HOW TO USE... Pocket Guide

4 ZOMACTON [somatropin (rdna origin)] for Injection Supplies ZOMACTON 5 mg (powder) Preparation syringe Syringe and injection needle Diluent (liquid) Diluent (liquid) contains bacteriostatic 0.9% sodium chloride injection, USP (normal saline), 0.9% benzyl alcohol as a preservative. Benzyl alcohol, a component used to reconstitute the ZOMACTON 5-mg vial, should not be used with newborns. Benzyl alcohol has been associated with serious adverse events and death. When administering ZOMACTON 5 mg to newborns, it should be reconstituted with sterile normal saline. Benzyl alcohol should not be used in patients with a known sensitivity. Dry gauze or adhesive bandages 1 Prepare ZOMACTON 5 mg is not ready for immediate use You must first mix (or reconstitute) the dry powder with the liquid included with your medication Mix the amount of liquid your physician has prescribed (between 1 ml and 5 ml) with your medication Please note that the liquids are different for the 5-mg and 10-mg vials* do NOT use the 10-mg liquid with 5-mg ZOMACTON or vice versa Preparing the 5-mg liquid for injection Place the supplies you will need on a clean, flat surface in a well-lit area Wash your hands thoroughly with soap and water Remove the hard plastic cap from the vial of liquid; do not remove the rubber stopper. Clean the rubber stopper with an alcohol swab; after cleaning, do not touch the rubber stopper Remove the needle cap from the preparation syringe; do not throw away the needle cap Pull back on the plunger of the preparation syringe and stop when the amount of air is equal to the amount of liquid your physician has prescribed Alcohol swabs Sharps disposal container Slowly insert the needle of the preparation syringe into the center of the clean rubber stopper and into the vial of liquid Gently push down on the plunger and release the air into the vial * 5 mg contains benzyl alcohol as a preservative and 10 mg contains metacresol as a preservative.

4 1 Prepare (Continued) Turn the vial upside down, making sure the syringe stays in the vial. The tip of the needle should be below the surface of the liquid Pull back on the plunger until the correct amount of liquid your physician has prescribed fills the syringe Remove the needle from the vial and recap the needle Diluting ZOMACTON [somatropin (rdna origin)] for Injection Only use the liquid that comes with the 5-mg ZOMACTON to mix Remove the hard plastic cap of the ZOMACTON vial Clean the top of the vial with an alcohol swab Remove the needle cap from the syringe filled with liquid and insert the needle into the center of the rubber stopper on the vial Mixing ZOMACTON Hold the ZOMACTON vial between your hands and gently roll it (never shake it) until the mixture is clear. This may take a few seconds Do not inject the medication if it is cloudy or has particles If air bubbles appear, let the mixture sit until they disappear Write the date you mixed the growth hormone on the vial label. The 5-mg vial must be used within 14 days Store your mixed growth hormone and all unopened vials of growth hormone in the refrigerator at 36 F to 46 F (2 C to 8 C). Do not freeze Slowly inject the liquid against the side of the vial to avoid creating air bubbles. Do not squirt the liquid directly onto the powder When all the liquid is in the vial, remove the needle Recap the needle and throw away the syringe in a sharps disposal container

2 Site Selection ZOMACTON [somatropin (rdna origin)] for Injection can be injected into any of the following areas of the body (these sites should be rotated): 3 Inject Wash hands thoroughly with soap and water Check that the ZOMACTON vial is clear and the date of mixing is within 14 days Stomach (except the navel) Back of the upper arms Clean the top of the growth hormone vial with an alcohol swab. Do not touch the rubber stopper after cleaning Remove the needle cap from the injection syringe Upper thighs (child can sit with feet flat on the floor or lie down) Upper buttocks (child should lie face down on a flat surface with toes pointing inward) Slowly insert the needle into the center of the rubber stopper and into ZOMACTON vial Contact your child s doctor if you notice any of the following: Lump, bruise, or redness at the injection site that doesn t go away Any sign of infection at the injection site (pus, redness, heat, or persistent pain) Severe, sharp pain or ache at injection site that does not go away Rash at injection site Keeping the tip of the needle below the surface of the medication, turn the vial upside down - Gently pull back on the plunger until your prescribed amount of medication fills the syringe. (If you do not have enough medication for a full dose, reconstitute a new vial to make up the difference)

3 Inject (Continued) With the needle still in the upside down vial, gently tap the syringe to loosen any air bubbles - Remove the needle from the vial and carefully replace the needle cap until ready to inject Thoroughly clean the injection site with an alcohol swab - Check that the correct dose is in the syringe - Remove the needle cap and hold the syringe the way you hold a pencil - With your free hand, gently pinch your child s skin around the injection site between your fingers Insert the needle with a quick dart-like motion into the tissue beneath the skin s surface at a 45 to 90 angle to reduce discomfort - Holding the syringe in place, pull back (if there is blood in the syringe, it means you ve entered a blood vessel. DO NOT INJECT ZOMACTON [somatropin (rdna origin)] for Injection; withdraw the needle, discard all supplies, and go back to step 1. Choose and clean a new injection site) - If no blood appears, slowly push the plunger until the syringe is empty - Quickly pull the needle straight out and apply pressure to the site of injection with a sterile gauze pad. Throw away the needle and syringe in your sharps disposable container - Do not share your syringes, needles, or vials with anyone else. You may give them or get an infection from them 4 1 Store/Discard Write the date on the vial label - The 5-mg vial must be used within 14 days Store the mixed ZOMACTON and all unopened vials in the refrigerator at 36 F to 46 F (2 C to 8 C). Do not freeze For safety reasons, it is important to properly store your medication and correctly dispose of used needles and syringes Remember, needles can only be used once; do not try to recap the needle Dispose of needles, syringes, and vials in a sharps disposal container; never dispose of needles, syringes, and vials in your household trash Keep your sharps disposal container out of the reach of children When your sharps disposal container is almost full, you will need to follow your community guidelines for the right way to dispose of your sharps disposal container. There may be state or local laws about how you should throw away used needles and syringes. For more information about safe sharps disposal, and for specific information about sharps disposal in the state that you live in, go to the FDA s website at http://www.fda.gov/safesharpsdisposal
