Instructions: DHCS publishes the list of diagnoses covered by Medi-Cal in format. The DSM is used because does not provide specific diagnostic criteria. Providers must always use DSM-5 first and whenever possible. A few allowable diagnoses do not have a better or equivalent DSM-5 diagnosis and DSM-IV-TR criteria may be used for these diagnoses. For these diagnoses, shaded in green, providers must use the code and name in the medical record; Do not use the DSM-IV-TR name or code. Crossed out diagnoses may not be used at this time. Z03.89, No Diagnosis may be used during the assessment period, but once an assessment determines that the client does not have a diagnosis, or has a non-included diagnosis, no additional claiming can be made until medical necessity is established for that client. codes are repeated when there are different diagnoses that use the same code.
