Provider Notice To: From: PerformCare Provider Network Scott Daubert PhD, VP Provider Network & Account Management Date: April 23, 2015 Subject: AD 15 105 ICD-10-CM Frequently Asked Questions The Centers for Medicaid and Medicare Services (CMS) has mandated that all health organizations be compliant with ICD-10-CM by October 1, 2015. Q. What is PerformCare currently doing to prepare for the ICD-10-CM transition? A. PerformCare began assessing the impact in 2009 and has participated with our parent company, AmeriHealth Caritas, to complete our impact assessment. PerformCare has a dedicated team working on the ICD-10-CM transition requirements. The team worked with each PerformCare business area to complete an assessment to identify the impacts the ICD-10-CM transition has on the organization. Upgrades for ICD-10-CM have already started and many have been completed for a number of key systems impacted by ICD-10-CM. Q. When will PerformCare be ICD-10-CM compliant? A. PerformCare is on track to be ICD-10-CM compliant by October 1, 2015. Q. Will PerformCare accept ICD-10-CM codes prior to October 1, 2015? A. No. ICD-10-CM codes should not be used prior to October 1, 2015. Q. Will PerformCare conduct testing with providers using ICD-10-CM codes? A. Providers are encouraged to test with their 837 vendor or clearinghouse. Further targeted communication regarding testing will be sent to those providers who directly submit electronic claims to PerformCare. Q. Will PerformCare accept ICD-9 codes after October 1, 2015, for dates of service prior to October 1, 2015? A. Yes. PerformCare will be able to process both ICD-9-CM and ICD-10-CM codes. ICD-9 codes should only be submitted on claims with dates of service prior to October 1, 2015. Current regulations require the use of ICD-9 codes for dates of service prior to October 1, 2015. All outpatient and professional claims with dates of service on or after October 1, 2015 must contain ICD-10-CM diagnosis codes. To avoid delays or corrections, PerformCare is requiring that professional claims be split. A single professional claim should not contain both dates of service before October 1, 2015 (with ICD-9 codes) and dates of service on or after October 1, 2015 (with ICD-10 codes). This applies to both paper and electronic claims. Inpatient claims with discharge dates on or after October 1, 2015, must be coded in ICD-10-CM. However, PerformCare is requiring all facilities submitting Institutional claims to provide an interim bill that ends with dates of service September 30, 2015. To avoid delays or corrections, PerformCare is requiring that institutional claims be split. A single inpatient or other facility claim should not contain both dates of service before October 1, 2015 (with ICD-9 codes) and dates of service on or after October 1, 2015 (with ICD-10 codes). Q. Will PerformCare process using ICD-9-CM and ICD-10-CM, or will maps be used? A. PerformCare will process using the code that is applicable to the date of service. PerformCare will not use a map or crosswalk for processing claims. Our plan is to be able to accept both ICD-9 and ICD-10 at the same time within the same system and manage clinical and claims transactions in either format. This will allow us to accept ICD-9 codes after October 1, 2015 for dates of service prior to October 1, 2015. Our approach is as follows: Utilize a dual-processing approach so that we can process both ICD-9 and ICD-10 through our core systems without the use of a crosswalk map Configure the Claims module to accommodate either ICD-9 or ICD-10 codes for any given claim (the same code set must be used for a single claim). The diagnosis code will match to the one submitted and the system will know if it is ICD-9 or ICD-10 and store it. For this reason, providers will have to split claims as a single claim cannot contain multiple claim lines with dates of service both prior to and beyond October 1, 2015. ICD-9 and ICD-10 codes cannot be mixed on the same claim. Q. Will PerformCare have a contingency plan in the event that claims do not process correctly following the transition? A. Business continuity for our providers and the prompt processing of claims is of the utmost importance for PerformCare. Steps are being planned to mitigate any potential delays that might occur in payment processing as a result of the transition to ICD-10-CM. The first step for providers in resolving any claims questions / issues would be to call the PerformCare Provider Line at 888-700- 7370. Q. How does the ICD-10-CM implementation impact providers that file paper claims or claims through ProviderConnect? A. The method used to submit the claim has no impact on the code set contained on the claim. The ICD- 10-CM code set must be used on all claims with dates of service on or after October 1, 2015, and inpatient claims with a date of discharge on or after October 1, 2015. ICD-10-CM must be used, based on these criteria, as described in more detail above. Q. What is the relationship of ICD-10-CM to DSM-5? Can clinicians continue the current practice and use the DSM-IV and DSM-5 diagnostic criteria? A. Many clinicians have historically used the DSM-IV in diagnosing mental s. As of May 19, 2013, the DSM-5 was released by the American Psychiatric Association. While neither the DSM-IV nor the DSM-5 are HIPAA adopted codes sets, the introductory material to the DSM-IV and DSM-5 indicates that the DSM-IV and DSM-5 are compatible with the ICD-9-CM diagnosis codes and crosswalks are provided. The updated DSM-5 codes are crosswalked within the APA publication to both ICD-9-CM and ICD-10-CM. However, full compatibility and a published crosswalk does not exist between DSM-IV and ICD-10-CM. As of October 1, 2015, the ICD-10-CM code set is the HIPAA adopted standard and is required for reporting diagnosis for dates of service on or after October 1, 2015. Therefore, all clinicians must switch to using DSM-5 by October 1, 2015 at the latest. We strongly recommend that all clinicians who have not switched to DSM-5 do so as soon as possible. 2 AD 15 105 ICD-10-CM-CM Frequently Asked Questions

Q. Will PerformCare change its policies with ICD-10-CM codes prior to October 1, 2015? A. When a substantive policy change is required, PerformCare policies and procedures will be updated to include the references to both the ICD-10-CM code set and DSM-5 where appropriate. Q. Will PerformCare require the execution of new contracts or contract addendum? A. Contracts are currently being reviewed to determine if any updates are required to accommodate ICD- 10-CM. However, new contracts or contract addendums are not anticipated at this time. Q. How will PerformCare communicate with providers about the ICD-10-CM transition? A. PerformCare will communicate with providers about the ICD-10-CM transition through its current channels of communication with providers including: Account Executive meetings, Provider Notices, e-mail distribution lists, the PerformCare website, and additional upcoming webinars and trainings. The intent is to map and provide an easy-to-use desk reference to the DSM-5 codes used by behavioral health providers to their approximate ICD-10 and ICD-9 code. Using DSM-5 TM as the source may result in some variances when compared to the official General Equivalency Mappings (GEMs), which are jointly maintained by the Centers for Medicare and Medicaid Services (CMS) and the Centers for Disease Control and Prevention (CDC). The GEMs are a comprehensive bi-directional translation dictionary to translate information system data for all ICD-10 to ICD-9 codes, and vice versa, without consideration of DSM-5.
