About Stage Three, Part One Candidate Physical Ability Test (CPAT) PRE-TEST PROCEDURES CPAT ORIENTATION The CPAT is a widely used, comprehensive physical ability test designed specifically for the fire service. Use of the CPAT has specific requirements for candidate preparation, orientation and practice sessions. Pursuant to the conciliation agreement with the EEOC, the CPAT program must provide all candidates an opportunity to attend at least two CPAT orientations. Additionally, all candidates should receive hands-on familiarity with the test apparatus and receive guidance on specific conditioning regimens and techniques to help them prepare for the test. Each candidate can also be provided an opportunity to perform practice runs of the CPAT. The OFAI offers CPAT Orientation sessions once a week. Page 2 of 20 August 2017

WAIVER Although the orientation and practice must be treated as a mandatory condition for candidates taking the CPAT test, it is recognized that candidates with formalized training believe that they are capable of passing the CPAT without attending an orientation. It is also recognized that resources devoted to the orientation and are best spent on candidates who will truly benefit from this assistance. It is therefore permissible to excuse candidates from this requirement upon receipt of a written and signed waiver from the candidate acknowledging that orientation and practice was made available to all candidates on an equal basis and that the candidate voluntarily and knowingly waives the opportunity to participate in the orientation and practice program. Candidates must sign the waiver if they wish to participate in the CPAT Test without participating in the CPAT Orientation. The job of a fire fighter is one of the most physically demanding jobs in North America. It requires high levels of cardiopulmonary endurance, muscular strength and muscular endurance. The Candidate Physical Ability Test consists of eight critical physical tasks that simulate actual job duties on the fire ground. This test is physically demanding and requires that you be physically fit to be successful. This guide was developed to assist you with physically preparing yourself for the test. The CPAT consists of eight separate events. This test is a sequence of events that requires the candidate to progress along a predetermined path from event to event in a continuous manner. This is a pass/fail test based on a maximum total time of 10 minutes and 20 seconds. In these events, the candidate wears a 50-pound (22.68-kg) vest to simulate the weight of self-contained breathing apparatus (SCBA) and fire fighter protective clothing. An additional 25 pounds (11.34 kg), using two 12.5-pound (5.67-kg) weights that simulate a high-rise pack (hose bundle), is added for the stair climb event. Throughout all events, the candidate must wear long pants, a hard hat with chin strap, work gloves and footwear with no open heel or toe. Watches and loose or restrictive jewellery are not permitted. All props were designed to obtain the necessary information regarding the candidate s physical ability. The tools and equipment were chosen to provide the highest level of consistency, safety and validity in measuring the candidate s physical abilities. The events are placed in a sequence that best simulates their use in a fire scene while allowing an 85-foot (25.91-m) walk between events. To ensure the highest level of safety and to prevent candidate from exhaustion, no running is allowed between events. This walk allows the candidate approximately 20 seconds to recover and regroup before each event. If the candidate runs between events they receive one warning. A second infraction constitutes a failure, the test time is concluded and the candidate fails the test. To ensure scoring accuracy by eliminating timer failure, two stopwatches are used to time the CPAT. One stopwatch is designated as the official test time stopwatch, the second is the backup stopwatch. If mechanical failure occurs on the official stopwatch, the time on the backup stopwatch is used. The stopwatches are set to the pass/fail time and count down from 10 minutes and 20 seconds. If time elapses prior to the completion of the test, the test is concluded and the candidate fails the test. TEST PROCEDURES The CPAT includes eight sequential events as follows: 1. EVENT 1 STAIR CLIMB EQUIPMENT StairMaster StepMill NOTE: Position the unit with one side up against a wall and the specified elevated platform on the side opposite the wall. The handrail on the side opposite the wall is to be removed. The handrail on the wall side is left in place for the candidate to grasp while mounting and dismounting the StepMill. Additional steps are to be placed at the base of the StepMill to reduce the height needed to mount the StepMill. PURPOSE OF EVALUATION This event is designed to simulate the critical tasks of climbing stairs in full protective clothing while carrying a high-rise pack (hose bundle) and climbing stairs in full protective clothing carrying fire fighter equipment. This event challenges the candidate s aerobic capacity, lower body muscular endurance and ability to balance. This event affects the aerobic energy system as well as the following muscle groups: quadriceps, hamstrings, glutes, calves, and lower back stabilizers. EVENT During this event, the candidate is required to wear two 12.5-pound (5.67-kg) weights on the shoulders to simulate the weight of a high-rise pack (hose bundle). Prior to the initiation of the timed CPAT, the candidate has a 20-second warm-up on the StepMill at a set stepping rate of 50 steps per minute [Level 3]. During this warm-up period, the candidate is permitted to dismount, grasp the rail or hold the wall to establish balance and cadence. If the candidate falls or steps off the StepMill during the 20-second warm-up period, the candidate is required to remount the StepMill and restart the entire 20-second warm-up period. The candidate is allowed to restart the warm-up period twice. There is no break in time between the warm-up period and the actual timing of the test. The timing of the test begins at the end of this warm-up period when the proctor calls out START. For the test, the candidate is required to walk on the StepMill at a set stepping rate of 60 steps per minute [Level 4] for 3 minutes. This concludes the event. The two 12.5-pound (5.67-kg) weights are removed from the candidate s shoulders. The candidate walks 85 feet (25.91 m) within the established walkway to the next event. The following practices are allowed: The candidate is allowed to briefly touch the handrails or wall for balance The candidate is given up to two warnings for grabbing the handrails or bearing their body weight on the handrails / wall The candidate is allowed to restart the warm-up period twice The following practices constitute a failure: The candidate falls or voluntarily dismounts the Step Mill three times during the warm up. The candidate falls or voluntarily dismounts the Step Mill after the start of the test. The candidate commits a third infraction for grasping the handrails or bearing weight on the handrails / wall after the start on the test. Reasons for failure Falling demonstrates poor balance or muscular endurance and could cause injury to the candidate. Using the handrails or wall for weight bearing gives the candidate a mechanical advantage that may not be available to them on the fire ground or demonstrates poor balance, conditioning or muscular endurance. EVENT 2 - HOSE DRAG EQUIPMENT This event uses an uncharged fire hose with a hoseline nozzle. The hoseline is marked at 8 feet (2.24 m) past the coupling at the nozzle to indicate the maximum amount of hose you are permitted to drape across your shoulder Page 4 of 20 August 2017

or chest. The hoseline is also marked at 50 feet (15.24 m) past the coupling at the nozzle to indicate the amount of hoseline that you must pull into a marked boundary box before completing the test. PURPOSE OF EVALUATION This event is designed to simulate the critical tasks of dragging an uncharged hoseline from the fire apparatus to the fire occupancy and pulling an uncharged hoseline around obstacles while remaining stationary. This event challenges the candidate s aerobic capacity, lower body muscular strength and endurance, upper back muscular strength and endurance, grip strength and endurance, and anaerobic endurance. This event affects the aerobic and anaerobic energy systems as well as the following muscle groups: quadriceps, hamstrings, glutes, calves, lower back stabilizers, biceps, deltoids, upper back, and muscles of the forearm and hand (grip). EVENT During this event, the candidate grasps an automatic nozzle attached to 200 feet (60 m) of 1 3/4-inch (44-mm) hose. The candidate places the hoseline over the shoulder or across the chest, not exceeding the 8-foot (2.44-m) mark. The candidate is permitted to run during the hose drag. The candidate drags the hose 75 feet (22.86 m) to a prepositioned drum, makes a 90 turn around the drum and continues an additional 25 feet (7.62 m). The candidate then stops within the marked 5 foot x 7 foot (1.52 m x 2.13m) box, drops to at least one knee and pulls the hoseline until the hoseline s 50-foot (15.24-m) mark is across the finish line. During the hose pull, the candidate must keep at least one knee in contact with the ground and knee(s) must remain within the marked boundary lines. This concludes the event. The candidate walks 85 feet (25.91 m) within the established walkway to the next event. The following practices are allowed: The candidate is given one warning to keep one knee down. The candidate is given one warning to keep the knees in bounds. The candidate is given one warning for taking one step out of the box. The candidate is permitted to run during the hose drag The following practices constitute a failure: The candidate fails to go around the drum. The candidate travels outside of the marked path. The candidate takes two steps out of the back of the box The candidate receives a second infraction for not keeping one knee in contact with the ground. The candidate receives a second infraction for the knees being outside of the marked boundary. Reasons for failure: Running beyond the marked path gives the candidate a mechanical advantage by decreasing the distance required to pull the hose by hand. This advantage may not be available on the fire ground. This demonstrates a lack of upper body strength by using lower body strength to compensate. By not keeping their knee on the floor a candidate could compensate for a deficiency in grip and upper body strength by standing up. EVENT 3 EQUIPMENT CARRY EQUIPMENT This event uses two saws and a tool cabinet replicating a storage cabinet on a fire truck. PURPOSE OF EVALUATION This event is designed to simulate the critical tasks of removing power tools from a fire apparatus, carrying them to the emergency scene and returning the equipment to the fire apparatus. This event challenges the candidate s Page 5 of 20 August 2017

aerobic capacity, upper body muscular strength and endurance, lower body muscular endurance, grip endurance, and balance. This event affects the aerobic energy system as well as the following muscle groups: biceps, eltoids, upper back, trapezius, muscles of the forearm and hand (grip), glutes, quadriceps, and hamstrings. EVENT During this event, the candidate removes the two saws from the tool cabinet, one at a time, and places them on the ground. The candidate then picks up both saws, one in each hand, and carries them while walking 75 feet (22.86m) around the drum, then back to the starting point. The candidate is permitted to place the saw(s) on the ground and adjust the grip. Upon return to the tool cabinet, the candidate places the saws on the ground, then picks up each saw one at a time, and replaces the saw in the designated space in the cabinet. This concludes the event. The candidate walks 85 feet (25.91 m) within the established walkway to the next event. The following practice is allowed: The candidate is given one warning for running. The candidate is allowed to set the tools on the ground to adjust and re-establish the grip. The following practices constitute a failure: The candidate drops either saw during the carry. The candidate receives a second infraction for running with the saws. Reasons for failure: Dropping the saws could injure the candidate and demonstrates poor grip strength or muscular endurance. Running with saws could cause injury if the candidate trips. EVENT 4 LADDER RAISE AND EXTENSION EQUIPMENT This event uses two 24-foot (7.32-m) fire department ladders. For your safety, a retractable lanyard is attached to the ladder that you raise. PURPOSE OF EVALUATION This event is designed to simulate the critical tasks of placing a ground ladder at a fire structure and extending the ladder to the roof or window. This event challenges candidate s aerobic capacity, upper body muscular strength, lower body muscular strength, balance, grip strength, and anaerobic endurance. This event affects the aerobic and anaerobic energy systems as well as the following muscle groups: biceps, deltoids, upper back, trapezius, muscles of the forearm and hand (grip), glutes, quadriceps, and hamstrings. EVENT During this event, the candidate walks to the top rung of the24-foot (7.32-m) aluminum extension ladder, lifts the first rung at the unhinged end from the ground, and walks it up until it is stationary against the wall. This must be done in a hand over hand fashion, using each rung until the ladder is stationary against the wall. The candidate must not use the ladder rails to raise the ladder. The candidate immediately proceeds to the pre-positioned and secured 24-foot (7.32-m) aluminum extension ladder, stands with both feet within the marked box of 36 inches x 36 inches (91.44 cm x 91.44cm) and extends the fly section hand over hand until it hits the stop. The candidate then lowers the fly section hand overhand in a controlled fashion to the starting position. This concludes the event. The candidate walks 85 feet (25.91 m) within the established walkway to the next event. The following practices are allowed: The candidate is given one warning for missing any rung during the raise. The candidate is given one warning for a boundary violation during the ladder extension. The following practices constitute a failure: The candidate receives a second infraction for missing any rung during the raise. The candidate allows the ladder to fall to the ground during the raise. The candidate releases their grip on the ladder and the safety lanyard activates. The candidate receives a second infraction for not remaining within the marked boundary during the ladder extension. The candidate does not control the halyard in a handover hand manner. The candidate allows the halyard to slip in an uncontrolled manner. Reasons for failure: Skipping rungs would give a taller candidate an advantage over a shorter candidate and is therefore not permitted. It would also allow the candidate to throw the ladder up in the air which is both unsafe and unavailable to the candidate at a fire scene when the base of the ladder is not hinged to the ground. Failure to completely raise the ladder demonstrates poor grip and muscular strength. A candidate could gain an advantage by walking the halyard backward to compensate for poor upper body strength. This compensation is not available on the fire ground where the ladder is not bolted to the fire structure. Failure to control the ladder indicates poor grip strength as well as muscular strength and endurance. EVENT 5 FORCIBLE ENTRY EQUIPMENT This event uses a mechanized device located 39 inches (1m) off the ground that measures cumulative force and a10-pound (4.54-kg) sledgehammer. PURPOSE OF EVALUATION This event is designed to simulate the critical tasks of using force to open a locked door or to breach a wall. This event challenges the candidate s aerobic capacity, upper body muscular strength and endurance, lower body muscular strength and endurance, balance, grip strength and endurance, and anaerobic endurance. This event affects the aerobic and anaerobic energy systems as well as the following muscle groups: quadriceps, glutes, triceps, upper back, trapezius, and muscles of the forearm and hand (grip). EVENT During this event, the candidate uses a 10-pound (4.54-kg) sledgehammer and strikes the measuring device in the target area until the buzzer signal is activated. The candidate s feet must remain outside the toe-box. After the buzzer is activated, the candidate places the sledgehammer on the ground. This concludes the event. The candidate walks 85 feet (25.91m) within the established walkway to the next event. The following practice is allowed: The candidate is given one warning for stepping inside the toe-box. EVENT 6 SEARCH EQUIPMENT This event uses an enclosed search maze that has obstacles and narrowed spaces. PURPOSE OF EVALUATION This event is designed to simulate the critical task of searching for a fire victim with limited visibility in an unpredictable area. This event challenges the candidate s aerobic capacity, upper body muscular strength and endurance, agility, balance, anaerobic endurance, and kinaesthetic awareness. This event affects the aerobic and anaerobic energy systems as well as the following muscle groups: muscles of the chest, shoulder, triceps, quadriceps, abdominals, and lower back. EVENT During this event, the candidate crawls on hands and knees through a tunnel maze that is approximately 3 feet (91.44 cm) high, 4 feet (121.92 cm) wide and 64 feet (19.51 m) in length with two 90 turns. At a number of locations in the tunnel, the candidate navigates around, over and under obstacles. In addition, at two locations, the candidate crawls through a narrowed space where the dimensions of the tunnel are reduced. The candidate s movement is monitored through the maze. If for any reason, the candidate chooses to end the event, the candidate calls out or raps sharply on the wall or ceiling and the candidate is then assisted out. Upon exit from the maze, the event is concluded. The candidate walks 85 feet (25.91 m) within the established walkway to the next event. The following practices are allowed: The candidate can return into the tunnel if they exit through the entrance. The following practices constitute a failure: The candidate requests assistance from the proctor requiring the opening of an escape hatch or the entrance/exit covers. Reasons for failure: Failure to finish the event indicates a lack of confidence in dark or confined spaces. EVENT 7 RESCUE EQUIPMENT This event uses a weighted mannequin equipped with harness with shoulder handles. PURPOSE OF EVALUATION This event is designed to simulate the critical task of removing victim or injured partner from a fire scene. This event challenges the candidate s aerobic capacity, upper and lower body muscular strength and endurance, grip strength and endurance, and anaerobic endurance. This event affects the aerobic and anaerobic energy systems as well as the following muscle groups: quadriceps, hamstrings, lutes, abdominals, torso rotators, lower back stabilizers, trapezius, deltoids, latissimus dorsi, biceps, and muscles of the forearm and hand (grip). During this event, the candidate grasps a 165-pound (74.84-kg) mannequin by the handle(s) on the shoulder(s) of the harness (either one or both handles are permitted), drags it35 feet (10.67 m) to a pre-positioned drum, makes a 180 turn around the drum, and continues an additional 35 feet(10.67 m) to the finish line. The candidate is not permitted to grasp or rest on the drum. It is permissible for the mannequin o touch the drum. The candidate is permitted to lower the mannequin to the ground to adjust their grip. The entire mannequin must be dragged past the marked finish line. This concludes the event. The candidate walks 85 feet (25.91 m) within the established walkway to the next event. The following practices are allowed: The candidate receives one warning for grabbing or resting on the drum. The candidate is permitted to grab either one or both handles when dragging the mannequin The candidate is permitted to lower the mannequin to the ground to adjust their grip The following practices constitute a failure: The candidate receives a second infraction for grabbing or resting on the drum. Reasons for failure: Use of the drum by either grasping or resting on it indicates lack of muscular strength and endurance. EVENT 8 CEILING BREACH AND PULL EQUIPMENT This event uses a mechanized device that measures overhead push and pull forces and a pike pole. The pike pole is a commonly used piece of equipment that consists of a 6-foot long pole with a hook and point attached to one end. PURPOSE OF EVALUATION This event is designed to simulate the critical task of breaching and pulling down a ceiling to check for fire extension. This event challenges the candidate s aerobic capacity, upper and lower body muscular strength and endurance, grip strength and endurance, and anaerobic endurance. This event affects the aerobic and anaerobic energy systems as well as the following muscle groups: quadriceps, hamstrings, glutes, abdominals, torso rotators, lower back stabilizers, deltoids, trapezius, triceps, biceps, and muscles of the forearm and hand (grip). EVENT During this event, the candidate removes the pike pole from the bracket, stands within the boundary established by the equipment frame, and places the tip of the pole on the painted area of the hinged door in the ceiling. The candidate fully pushes up the 60-lb hinged door in the ceiling with the pike pole three times. The candidate then hooks the pike pole to the 80-lb ceiling device and pulls the pole down five times. Each set consists of three pushes and five pulls. The candidate repeats the set four times. The candidate s permitted to stop and, if needed, adjust the grip. Releasing the grip or slipping from pike pole handle, without the pike pole falling to ground, does not result in a warning or constitute a failure. The candidate may re-establish the grip and resume the event. If the candidate does not successfully complete a repetition (i.e. complete the up and down motion), the proctor calls out MISS and the candidate must push or pull the apparatus again to complete the repetition. The event and the total test time ends when the applicant completes the final pull stroke repetition as indicated by the proctor who calls out TIME. The following practices are allowed: The candidate receives one warning for dropping the pike pole on the ground. The candidate receives one warning for stepping out of bounds. The following practices constitute a failure: The candidate receives a second infraction for stepping outside of the boundary marked by the testing apparatus. The candidate receives a second infraction for dropping the pike pole. Reasons for failure: Stepping out of bounds creates an advantage that may not be available to the candidate on the fire ground, which would allow the candidate to compensate for poor upper body strength Failure to maintain control of the pike pole indicates poor grip strength and muscular endurance. About Stage Three, Part Two Firefighter Technical Skills Assessment The Firefighter Technical Skills Assessments are based on of Bona Fide Occupational Requirements. Each candidate will be given specific instructions and MUST perform each evolution as per evaluator instructions and in the safest manner possible. Evaluator must ask if candidate would like to read instructions or have instructions read to them. Candidate has a maximum of 5 minutes to review instructions, then candidate must start the evolution. Candidate may request one re-read of the skill instructions during the evaluation. The time clock will not be stopped during the re-read. If you choose a re-read during your evaluation, you can choose to read the skill yourself, or the evaluator can read it to you. If you choose to have the evaluator read the skill to you, they are only permitted to read the skill in full. At no time will they only read part of the skill. It is also important to note that a re-read will not be taken into consideration in a grievance process (i.e., evaluator is not responsible for a failure based on a re-read). The testing grounds can be noisy, making hearing difficult and candidate comprehension can be altered due to the time sensitive nature of the evaluation. Each evolution has a time limit of 10 minutes. Candidates will move through each evolution in the same order. You must pass each evolution skill to receive a final pass to obtain your OFAI certificate. The Firefighter Technical Skills Assessment is valid for 12 months. If you are not successful in this assessment, candidates must register to re-take this assessment as per the re-test policy. Important to Note: You will be asked before each evolution if you understand the directions and if you would like to voluntary withdraw. Ontario Fire Administration Inc. reserves the right to change the order of the Firefighter Technical Skills Assessment due to weather, logistics etc. You must pass ALL 6 EVOLUTIONS to pass this assessment. If you fail an evolution, you are required to leave the fire grounds immediately. Please Note: In order for candidates to be successful in the Firefighter Technical Skills Assessment, some form of formal firefighter training should be completed prior to attempting the six evolutions. This includes, but is not limited to: pre-service, private institution training and volunteer firefighting experience. Firefighter Technical Skills Assessments are based on Bona Fide Occupational requirements, with many of the skills coming from NFPA 1001 Level I and Level II, and material found in IFSTA's Essentials of Firefighting and Fire Department Operations (Version 6). Candidates are encouraged to review IFSTA and ensure they are familiar with the equipment required and instructions for each skill. Please Also Note: Candidate will be required to perform the evolutions dressed properly in Personal Protective Equipment (PPE). Candidates may bring their own Personal Protective Equipment (PPE). If you do not have your own, we have many types and sizes available. It will be up to each candidate to ensure they have PPE and that it is donned properly. You will receive one warning if PPE is incorrectly donned, second warning results in a failure. It is the responsibility of the candidate to communicate with the evaluator regarding fit of all PPE. You must advise the evaluator of any concerns prior to the commencement of your assessment. Page 10 of 20 August 2017

You are required to partake in evolutions using SCBA. SCBA will be provided on-site. We do not test candidate knowledge on SCBA. Candidates are encouraged to be clean shaven for this assessment. Facial hair can cause the SCBA mask to have an improper seal leading to air leakage and possible failure of this assessment. Questions can be forwarded to the OFAI Administration Office. The six evolutions and descriptions are described below. They are in the order in which you take the assessment: Skill 1: 10 Metre Ladder Climb Each candidate must be dressed properly in full Personal Protective Equipment (PPE) for this skill test. Candidates will use the following equipment: Bunker gear, helmet and gloves 10m ladder tied off to balcony railing Pick head axe Ropes Roof Ladder Safety Harness You will be required to wear a safety harness for this skill. Prepare the axe for lifting using the appropriate rope & clove hitch knot with two half hitches. You must then tie two additional knots. You must tie a bowline knot around the ladder prop as though you would be hoisting it, and then a Figure 8 on a bight knot on a separate rope. When you have completed your knots, you must ask the evaluator if you can proceed. If the evaluator says no, you must determine and fix your errors. When advised to proceed by the evaluator, climb the 10m ladder until you encounter a rung with tape wrapped around it and apply a proper leg lock around the taped rung. Take both hands off the ladder, reach around and touch each of your toes. Climb to the 3rd floor balcony and step off the ladder and onto the balcony. Raise the axe up to balcony, and then lower back to the ground, using the hand-over-hand method. Climb back onto the ladder and descend. Skill 2: Self Contained Breathing Apparatus (SCBA) Proficiency Test Each candidate must be dressed properly in full Personal Protective Equipment (PPE) with a SCBA (Self Contained Breathing Apparatus) and a blacked out face mask for this skill test. Test facility will ensure candidate has a full tank of air for this evolution. Candidates will use the following equipment: Bunker gear, helmet and gloves, flash hood SCBA (Self Contained Breathing Apparatus) Blacked out face mask Don SCBA (Self Contained Breathing Apparatus) and approach SCBA Proficiency structure on air. Enter the structure when indicated to do so by evaluator and perform a proper search technique through the entire maze, passing each obstacle as they are encountered. You must exit at point of entry, once the proper search has been completed. Remain in a crawl position. Do not attempt to stand up, crawl outside or remove obstacles while inside maze. This will constitute a fail. If at any point you feel uncomfortable inside the maze please inform your evaluator. The evolution will end, and this will constitute a fail. The partial or full removal of the SCBA is also not permitted, and will result in a fail. For our purposes today the evaluator will accompany you to observe you while you are being tested. Skill 3: Roof Ventilation Each candidate must be dressed properly in full Personal Protective Equipment (PPE) and SCBA (Self Contained Breathing Apparatus) for this skill test. Test facility will ensure candidate has a tank of air that is at least ¾ full for this evolution. Candidates will use the following equipment: Bunker gear, helmet and gloves SCBA Page 11 of 20 August 2017

Roof ladder Pick head axe Pike pole You must choose the equipment needed to be taken up the roof ladder to ventilate the roof. You must correctly place the roof ladder and demonstrate actions required to determine the roof is safe to work on. Once positioned on the roof, cut a 22 x 22 (55cm x 55cm) vent hole using the appropriate tools in the location indicated by the evaluator. You will ask the evaluator to proceed when you have advised the evaluator that you have completed cutting your hole. The evaluator will use a template to measure the hole size. If the hole size is too large, you will automatically fail. If the hole size is too small, you will have the opportunity to correct the size. If the evaluator advises the hole is the sufficient size, you will simulate opening the ceiling below the roof using the pike pole to make contact with the base of the structure three times. Once completed, you will proceed to climb down the ladder, return to the ground and place equipment on the ground at the base of the ladder. For our purposes today the evaluator will stay at the foot of the roof simulator and observe you. Skill 4: 7 Metre Ladder Roof Ladder Raise Each candidate must be dressed properly in full Personal Protective Equipment (PPE) and SCBA (Self Contained Breathing Apparatus) for this skill test. Candidates will use the following equipment: Bunker gear, helmet and gloves SCBA (not on air) One 7m ladder One roof ladder You must be dressed in full PPE (Personal Protective Equipment) and SCBA (not on air). Using proper lifting techniques, pick up the 7m ladder and perform a one firefighter raise. Inform the evaluator whether you will complete a beam or flat raise. These are the only acceptable raises. Once the 7m ladder is correctly in place, evaluator will inform you if it s safe to proceed. If the evaluator says NO you must make the appropriate corrections to proceed. When advised it is safe to proceed, climb the 7m ladder and proceed to deploy the roof ladder correctly, and safely. Once the evolution is complete, return the roof ladder to the ground. For safety reasons, you must request the assistance of the evaluation staff to butt and steady the 7m ladder before raising fly and prior to climbing. Your time will start when the 7m ladder is picked up. Time will stop when you and the roof ladder return to the ground. Skill 5: Fine Motor Skills Hose Assembly Each candidate must be dressed properly in full Personal Protective Equipment (PPE) for this skill test. Candidates will use the following equipment: Bunker gear, helmet and gloves SCBA (no mask) Hydrant Hydrant Wrench Hose Wrenches 65 mm hydrant gate valve 2 lengths of 100mm hose 2 lengths of 65mm hose 1 length of 45mm hose 2 gated wyes (100mm to two 65mm and 65mm to two 38mm) 45mm combination nozzle 65mm nozzle Adapter from 65mm to 100mm Make all connections with the appropriate hoses and appliances provided. You must assemble hoses and appliances safely and properly as you would on the fire ground. Establish a water supply from the hydrant with a 100mm hose to the pump panel. Establish a hose lay from the pump panel with a 100mm hose to two 65mm Page 12 of 20 August 2017

hoses. One 65mm hose will terminate with a 65mm nozzle. The other 65 mm hose will terminate with a gated wye reduced to two 38mm discharges. One 45mm attack line must be attached to the 38mm gated wye and terminated with a combination nozzle on the end. Once these connections have been successfully established, the evaluator will advise you to proceed to the charged line and direct water through the hole on the target. Skill 6: Medical Skills Candidate must be dressed in Personal Protective Equipment (PPE), gloves and eye protection, for this skill test. In order to successfully complete this skills test, candidates must have advanced first aid training, CPR training and the competence and training required to insert a properly sized oropharyngeal airway, operate a bag valve mask, apply an adult non-re-breather mask and describe the oxygen flow rate or concentration required. Patient Assessment/Management Skills Test In this skill, you will perform your assessment and verbalize your treatment for all conditions and injuries discovered. You must conduct your assessment as you would in the field, including communicating with your simulated patient. As you progress through this skills test, you must state everything you are assessing. Specific clinical information not obtainable by visual or physical inspection, for example respiration, signs and symptoms, will be provided to you by asking the evaluator after you demonstrate how you would normally obtain that information in the field. You can assume you have at least one firefighter with you who is trained to your level of care. They will correctly perform the verbal treatments and direction that you provide as necessary. YOU MUST DEMONSTRATE ALL SKILLS PRIOR TO DELEGATING The evaluator will acknowledge your treatments and may ask for additional information if clarification is needed. Stage Three, Part Two Firefighter Technical Skills Safety Safety during the Firefighter Technical Skills is of the utmost importance and the number one priority of the evaluators. Participating in an un-safe manner may constitute in an automatic failure. Your evaluator may give you one warning depending on the severity of the situation. If you receive a warning, there will be no second warning. You will be advised to cease your task immediately. Stage Three, Part Two Firefighter Technical Skills Assessment Validity Stage Three, Part Two Firefighter Technical Skills Assessment is valid for 12 months. Important to Note: You are issued one Stage Three certificate when you have successfully completed both part one and two of Stage Three. We do not provide separate certificates for the CPAT and/or the Firefighter Technical Skills. Please also note that if you have not successfully completed your Firefighter Technical Skills within six months of passing your CPAT, you will be required to re-test all of Stage Three. Candidate ID All candidates are given a unique ID when registering for an assessment. This unique number will not change and will remain as your personal identifier. Do not share this number with anyone. The OFAI refers to this identification number when reviewing results, for generation of certificates and to provide municipalities with verification of validity. What to Bring on your Assessment Day Government issued identification Confirmation of registration Water and a light snack (for after the assessment) Comfortable clothing that maximizes movement. T-shirt, shorts/track pants, running shoes, socks etc. Change of clothes. Throughout the CPAT you must wear long pants, a hard hat with chin strap, work gloves and footwear with no open heel or toe. Watches and loose or restrictive jewellery are not permitted. Weights, hard hat, and work gloves will be provided. OFAI Medical Questionnaire and, if required, the OFAI Medical Clearance Form. PPE (for Firefighter Technical Skills only not required) Check-In Please ensure you arrive at least 15 minutes prior to your assessment start time. This will allow you enough time to check-in, complete the required forms and use the restrooms. Once you arrive at FESTI, you will be required to check-in. Please have a seat in the cafeteria and your evaluator will call you name for check-in. You will be required to show your identification and/or registration confirmation and initial your name on the check-in list. Please ensure you are ready to go and dressed appropriately. It is the responsibility of the candidate to ensure they have registered for the correct date/time. If you are not on the check-in list, you must contact the OFAI administration office immediately. It is recommended that you confirm your appointment 24 hours in advance of your assessment. Please note that FESTI staff is responsible for proctoring this test only and DO NOT facilitate the registration process. What to Expect when you arrive for Stage Three, Part One CPAT 1. You will be required to check-in (as noted above). At check-in, you will be required to submit the following forms: a. Acknowledgement and Waiver of CPAT Orientation This form must be completed if you do not wish to participate in the CPAT Orientation. (see forms at the end of this document) 2. Please wait in the cafeteria for an evaluator who will to take you to the test location and provide direction. Based on your assessment selection, the following may occur: If you successfully pass this assessment (Stage Three, Part One) and are registered for Stage Three, Part Two Firefighter Technical Skills Assessment, the evaluator will hand you your assessment results form. You will require your assessment results form in order to check into the next assessment. You will be advised that you have a thirty minute break. Your evaluator will give you instructions on where to check in for your next assessment. If you successfully pass this test (Stage Three, Part One) and you only registered to take this assessment today, the evaluator will show you your assessment result form. The evaluator will keep the form to send to the administration office for your file. Please gather your belongings and you are free to leave. If you are NOT successful and fail this assessment (Stage Three, Part One), the evaluator will show you your assessment result form. The evaluator will keep the form to send to the administration office. If you are registered in Stage Three, Part Two Firefighter Technical Skills, and you do not have a successful result in Stage Three, Part One CPAT Assessment, than you cannot continue in the assessment process. The OFAI administration office will refund you for any assessments not participated in, less a $10.00 plus HST administration fee. You will not be refunded for the failed assessment. Please see the re-test policy below. Questions regarding your assessment should be facilitated through the OFAI Administration office. Based on your assessment, the following may occur: If you successfully pass this assessment (Stage Three, Part Two) you have completed all necessary stages for your OFAI certificate. If you failed one of these evolutions, you will be notified immediately. You will be briefed and asked to leave the testing area. Questions regarding your assessment should be facilitated through the OFAI Administration office. Important Notes and Information This document is a guide to assist and prepare you for your Stage Three Testing Process. Some of the information contained in this guide may change once on-site. Please ensure you follow your evaluator s instructions. In the case of a technical problem, the evaluator will handle each situation on a case-by-case basis. If there is a wide-spread error (such as a loss of power) your assessment may be required to be re-scheduled. This will be determined at the discretion of FESTI evaluators/staff. FESTI has shower facilities that may be available for candidate use after any physical assessments (male and female facilities are available). You will have to provide your own toiletries, towels etc. Certificates Once you have successfully completed the CPAT and Firefighter Technical Skills, you will receive your OFAI Stage Three certificate. Your certificate will be uploaded to your online account within two to four business days. All valid certificates are offered as a downloadable PDF from your online user account www.ofai.ca/user. It is recommended that candidates download and save certificates to their personal computers. Once a certificate has expired it is no longer available for download from your account. Re-Testing Policy All OFAI failed assessments are subject to our re-test policy. The re-test policy identifies the wait times candidates must follow before coming back to re-test. The wait times are put in place to allow each candidate sufficient time to prepare (and in some cases, train) in order to be successful in their assessments. 4th Failure 180 Days Anything after a 4th failure will be subject to 180 day re-test policy. Inclement Weather Policy Safety is of the utmost consideration when we are conducting our assessments. When severe weather conditions exist, such as lightning, rain, sleet, snow or high winds, the staff at FESTI shall make an assessment of the conditions and will determine, at their discretion, if assessments will cease until conditions approve. Grievances All grievances are handled through the OFAI administration office. Grievances are not handled by FESTI staff. Please do not approach a FESTI evaluator or proctor with your grievance. You can retrieve a grievance form from FESTI, the OFAI administration office or download from the OFAI website https://www.ofai.ca. All grievances are handled in a timely manner by management. Terms and Conditions All fees are in Canadian Dollars. You are responsible for downloading the pre-assessment guides for your applicable assessment at www.ofai.ca/pre-assessment-guides and completing all the applicable forms in each guide to bring with you on your assessment day. Ontario Fire Administration Inc. (OFAI) reserves the right to cancel a scheduled assessment. In the unlikely event of an assessment cancellation, we will issue a full refund for assessment fees only. OFAI is not responsible for any statements, acts, materials, or omissions by our evaluators, proctors or participants. The use of audio and video taping devices, beepers, and cell phones by participants is not permitted at any assessment. Children and unregistered guests are not permitted in the testing facility. Cancellation of an assessment caused by any calamity, attack, or act of God, beyond the control of the OAFC and/or its third party affiliates and its agents does not constitute grounds for a refund. Rescheduling Policy: The OFAI will only re-schedule an appointment once. In order to be eligible for rescheduling, you must provide us with a written request that states your name, candidate ID number, your testing date/time and your requested re-schedule date/time. This request must be received no later than 7 days from your appointment date. Not all re-scheduling requests are granted. During peak recruitments, the re-scheduling policy may not be in effect. Candidates wishing to re-schedule are still bound to the terms and conditions of their original appointment. Cancellation Policy: There will be no refunds if your assessment is cancelled within 5 business days of your assessment date/time. Any cancellations made prior to the 5 business day deadline will be refunded, less a $50.00 plus HST administration fee. All cancellations must be in writing and emailed to info@ofai.ca to request the cancellation of the booked assessment(s). It is the user's responsibility to abide by the cancellation policy of each assessment. Candidates seeking cancellation within the 5 business day period prior to their test date will only receive a refund (less the $50 plus HST administration fee) if they can provide evidence that their ability to complete their assessment has been affected by serious illness or cause as listed below. Serious Causes: Serious Illness - hospital admission or serious injury: this must be accompanied by a medical note from a registered Medical Doctor. Loss or Bereavement - death of a close family member: accompanied by a death certificate. Hardship/Trauma - victim of a crime, victim of a traffic accident. ACKNOWLEDGEMENT AND WAIVER OF CPAT ORIENTATION AND/OR CONDITIONING PERIOD AND/OR TIMED PRACTICE RUNS. This form must be signed before you will be permitted to take the Candidate Physical Ability Test (CPAT) if you did not attend the orientation and practice sessions prior to this test. A fire department administering CPAT as a condition of hire, must ensure that all candidates were provided full and equal access to a CPAT orientation and practice program. The orientation and practice program must commence at least eight (8) weeks before commencement of the official CPAT test date. This program is composed of two phases. 1. The fire department will provide each candidate a full and equal opportunity to attend at least two (2) orientation sessions during which candidates will receive hands-on familiarity with the actual CPAT test apparatus. These required orientation sessions will be provided by certified Peer Fitness Trainers, fitness professionals and/or CPAT-trained fire fighters (proctors). These individuals will familiarize each candidate with each CPAT task and the test apparatus. They will advise all candidates concerning specific conditioning regimens and techniques to help each candidate prepare for the CPAT test. 2. The fire department will provide each candidate a full and equal opportunity to attend at least two (2) timed practice runs of the CPAT, using CPAT apparatus. These required practice runs must occur within thirty (30) days before the commencement of the official CPAT test dates. Following each practice session, certified Peer Fitness Trainers, fitness professionals, and/or CPAT-trained fire fighters (proctors) shall help the candidates understand the test elements and how they can improve their performance and conditions. This two-phased orientation and practice program is a mandatory condition for candidates taking the CPAT test. However, it is recognized that some individuals may be capable of passing CPAT without participation in these programs. These individuals may excuse themselves from this mandatory condition upon the receipt by the fire department of a written and signed waiver, acknowledging that the fire department made available these programs on an equal basis to all candidates and that the candidate knowingly and voluntarily waived participation in the orientation and practice sessions. Orientations and practice sessions are designed to give each candidate identical information regarding the test so that each will have the maximum probability for success. During the classroom orientation, candidates are shown the CPAT orientation video and are given the CPAT Candidate Preparation Guide. The orientation and practice sessions provide an equal and full opportunity for each candidate to view the test events, talk with qualified professionals and instructors and physically examine and use test equipment, tools, and props in a controlled and consistent setting. Candidates are directed to familiarize themselves with all elements of the test. Further information regarding the orientation and practice sessions may be obtained from your fire department.
