1

Assessment requirements Learners must produce a portfolio of evidence which includes: 1. Treatment portfolio 2. Summative practical assessment 1. Treatment portfolio Learners must produce a treatment portfolio which contains assessed evidence covering all the theory assessment criteria in the unit. The treatment portfolio must also contain evidence relating to the practical assessment criteria. At a minimum the practical evidence contained in the portfolio for this unit must include client consultation, information and treatment records covering all of the following: Carried out a minimum of 4 - Thai stamp massage treatments should be recorded within the treatment portfolio Carried all types of treatment - Large and small herbal compresses - Working Sen lines - Applying chakra compresses - Lymphatic drainage massage Used all therapy techniques - Patting - Pressing - Pounding - Pummelling - Rolling - Rotation - Side roll - Stretching - Effleurage - Deep breathing Considered all treatment objectives - Stress relief - Muscular pain - Relaxation - Stimulating Given all types of aftercare - Changes to lifestyle - Physical activities after treatment - Healthy eating - Methods of relaxation, e.g. meditation, breathing exercises etc - Healing response 2

Evidence from the one summative practical assessment must also be presented in the treatment portfolio. The treatment portfolio must be completed prior to learners undertaking the one summative practical assessment. 2. Summative practical assessment Learners must carry out a complete Thai stamp massage which will be observed and marked by centre assessors. Learners must achieve all assessment criteria in order to pass and achieve the unit. The practical assessment must take place in a realistic working environment on a real client. At a minimum the summative practical assessment for this unit must cover: Treatment one full body massage using Thai stamps using large and small stamps incorporating Sen lines, lymphatic drainage and chakra compresses (recommended treatment time 75 minutes) All therapy techniques patting, pressing, pounding, pummelling, rolling, rotation, side roll, stretching, effleurage, deep breathing Recorded professional discussion can also be used as an assessment method attached to the practical assessment and is particularly useful for gathering evidence for criteria related to evaluation and reflection. Professional discussions should be planned and recorded. 3

Unit content LO1 Know the salon requirements for Thai stamp massage treatments Describe salon requirements for preparing the work area for Thai stamp massage treatments: Ensure a safe working environment - Clean and hygienic work area - Adequate ventilation - No trailing wires - Effective positioning of couch to allow ergonomic and safe practice - Maintain visible security of client's belongings (box under couch in which to place clothes, bag, shoes, or coat hooks and cupboard for storing belongings securely) Create an effective treatment environment - Soft lighting, adequate space and comfortable temperature, peaceful with soft music playing, pleasant aroma and well ventilated - Providing privacy with effective cleanliness/good hygiene practices - A sanitised trolley and couch, clean linen, couch roll, tissues, bin bag/waste disposal and the importance of visible use of hygiene procedures - Maintaining environmental and sustainable working practices Clean the equipment used - Refer to the manufacturer s instructions before cleaning any facial electrical equipment Explain how professional massage therapists present themselves: Clean, professional uniforms create a positive impression of the massage therapist and the salon. Uniforms should be made from a comfortable fabric to facilitate the stretching involved in the treatment Massage therapists should wear closed-in footwear, no jewellery, no piercings, hair neatly tied back, and any fringe secured Personal hygiene and cleanliness including showering, covering cuts and abrasions, wearing deodorant or antiperspirant, oral hygiene, including clean teeth, fresh breath, are all important elements of professional presentation Explain the salon requirements for record keeping: Accurate appointment systems, stationery, loyalty, rewards, acknowledgement of occasions Consultation record keeping, contra-indications, signatures, refer to existing records, information clear, accurate and in logical order (name, address, contact numbers, age range, reason for treatment, occupation, sport/hobbies, medical history, allergies/hypersensitivity, contact lenses, contra actions) Skin sensitivity tests, adaptations and modifications, recommendations Treatment plan, update record at the end of the treatment, update at each visit, maintained electronically, paper records 4

State the insurance and licensing requirements for Thai stamp massage treatments: Licences are applied for at local council level. Each council will have slightly different requirements but as a general rule both the therapist and the premises will need to be licensed. The most current information on licensing is available on local authority websites and from professional membership association groups As a minimum a salon should hold where applicable, employer s liability insurance as well as public liability insurance and professional indemnity Explain the legal requirements for providing Thai massage treatments to minors: The age up to which an individual is classed as a minor and how this differs nationally 5

LO2 Understand the ingredients used, effects and indications for Thai stamp massage treatments Explain the uses of the main ingredients of a Thai herbal compress: Typical ingredients - Lavender relaxing scent and anti-inflammatory properties - Camphor strong antiseptic, analgesic and anti-inflammatory, stimulates the brain, heart and blood circulation, eases stress, anxiety, depression and insomnia - Cardamom spicy, exotic floral aroma, pain reliever - Cloves for treating chronic pain - Elderberry flowers rids the body of toxins, increases circulation and purifies the blood - Ginger eliminates waste and congestion, enhances circulation and acts as a catalyst for other herbs to increase their effectiveness - Kaffir lime potent antioxidant, banishes colds and congestion, boosts digestion and regulates blood pressure - Lemongrass anti-inflammatory, boosts digestion, releases tension, helping colds and treats skin infections - Mint calms the stomach, intestinal tract and the nervous system, cleanses the skin - Myrrh soothes muscles and wounds, detoxifies the mucous membranes - Tamarind antiseptic and blood purifying effect, used for constipation, colds and fever, skin cleansing and refining - Turmeric antiseptic, antibacterial and anti-fungal properties, relieving pain and has an anti-inflammatory effect Examine different methods that may be used to prepare yourself and the client for a treatment to aim to achieve a meditative mind-set: Meditation, grounding, clearing the mind, protection of negative energies, cleansing of area, crystal clearing, Tibetan sounding bowl Identify the different Sen lines and explain their energy use: 10 Sen-lines - Itha, starts at the left nostril and travels up inside the head, down the throat and neck, down the back, buttock to the back of the knee used to aid headaches, sinus problems, poorly focused eyes, back pain, dizziness, drowsiness, problems relating to hearing, joint pain and cold feet - Pingala, starts at the right nostril follows the same route as Itha but on the other side of the body used to aid red eyes, nasal drips, sneezing, aches and pains, uneasy breathing - Sumana, starts at the tip of the tongue and goes through the throat and chest into the solar plexus region aids issues related to the tongue and talking, tiredness, chest pain, psychological disorders, poor appetite, breathing problems, insomnia - Kalatharee, starts at the navel, divide into four branches, one to each limb and ends at the fingers and toes used to aid poor digestion and sensations, fatigue and muscular cramps in the limbs, joint pain - Sahatsarangsi, starts at the left eye and travels inside the head down the front of the body down the left side of the chest and abdomen, down the outer leg to the feet and up the front of the leg ending just below the navel, aids chest pain, tooth ache, cataract, knee joint pain, numbness, mental health disorders 6

- Tawaree, starts at the right eye follows the same route as Shatasarangsi on the right side of the body aids eye problems, headaches, insomnia - Lawusang starts at the left ear, down the left side of the face and throat towards the nipple and then down to the navel - aids ear problems, facial paralysis, digestive problems - Ulanga, starts at the right ear and follows the same route as Lawusang on the right side of the body also aids insomnia and itchy skin - Nanthakrawat, comprises of two lines, the first (Sikhini) descends through the urethra and ends at the end, the second (Sukhumang) starts at the navel, through the colon and ends at the anus aids hormonal problems relating to the reproductive system, hernias, frequent urination and diarrhoea - Khitchanna, runs from the navel to the penis in men, and the navel through the uterus into the vagina in women aids hormonal problems relating to the reproductive system, hernias, and helps to balance libido Explain the benefits of Thai compress massage both physiologically and psychologically: The possible physiological effects of massage on the body systems: Effects on the skin - Increased production of sweat from the sweat glands, helping to excrete urea and waste products through the skin - Increased sebum production, helping to improve the skin s suppleness and resistance to infection - Improved the skin s colour and elasticity of the skin. - Improved circulation to the skin, increased nutrition and cell regeneration Effects on the skeletal system - Improved muscle tone and balance, reducing the physical stress placed on bones and joints - Increased joint mobility, breaking down scar tissue and adhesions Effects on the muscular system - Increased flexibility in the muscles, relieving muscular tightness, stiffness and spasms - Reduction in muscle fatigue and soreness by increased blood circulation Effects on the cardiovascular system - Improved circulation and enhanced blood flow - Delivery of fresh oxygen and nutrients to the tissues improving the removal of waste products, toxins and carbon dioxide - Temporarily decreases blood pressure, due to dilation of capillaries - Decreases the heart rate due to relaxation Effects on the lymphatic system - Reduces oedema - Strengthens the immune system, due to increase in white blood cells Effects on the respiratory system - Decreased rate of respiration due to the reduced stimulation of the sympathetic nervous system - Improved lung capacity by relaxing any tightness in the respiratory muscles Effects on the digestive system - Increased peristalsis in the large intestine, helping to relieve constipation, colic and gas 7

Effects on the urinary system - Increased urinary output due to the increased circulation and lymph drainage from the tissues Effects on the nervous system - Stimulates or soothes nerves depending on the techniques used - Stimulates the parasympathetic nervous system, helping promote relaxation and the reduction of stress, reduces pain by the release of endorphins The possible psychological effects of massage; - Reduction in both stress and anxiety by relaxing both mind and body and eases emotional trauma - Creates a feeling of well-being and enhanced self-esteem - Promotes positive body awareness and an improved body image through relaxation - Improved sleep patterns Explain the spiritual association and location of each chakra: Base chakra is located in the perineum, the base of the spine it is linked to security and confidence and is spiritually linked to self awareness Sacral chakra located in the lower abdomen, place for passion and sexuality, and spiritually linked to self worth Solar plexus located between the navel and sternum, it is the place where our willpower and ego are located spiritually linked to self control Heart chakra is located in the centre of the chest, it is linked spiritually to self love and love for others Throat chakra is located at the base of the throat, it is spiritually linked to self expression Third eye located between the brows in the centre of the forehead, linked spiritually to psychic abilities and higher consciousness Crown chakra is located on top of the head, it is the direct channel to higher consciousness and for getting in contact with our higher self, our window to the spiritual world or our connection with our God Throat, heart and base chakras are treated with Reiki not direct herbal compress due to heat or client modesty respected Describe the contra-indications to Thai stamp massage: Example of contra-indications that may prevent treatment from going ahead are: contagious diseases and disorders, first trimester of pregnancy, fever, disorders of the nervous system, undiagnosed lumps, osteoporosis, recent major operations, rheumatoid arthritis in acute phase, migraines, if client is under the influence of alcohol, deep vein thrombosis (DVT) 3-6 months after diagnosis, severe haemophilia, acute hepatitis, loss of tactile sensation Examples of contra-indications that may restrict treatment clients undergoing medical treatment, epilepsy, diabetes, high/low blood pressure, history of thrombosis or embolism, varicose veins, medication, pregnancy, piercings, cuts and abrasions, cancer if a client is undergoing chemotherapy or radiotherapy 8

Describe the possible contra-actions that may occur during or after a Thai stamp treatments: Skin irritation such as itchiness, redness, or swelling may occur due to intolerance or allergy to products used Bruising Tender muscles Healing response could include an emotional release, headaches, possible nausea, euphoria, possible increased urination, excessive tiredness 9

LO3 Be able to prepare the client, self and the work area for Thai stamp massage treatments Prepare and check the client, work area, equipment and products prior to the Thai stamp massage treatment: Prepare treatment area according to salon requirements Prepare yourself and your client using the appropriate PPE Choose the herbs that meet the client s requirements Use safe working practices - Positioning of yourself to avoid injury - Positioning of your client - Positioning of your equipment Agree the treatment and check for contra-indications: Verbal communication speaking manner and tone, sensitive to client, open questioning related to treatment Non-verbal communication eye contact, body language, listening Record keeping contra-indications check, signatures, name, address, contact numbers, age range, reason for treatment, occupation, sport/hobbies, medical history, allergies/hypersensitivity, contact lenses, contra actions, skin sensitivity tests Client expectations and preparation ensure that the client has realistic expectations for the treatment, do not give false hope, a thorough consultation should be given to include a clear description of the treatments and its effectiveness for the individual Skin sensitivity test lack of sensation may mean that the treatment cannot be carried out Contra-indications present that may prevent or restrict a treatment Skin condition this will reflect the possible reactions and effects of the treatment Medication some medication can sensitise the skin, e.g. any medication that begins with anti, i.e. anti-biotics, anti-depressants, anti-histamines or make the skin contraindicated to treatment e.g. roaccutane or any retin A based medication and steroid creams Poor homecare routine this will be detrimental to the treatments given and can counteract the effects of the treatment or make the results much slower Explain the treatment and treatment procedure to the client: Explain how the client should position themselves for treatment, how long the treatment will take and the treatment areas for Thai stamp massage. Discuss the client s expectations from the treatment and how this may influence the treatment Instruct the client on clothing removal and positioning for a Thai stamp massage treatment: Removal of clothing as appropriate, positioning throughout the treatment, closing eyes, communicating clearly when you are applying Thai stamp treatments to the body, to explain what is happening 10

LO4 Be able to provide Thai stamp massage treatments Prepare the client for Thai stamp massage treatment: Prepare the body and face for treatment using the correct products for the client s needs Carry out thermal sensitivity test Apply the Thai stamp massage treatment in accordance with manufacturers instructions: Equipment used in line with the manufacturers instructions Apply Thai stamp (herbal compress) using patting, pressing, pounding, pummelling, rolling, rotation, side roll, stretching, manual massage techniques, effleurage, deep breathing Follow Sen lines kalatharee, itha, pingala, sumana, sahatsarangsi, tawaree, lawusang, ulanga using rhythmic rocking movements Apply chakra compresses to meet the client s needs Apply lymphatic drainage massage using effleurage movements working towards the lymph nodes on the back, back of legs, feet, front of legs, arms, hands, abdomen, neck and shoulders, face, head, scalp Carry out the Thai stamp massage treatment using safe and hygienic working practices: Wash hands before and after treatment Use equipment and resources suitable for the treatment plan, following the manufacturers instructions Use Thai stamp in a controlled way to achieve the desired effect Use techniques that minimise the risk of products being spilt Effectively adjust the application techniques to suit the areas of application Ensure that the service is cost effective and is carried out within a commercially viable time Provide aftercare advice: Increase water intake to flush out toxins, this will prevent a headache after treatment Rest after the treatment to optimise the treatment benefits Recommend healthy eating and advice on fluid intake to ensure the client s skin and body is at its optimum health and is receptive to future treatment Improvement to lifestyle activities individual to the client such as cessation/reduction of smoking, stress management Advice and guidance on a suitable skin and body care regime and homecare treatments to suit client s skin type and conditions Postural exercise to prevent muscles strain and fatigue Advice on retail products to continue the beneficial effects of treatment Advise of the possible contra-actions that may occur such as a skin reaction, oedema, headache, bruising, stiffness in muscles, a change in sleeping pattern, increased urination Recommend methods of relaxation, e.g. meditation, yoga, breathing exercises Recommend further treatments, identify the benefits of courses of treatments 11

Update the client records: Record the outcomes of the treatment ensuring records are up to date with the products and equipment used including any notable outcome of treatment, aftercare given and feedback from the client Gain client feedback Reflect on treatment, what went well, what could you have done to make it better Action yourself Prepare the treatment area for the next treatment: Dispose of all waste, clean equipment and products as per manufacturers instructions, ensure the treatment area is left clean and prepared for the next treatment 12

Skin cancer awareness Please note this information will not be assessed for the achievement of this unit. Public awareness of skin cancer has never been higher, and yet skin cancer remains the fastest growing cancer in the UK, especially amongst young people. The chances of a positive outcome can be dramatically increased with early identification and diagnosis. Professionals in hair, beauty, sports massage and health and wellbeing industries work closely with clients and in many cases have sight of areas of skin which may not be easily visible to the client. An informed awareness of the signs, symptoms and changes of appearance to be aware of when checking for early signs of cancer is a crucial tool for the conscientious practitioner in order to provide the most thorough service and in some cases, possibly lifesaving information signposting. Signs to look for when checking moles include utilising the ABCDE guide: A - Asymmetry the two halves of the area/mole may differ in their shape and not match. B - Border the edges of the mole area may be irregular or blurred and sometimes show notches or look ragged. C - Colour this may be uneven and patchy. Different shades of black, brown and pink may be seen. D - Diameter most but not all melanomas are at least 6mm in diameter. If any mole gets bigger or changes see your doctor. E - Elevation/evolving elevation means the mole is raised above the surface and has an uneven surface. Looks different from the rest or changing in size, shape or colour. Anyone can get a suspicious mole or patch of skin checked out for free by the NHS by visiting their doctor, who may then refer to a dermatologist (an expert in diagnosing skin cancer). Assessment guidance Assessors must use the amplified assessment guidance in this section to judge whether assessment criteria have been achieved. P13 Prepare and check the client, work area, equipment and products prior to the Thai stamp massage treatment Learners must demonstrate that they have set their working area correctly in line with organisational procedures, all equipment is tested, checked and confirmed to be in safe working order prior to the treatment, all the required products for the treatment are to hand and are fully stocked in advance. P14 Agree the treatment and check for contra-indications Learners must demonstrate that they have consulted with the client using appropriate questioning techniques, as well as conducting visual and manual checks for contraindications. Learners should explain the treatment in full to the client and ascertain the client s desired result. Learners should correctly identify the client s needs and requirements and recommend the correct products and treatment for the condition. P15 Explain the treatment and treatment procedure to the client Learners must explain the treatment step by step to the client. At a minimum the explanation should include how the body electrical equipment works, the purpose of the different products used in the treatment, the sensations that they may experience throughout the treatment, the features and benefits of the treatment, as well as the likely duration of the treatment. P16 Instruct the client on clothing removal and positioning for a Thai stamp massage treatment Learners must demonstrate that they have provided their client with instructions on how to prepare for the Thai stamp massage treatment. Learners must demonstrate that they instructed the client on what they need to remove, what they need to wear and how they should be positioned to facilitate the treatment. P17 Prepare the client for Thai stamp massage treatment Learners must demonstrate that they have protected the client s hair and clothing, ensured their comfort, carried out skin tests and prepared their equipment for the Thai stamp massage treatment. P18 Apply the Thai stamp massage treatment in accordance with manufacturer s instructions Learners must demonstrate that they have chosen the correct ingredients for the Thai stamp treatment for the client s condition and desired result. Learners must demonstrate that they have successfully followed a Thai stamp massage sequence, as determined by the manufacturer s treatment routine or instructional manual and completed it within a commercially viable time. 17

P19 Carry out the Thai stamp massage treatment using safe and hygienic working practices Learners must demonstrate that they have maintained safe and hygienic practices before, throughout and after the Thai stamp massage treatment to minimise the risk of crossinfection and the risk of injury and fatigue to themselves or others. P20 Provide aftercare advice Learners must demonstrate that they have provided the client with a minimum of the following advice, in addition to any advice recommended by the manufacturer: Rest after treatment, avoid exercise for the next 12-24 hours, changes to lifestyle and diet, increase water intake, methods of relaxation, e.g. meditation, yoga, breathing exercises, professional product recommendations to enhance and maintain the effects of the treatment, courses of treatments/further treatment recommendations. P21 Update the client records Learners must demonstrate that they have updated and recorded the client records in line with salon requirements to cover a minimum of the following information aftercare advice, products used, treatment performed, client s skin reaction to treatment, future recommendations for treatments P22 Prepare the area for the next treatment Learners must demonstrate that they have prepared the treatment area, by cleaning the equipment used, trolley, couch, linen and clearing away and removing any disposable items. The learner should also demonstrate that they have prepared the area for the next client, including by re-stocking any used stock. 18

Resources The special resources required for this unit are access to a real or realistic working environment which supports the provision of Thai stamp massage treatments. Delivery guidance Teachers are encouraged to use innovative, practical and engaging delivery methods to enhance the learning experience. Learners may benefit from: Meaningful employer engagement so they relate what is being learned to the real world of work and understand commercial competency and the use of product, tools and equipment Work experience within a commercial beauty salon or spa so they can practise to hone their skills in a real environment Using interactive information and technology, systems and hardware so they can learn about concepts and theories, research current trends, research product knowledge and produce visual aids Links with other units This unit is closely linked with the following units: UCO28M Health and safety in the salon The health and safety unit will provide knowledge and understanding of the responsibilities for health and safety as defined by any specific legislation covering the role of the professional therapist. This unit greatly underpins all practical unit delivery. Learners will be required to apply their knowledge and understanding of health and safety when preparing for and providing Thai stamp massage treatments in real or realistic working environment. UBT90M Client care and consultation Client consultation before all treatments is actually a legal requirement and failure to consult properly with clients prior to treatment could invalidate your therapy insurance. It is essential that spa therapists elicit information from their clients about their medical history, including any allergies as well as checking for contra-indications. The client consultation unit underpins all technical units within this qualification and should be delivered prior to the delivery of any technical therapy units. UBT95M Anatomy and physiology for the face and body systems Knowledge of the structure and function of the skin, as well as skin diseases and disorders most significantly underpin the knowledge and practical skills gained in this unit. Other elements of anatomy and physiology are also relevant to this unit in terms of understanding contra-indications to treatment as well as possible contra-actions. 19
